First, inspect the eyes: In cases of internal injury with accumulated blood stasis, the sclera (white part of the eye) must exhibit red veins. More veins indicate more stasis; fewer veins indicate less stasis. If the eyeballs move with spirit and vitality, it is treatable; otherwise, it is difficult to treat.
Second, inspect the fingernails: Press the patient's nail with your thumb. If the color returns immediately upon release, it is treatable. If the return is slow, or if the color turns blackish-purple or purple, it is untreatable.
Third, inspect the genitalia (Yang Dao). If there is no retraction and urination is possible, it is treatable; otherwise, it is difficult to treat.
Fourth, inspect the toes: The method is the same as inspecting the fingernails.
Fifth, inspect the soles of the feet: Redness indicates easy treatment; yellowing indicates difficult treatment; involvement of all five colors (abnormal appearances of complexion, tongue, etc.) indicates untreatability.
Sixth, inspect the pulse: If the Stomach Qi is harmonious, it is easy to treat. If the six pulses are floating and rapid with concurrent exterior pathogenic factors, it is difficult to treat. If the six pulses are faint and nearly absent, indicating the exhaustion of Stomach Qi, it is untreatable.
In treating severe cases of traumatic injuries, do not administer medication hastily. If the patient cannot open their mouth, first chew a dried plum (Wumei) thoroughly and rub it on the gums, or blow powder of Gleditsia Sinensis Fruit (Zaojiao) into the nose to induce sneezing, which will open the mouth. Then, prepare juice from Chinese Chive Root (Jiucai Gen), stewed hot, mixed with virgin urine (Tongbian) for oral administration. If the patient cannot swallow, it is difficult to treat. If administered successfully, blood stasis will be vomited out. Based on the severity of the injury, first administer Duoming Shendan (Life-Rescuing Divine Pill), followed by Shufeng Liqi Tang (Wind-Dispersing and Qi-Regulating Decoction), and externally apply Diaoshang Dan (Hanging Injury Pill). For urinary retention, use moxibustion or fumigation/washing methods. For open fractures, apply sealing medicine to protect the wound, then administer Jiegu Zijin Dan (Bone-Connecting Purple Gold Pill). For abdominal pain, qi-moving herbs are appropriate; after moving qi, assess the severity of symptoms and adjust the prescription accordingly.
Within seven days of being beaten, when Qi and Blood have not yet accumulated, only dispersing and blood-invigorating methods should be used. Within fourteen days, blood stasis accumulates in the chest and tends to move toward the Large Intestine, causing abdominal pain; thus, qi-moving medications are suitable.
Initially after trauma, there is inevitably Qi stagnation, possibly compounded by Wind-Cold invasion, risking Qi and Blood attacking the Heart; therefore, Huxin Dan (Heart-Protecting Pill) is suitable.
When inspecting nails, start with the middle finger. Blackening indicates severe injury; blackening of toenails also indicates severity. Red veins in the eyes or reddish-white sclera are ominous signs. A black facial complexion is extremely ominous. Retraction of the testes is even more ominous. Yellowing of the soles is also an ominous sign.
For severe trauma resulting in immobility, use pushing/manipulation techniques. Two assistants lift the patient's head four or five times, which should cause salivation and revival. If no vomiting occurs, mix five fen of Saponaria officinalis powder (Yazao) with wine and force-feed; vomiting will lead to awakening. For lockjaw, use the method of rubbing teeth with dried plums.
Regarding the direction of impact: Blows directed upward result in smooth Qi flow; horizontal blows cause Qi obstruction; downward thrusts cause Qi rebellion. Rebellion leads to blood stasis, making downward-thrust injuries particularly difficult to treat.
If the patient feels the injury is severe and medical help is delayed, drink two or three spoonfuls of hot virgin urine, or take five to seven fresh earthworms (Sheng Dilong), washed and mashed, taken with hot wine, while applying the dregs to the injury site. Seek urgent medical attention to ensure safety.
