The Ying and Wei (Nutritive and Defensive Qi) of a person are originally deficient, and are subsequently attacked by pathogenic heat. After treatments such as sweating, vomiting, or purging, the meridians and collaterals are damaged, Yin and Yang are exhausted and severed. Although the pathogenic factors have dispersed, the True Qi is still scarce, the Five Zang Organs remain deficient, the function of the Spleen and Stomach (Gu Shen, Grain Spirit) has not recovered, and there is a lack of Jin Ye (Body Fluids) to nourish the body. Therefore, emaciation and weakness occur, leading to various diseases.
This is caused by Kidney deficiency. The Kidney Qi connects with the Yin organs/genitals. Now that the Kidneys are injured by pathogenic heat, toxic heat flows downward, causing swelling of the genitals.
The so-called "Shi Qi" (Seasonal Pathogen) diseases all result from irregular seasonal climates and improper regulation of warmth and cold. People contract abnormal pathogenic qi and become ill. These diseases are often contagious; therefore, one should take medicine in advance or adopt methods to prevent them.
Re Yi Bing (Heat Disease) belongs to the category of Shang Han (Cold Damage). Cold contracted in winter transforms into Wen Bing (Warm Disease) in spring, and into Shu Bing (Summerheat Disease) in summer. Shu Bing involves heat that is more severe than Wen Bing.
In Liver Re Yi Bing, urine becomes yellow first, accompanied by abdominal pain, lethargy, and fever. When Zheng Qi (Upright Qi) struggles against Xie Qi (Pathogenic Qi), delirium and惊厥 (convulsions) occur, along with distension and pain in the hypochondrium, restlessness of the hands and feet, and inability to lie down. The condition worsens on Geng-Xin days, improves with profuse sweating on Jia-Yi days, and death occurs on Geng-Xin days if Qi reverses.
In Heart Re Yi Bing, one first feels unhappy, followed by fever after several days. During the struggle between Zheng Qi and Xie Qi, sudden heart pain, vexation, frequent vomiting, headache, facial redness, and absence of sweat occur. The condition worsens on Ren-Gui days, improves with profuse sweating on Bing-Ding days, and death occurs on Ren-Gui days if Qi reverses.
In Spleen Re Yi Bing, heaviness of the head and cheek pain occur first, followed by mental vexation, desire to vomit, and fever. During the struggle, lower back pain, abdominal fullness, diarrhea, and pain in both mandibles occur. The condition worsens on Jia-Yi days, improves with profuse sweating on Wu-Ji days, and death occurs on Jia-Yi days if Qi reverses.
In Lung Re Yi Bing, one first experiences goosebumps and aversion to wind, yellow tongue coating, and fever. During the struggle, wheezing and coughing occur, bi-syndrome (painful obstruction) moves through the chest to the back, difficulty in deep breathing, mild headache, and sweating with chills occur. The condition worsens on Bing-Ding days, improves with profuse sweating on Geng-Xin days, and death occurs on Bing-Ding days if Qi reverses.
In Kidney Re Yi Bing, lower back pain and soreness of the calves occur first, followed by bitter thirst with frequent drinking, and fever. During the struggle, neck pain and stiffness occur, calves feel cold and sore, the soles of the feet feel hot, and there is an unwillingness to speak. The neck pain may appear moist and fluctuating. The condition worsens on Wu-Ji days, improves with profuse sweating on Ren-Gui days, and death occurs on Wu-Ji days if Qi reverses.
In Liver Re Yi Bing, the left cheek turns red first. In Heart Re Yi Bing, the forehead turns red first. In Spleen Re Yi Bing, the nose turns red first. In Lung Re Yi Bing, the right cheek turns red first. In Kidney Re Yi Bing, the chin turns red first. For any disease that has not yet manifested, if redness is observed on specific facial areas, acupuncture should be applied there. This is called "Zhi Wei Bing" (Treating Disease Before It Arises).
There are nine situations in Re Yi Bing where acupuncture is contraindicated: 1. No sweat, redness of the zygomatic arch, and hiccups indicate death; 2. Diarrhea with severe abdominal fullness indicates death; 3. Impaired vision with persistent heat indicates death; 4. Fever with abdominal fullness in the elderly and infants indicates death; 5. No sweat with hematemesis indicates death; 6. Ulceration of the root of the tongue with persistent heat indicates death; 7. Hemoptysis, epistaxis, no sweat, or sweat that does not reach the feet indicates death; 8. Marrow heat indicates death; 9. Fever with Jing Bing (Spasm) indicates death. None of these nine conditions should be treated with acupuncture.
If Re Yi Bing has produced sweat but the pulse remains rapid and restless, this represents the extreme of Yin pulses and indicates death; if the pulse becomes calm after sweating, it indicates survival. If the pulse is full and restless without sweat, this represents the extreme of Yang pulses and indicates death; if the pulse becomes calm after sweating, it indicates survival. After seven or eight days of Re Yi Bing, if the pulse is minute, the patient urinates blood, and the mouth is dry, death occurs within one and a half days; if a Dai Mai (Knotted/Intermittent Pulse) appears, death occurs within one day. If Re Yi Bing has produced sweat but the pulse remains rapid and restless with wheezing, and fever recurs, do not use acupuncture; severe wheezing indicates death. After seven or eight days of Re Yi Bing, if the pulse is not restless or rapid, and sweat occurs within three days, survival is likely; if no sweat occurs after three days, death occurs on the fourth day. Those who have never sweated should not be treated with acupuncture.
When diagnosing Re Yi Bing after seven or eight days, if the pulse is minute, the mouth is dry, a Dai Mai appears, and the tongue coating is scorched black, death ensues. If the pulse is not rapid and there is no wheezing, aphonia (loss of voice) will occur; if warm sweat fails to appear within three days thereafter, death ensues. If Re Yi Bing has produced sweat but high fever persists, it is untreatable and fatal. If Re Yi Bing has produced sweat and the pulse is calm, survival is indicated; if the pulse is restless, it is difficult to treat. If the pulse remains rapid and full, representing the extreme of Yin Qi, death ensues. Abdominal fullness with persistent wheezing and unremitting heat indicates death. Profuse sweating with a deficienct and small pulse indicates survival; a firm and strong pulse indicates death.
The *Yang Sheng Fang* (Prescriptions for Nourishing Life) states: Do not eat aged pickled vegetables in the third month, or you will surely contract Heat Disease.
