Summerheat disorders are divided into heatstroke (Zhongre, excessive heat) and miasma-stroke (Zhongshu, summerheat stroke). Generally speaking, miasma-stroke is mostly Yin syndrome, while heatstroke is mostly Yang syndrome. How to distinguish them? Those suffering from miasma-stroke often reside in high halls and large mansions. Although the summer heat outside is obviously hot, why is it said to be a disease of the Yin meridians? This is because after the deficiency of Yin Qi, Yang pathogenic factors invade; therefore, it should still be treated as a Yin syndrome. The symptoms inevitably include abdominal pain, dizziness, vomiting, and diarrhea occurring simultaneously. In severe cases, opisthotonos (rigid arching of the back) and cholera-like vomiting and diarrhea may occur. The treatment method should focus on strengthening the Spleen, assisted by herbs that dispel summerheat. This is appropriate. The formula used is Queshu Xiandan (Cooling Summerheat Immortal Elixir). If opisthotonos occurs, add Rougui (Cortex Cinnamomi, 5 fen); otherwise, do not add it. This is the method for treating Yin syndrome. As for the Yang syndrome of heatstroke, what is its nature? It usually affects porters and vendors who carry heavy loads under the scorching sun like a fire wheel, sweating profusely like rain, suddenly struck by heat. At this time, resolving summerheat should be prioritized, followed by promoting urination; one must not tonify Qi. The formula used is Huare Xiandan (Transforming Heat Immortal Elixir). The brilliance of this formula lies in the use of Qinghao (Herba Artemisiae Annuae), as Qinghao is most effective at removing summerheat. Once summerheat is removed, promoting bladder function allows summerheat to exit via urine, leading to recovery after one dose. This is the method for treating heatstroke within Yang syndrome.

Wheezing (Chuanzheng) requires differentiation. Wheezing that arises suddenly is due to contracting external Wind pathogenic factors. It necessarily involves rapid breathing and inability to breathe smoothly, with sounds resembling snoring. The shoulders must lift upward, and the back feels cold; even with warming applications, no heat is visible. Phlegm is expectorated like a gushing spring, and the patient cannot lie down. This is Yang syndrome wheezing. Taking Sushu Yin (Ginseng and Perilla Decoction) for one dose alleviates symptoms, and two doses cure it. Alternatively, Xiaochaihu Tang (Minor Bupleurum Decoction) can be modified and used, which will also yield immediate results without fail. Therefore, no new methods need to be established. However, Yin syndrome wheezing is the most terrifying and difficult to treat. Its symptoms also present as wheezing, and the patient cannot lie down. Symptoms decrease slightly after eating but worsen with bloating if eaten too much. Coughing persists and intensifies at night. This type of wheezing appears like wheezing but is not true wheezing; it presents as excess Qi but is actually deficiency of Qi. This is because Kidney Qi is severely deficient, threatening to detach from its root. Only through this wisp of Yuan Yang (Original Qi) around the navel, during the moment when life is about to end but has not yet ceased, can vitality be restored. The treatment method should greatly tonify Qi and vigorously tonify Yin in the Kidney, allowing Wood and Fire to interact harmoniously (Mu Huo Ji Ji) before success can be achieved. Otherwise, Qi will cut off, leading to rapid death. The formula used is Huijue Shenqi Tang (Reviving Extinction Divine Energy Decoction). One dose alleviates wheezing, two doses stabilize it, and four consecutive doses show improvement. Only then can Rougui (Cortex Cinnamomi) and Fuzi (Radix Aconiti Lateralis Preparata) be added in small amounts. They must not be used excessively to avoid depleting Zheng Qi (Upright Qi). For instance, Qi cannot be restored from non-existence without Renshen (Radix Ginseng). Kidney deficiency cannot be remedied without Shudi (Radix Rehmanniae Preparata) and Shanyao (Rhizoma Dioscoreae). However, the root cause of Kidney deficiency ultimately lies in Lung Qi deficiency. Since Lung Qi is deficient, Kidney Water cannot be generated quickly. Therefore, one must assist in boosting Lung Qi so that Metal generates Water. With the harmonious relationship between mother (Metal/Lung) and child (Water/Kidney), Lung Qi naturally descends and Kidney Qi ascends. How could there be an invasion of wheezing disorder then?
