The true and false nature of Jue syndrome (Cold Extremities Syndrome) must be clearly distinguished; otherwise, one may immediately die under the hands of a physician. This is because Jue syndrome often arises suddenly, with disaster striking in haste. If the condition is recognized clearly, the treatment will naturally be decisive.
Generally, there are more cases of heat-type Jue than cold-type Jue. However, heat diseases often present deceptive false signs to mislead, while cold conditions may also exhibit misleading heat-like symptoms. Although heat syndromes are numerous and difficult to enumerate exhaustively, the *Neijing* (Classic of Internal Medicine) has thoroughly expounded upon heat Jue but did not discuss its true and false distinctions. How then can I differentiate each case? If one grasps the key principle, the distinction between true and false can be determined in a single statement, making it unnecessary to engage in tedious discussions on individual symptoms.
Regarding the appearance of false cold in heat Jue, it can be summarized by the four characters "feeling cold and fearing chill." When discussing various types of Jue, one need only distinguish whether the tongue coating is dry or slippery: slippery indicates cold, while dry indicates heat. A tongue that is dry and red is particularly indicative of a heat syndrome; even if the tongue is dry and white, it may still not exclude a heat syndrome. I have established a formula named Fuwei Zhuanjue Tang (Rescue from Peril Decoction to Reverse Jue), which is miraculously effective for treating heat Jue. This formula solely soothes the Liver Wood to drain fire from the Liver. Once the Liver qi is harmonized and fire is eliminated, Jue affecting all meridians will be pacified. Why bother asking which meridian is involved and seeking specific formulas, which would be superfluous?
As for cold Jue, the formula is named Wenjing Zhuanjue Tang (Warm the Meridians Decoction to Reverse Jue). This formula strengthens the Spleen to dispel cold. Once the cold is removed, the Jue resolves spontaneously. If you observe a patient with a slippery tongue, vomiting, facial flushing (Daiyang syndrome/Floating Yang), and ice-cold feet, this is cold Jue. This is what is meant by false cold presenting as apparent true heat to deceive observers. Administration of this formula is invariably effective. In cases where cold is extreme and causes rejection of yang (Ge Yang), adding human urine and bovine gallbladder bile is advisable.