Unscrupulous doctors may initially use drastic purgatives (Jie Yao), such as raw Pinellia Tuber (Banxia) or Aconitum carmichaelii root (Caowu). This may cause facial swelling, eyelid edema, chills, and teeth grinding. Do not panic; these symptoms will subside naturally after three shichen (six hours). Saving lives is the urgent duty of a physician.
When using guiding herbs (Yin Jing Zhi Yao): Use Ligusticum Chuanxiong Rhizome (Chuanxiong) for the upper body; Cinnamon Twig (Guizhi) for the hands; Angelica Dahurica Root (Baizhi) for the head; White Peony Root (Baishao) for the chest and abdomen; Phellodendron Bark (Huangbai) below the navel; Green Tangerine Peel (Qingpi) for the left hypochondrium; Bitter Orange (Zhike) for the right hypochondrium; Eucommia Bark (Duzhong) for the waist; Achyranthes Bidentata Root (Niuxi) for the lower body; Malus Pumila Fruit (Mugua) for the feet; Notopterygium Rhizome and Root (Qianghuo) and Angelica Sinensis Root (Danggui) for the whole body. Regardless of the type of trauma, these guidelines must be consulted.
Liver injury: Complexion is green with purplish-red hues, eyes are red and feverish. Prognosis is death within seven days. First administer Liushang Yin (Flowing Injury Infusion), then Xiao Xuming Tang (Minor Life-Sustaining Decoction), and finally Zhonghe Wan (Harmonizing Pill).
Heart injury: Complexion is blue, shortness of breath, hemoptysis, severe pain on respiration, immobility. Prognosis is death within seven days. First administer Liushang Yin, then Xuming Tang (Life-Sustaining Decoction), and finally Zhonghe Wan.
Stomach/Abdomen injury: Paroxysmal epigastric pain, fever, distended abdomen like a drum, dyspnea, inability to eat, closed eyes, blackened nose and face. Prognosis is death within seven days. First administer Da Xuming Tang (Major Life-Sustaining Decoction), then Zhonghe Wan. Downward-thrust injuries are untreatable.
Kidney injury: Immediate deafness, black floating luster on the temples, expression resembling weeping. Prognosis is death within half a month. First administer Shengxue Yin (Blood-Generating Infusion), then Liushang Yin, and finally Zhonghe Wan.
Lower Abdomen injury: Facial swelling, dyspnea, intermittent pain, vomiting sour fluid. Prognosis is death within three days. First administer Xuming Tang, then Zhonghe Wan.
Large Abdomen injury: Urgent and obstructed defecation with blood, flushed face, Qi stagnation. Prognosis is death within half a month. First administer Liushang Yin, then Xuming Tang, and finally Zhonghe Wan.
Bladder injury: Painful and obstructed urination, frequent sensation of bladder distension, fever. Prognosis is death within five days. First administer Xuming Tang, then Xingqi Huoxue Tang (Qi-Moving and Blood-Invigorating Decoction).
Genital injury (scrotum/vagina): Hematuria. Prognosis is death within three days. First administer Huxin Yangyuan Tang (Heart-Protecting and Origin-Nourishing Decoction), then Da Xuming Tang.
Chest and Back injury: Pale complexion, emaciation, reduced appetite, fever, cough. Prognosis is death within half a month. First administer Liuqi Yin (Qi-Flowing Infusion), then Zhonghe Wan.
Qihai (Sea of Qi) injury: Dyspnea, severe pain, emaciation, night sweats, reduced appetite, painful swelling and restlessness. Prognosis is death within one month. First administer Liushang Yin, then Zhonghe Wan.
Xuehai (Sea of Blood) injury: Uncontrolled bleeding, frequent spitting of blood, board-like stiffness and pain in the chest. First administer Huoqi Tang (Qi-Activating Decoction), then Liushang Yin, followed by medicinal wine. If ineffective, death is certain within one month.
Hypochondriac injury: Dyspnea, severe pain, stabbing pain when lying down, pale complexion, Qi deficiency. First administer Huoxue Shunqi Tang (Blood-Invigorating and Qi-Regulating Decoction), then Xuming Tang. Prognosis is death within three days.
