The Heart manifests in the tongue, which is nourished by Blood. Laborious injury causing Blood deficiency allows Heat pathogens to take advantage of this weakness. Furthermore, the large collateral vessel of the Spleen emerges from beneath the tongue. If there is Heat in the Heart and Spleen, it causes swelling of the tongue.
This arises from the insufficiency of Qi in the Five Viscera. Blood circulates smoothly to nourish the Five Viscera, and the Qi of the Five Viscera moistens and nourishes the skin and muscles. Internal injury due to consumptive disease leads to a decline in Qi and Blood, which fail to externally nourish the skin, resulting in peeling or scaling of the skin.
The Kidney governs Water, and the Spleen governs Earth. If the Spleen is deficient, it cannot control Water; if the Kidney is deficient, Water Qi overflows and scatters into the skin, causing general edema. If both Qi and Blood are stagnant, this often transforms into edema diseases.
This is due to deficiency in both Yin and Yang, with Yin Qi being relatively scarce while Yang Qi suddenly becomes excessive. Heat then invades the Heart, causing irritability and oppression.
Consumptive disease reduces body fluids due to insufficiency of Kidney Qi. The fluid of the Kidney is saliva. When Heat generates in the Upper Jiao and rushes upward to the throat, the saliva becomes condensed.
Consumptive disease often injures the Kidney. The Kidney governs saliva, the Liver stores Blood, and the Stomach is the Sea of Water and Grain. When Stomach Qi rebels upward, vomiting occurs; when the Kidney and Liver are injured, one may expectorate blood due to retching.
This represents internal injury damaging the Zang organs. The Liver stores Blood, and the Lung governs Qi. Injury to Qi and Blood through overwork causes Qi to rebel upward, leading to vomiting; if the Liver is injured, Blood follows the vomitus out. Mild injury results in expectorating blood, while severe injury results in hematemesis (vomiting blood).
The Lung governs Qi and opens into the nose, while the Liver stores Blood. Blood and Qi travel together, jointly nourishing the Zang and Fu organs. In persons suffering from laborious injury, Blood deficiency and rebellious Qi cause epistaxis (nosebleed). "Nu" refers to bleeding from the nose.
Injury to the Zang and Fu organs through overwork constitutes an internal collapse-type disease. Blood and Qi travel together, externally nourishing muscles and internally nourishing the viscera. When the viscera are damaged, Blood flows recklessly. If Qi rebels in the chest and diaphragm, hematemesis occurs. If blood flows into the intestines and stomach, and the intestines are deficient, hematochezia (blood in stool) occurs. If the intestines are deficient and Qi rebels again, both hematemesis and hematochezia occur. Those with exterior deficiency may experience hematohidrosis (bleeding sweat). All these result from extreme damage and deficiency.
Deficiency of the Large Intestine leads to diarrhea, and rebellious Stomach Qi leads to vomiting. In consumptive disease, if there is intestinal deficiency and Stomach rebellion, one experiences both vomiting and diarrhea.
Zang and Fu organ deficiency and damage are caused by Wind-Cold pathogens. The Stomach is the Sea of Water and Grain; Cold in the Stomach and deficiency in the intestines lead to dysentery.
