此即白虎汤变方,用石膏、知母以泻其阳明之火邪;用柴胡、栀子以断其少阳之路径;
这就是白虎汤的变方,用石膏、知母来泻除阳明的火邪;用柴胡、栀子来阻断少阳的路径;
In winter, contracting cold pathogen (Shanghan), presenting with fever and headache, followed by sweating and thirst. People assume it is a Taiyang disease (disease of the Taiyang channel). Little do they know that the pathogen has already moved from Taiyang to Yangming. If one merely uses Gan Ge Tang (Pueraria Decoction) to treat Yangming, the headache will not be resolved; if one merely uses Mahuang Tang (Ephedra Decoction) to treat Taiyang, the sweating will not stop, nor will the thirst be relieved. This inevitably leads to numerous complications, turning mild cases into severe ones. The appropriate method is to primarily treat Yangming while concurrently treating Shaoyang. Why? Because the pathogen entering Yangming leaves only scattered residual evil in Taiyang; treating Taiyang directly would instead injure Taiyang. Therefore, Taiyang need not be treated; rather, Yangming should be treated primarily. Yangming is the fu-organ abundant in qi and blood. When pathogen enters here, it can wreak havoc, carrying the qi and blood of the fu to create fierce inflammatory heat, which is common. Thus, large doses of cooling herbs are required to dispel this violent pathogen. Prescription:
Shi Gao (Gypsum Fibrosum, 1 liang), Zhi Mu (Anemarrhenae Rhizoma, 2 qian), Mai Dong (Ophiopogonis Radix, 2 liang), Zhu Ye (Bambusae Folium, 200 leaves), Fu Ling (Poria, 3 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma, 1 qian), Ren Shen (Ginseng Radix, 3 qian), Chai Hu (Bupleuri Radix, 1 qian), Zhi Zi (Gardeniae Fructus, 1 qian). Decoct in water and take orally. After one dose, headache resolves; after two doses, body heat subsides, sweating stops, and thirst ceases.
This is a modified Bai Hu Tang (White Tiger Decoction). Shi Gao and Zhi Mu clear the fire pathogen of Yangming; Chai Hu and Zhi Zi block the pathway of Shaoyang; Mai Dong clears and tonifies Lung Metal qi, preventing fire pathogen from forcing upward; Fu Ling guides fire downward to the Bladder for excretion via urination, causing residual Taiyang pathogen to exit externally as well. Ren Shen, Gan Cao, and Zhu Ye merely harmonize the zang-fu organs, embodying the principle of simultaneous attack and supplementation.
Some may fear the previous prescription is too heavy; Qing Su Tang (Clear and Purify Decoction) can also be used, recorded here for selection.
Shi Gao (5 qian), Zhi Mu (1 qian), Mai Dong (1 liang), Gan Cao, Ren Shen, Chai Hu, Zhi Zi (each 1 qian), Du Huo (Angelicae Pubescentis Radix), Ban Xia (Pinelliae Rhizoma) (each 5 fen). Decoct in water and take orally.
In winter, contracting cold pathogen, presenting with fever, bitter taste in mouth, headache, hunger but lack of appetite, and intermittent abdominal pain. People assume it is a Taiyang symptom, little knowing it is a Shaoyang disease.
Cold pathogen always enters from Taiyang. Transmission from Taiyang to Yangming, then to Shaoyang follows the sequence of meridian transmission. Why does the pathogen enter Taiyang, skip Yangming, and go directly to Shaoyang? People attribute this to skipping meridians, but this is incorrect. Shaoyang is the Gallbladder meridian; Gallbladder belongs to Wood, and Wood most dislikes Metal. Lung belongs to Metal and governs skin and hair. When wind pathogen arrives, Lung Metal is affected first. Bullying the deficiency of Gallbladder Wood, Lung transfers the pathogen to Shaoyang. Hence, Taiyang symptoms often coexist with Shaoyang. Thus, this condition involves simultaneous infection of two meridians, not meridian transmission. Treatment seems to require addressing both meridians, yet this is not so; treating only Shaoyang allows Taiyang disease to heal spontaneously. Prescription:
Chai Hu (2 qian), Bai Shao (Paeoniae Radix Alba, 5 qian), Gan Cao (1 qian), Chen Pi (Citri Reticulatae Pericarpium, 1 qian), Huang Qin (Scutellariae Radix, 1 qian), Shen Qu (Massa Medicata Fermentata, 1 qian), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 3 qian), Fu Ling (3 qian). Decoct in water and take orally. After one dose, fever stops; after two doses, abdominal pain ceases, headache disappears, and bitter taste vanishes.
This formula is a modification of Xiao Yao San (Free and Easy Wanderer Powder). Since the disease lies between exterior and interior, Xiao Yao San disperses exterior-interior pathogens, leaving no room for Taiyang Bladder pathogen to remain alone. Moreover, the original formula contains Fu Ling and Bai Zhu to benefit the waist and umbilicus and unblock Bladder qi. I added only Shen Qu and Huang Qin to slightly resolve stomach fire and harmonize spleen qi, thereby eliminating all symptoms.
Shu Jing Tang (Unbind Meridians Decoction) is also excellent for this condition.
Bo He (Menthae Haplocalycis Herba, 2 qian), Bai Shao (5 qian), Gan Cao (8 fen), Huang Qin (2 fen), Bai Zhu (2 qian), Fu Ling (5 qian), Gui Zhi (Cinnamomi Ramulus, 3 fen). Decoct in water and take orally.
In winter, contracting cold pathogen, presenting with fever, thirst, delirium, and occasional cold limbs. People believe deep heat causes deep cold, suspecting Jueyin disease, little knowing it is Taiyin disease.
Taiyin belongs to Spleen Earth. Spleen and Yangming Stomach are exterior-interior pairs. Exterior heat implies interior heat; this is stomach pathogen transferring to Spleen meridian. This condition is critical and urgent. Humans rely on Spleen and Stomach as the foundation. If Spleen and Stomach are scorched by fire pathogen, will not Kidney Water dry up immediately? Treatment should urgently rescue Spleen and Stomach. However, rescuing Spleen intensifies Stomach fire, while rescuing Stomach causes Spleen Earth to collapse instantly. The nuance is difficult; one must quickly rescue the dryness of Kidney Water. Prescription:
Xuan Shen (Scrophulariae Radix, 3 liang), Gan Ju Hua (Chrysanthemi Flos, 1 liang), Shu Di Huang (Rehmanniae Radix Praeparata, 1 liang), Mai Dong (2 liang), Qian Shi (Nelumbinis Semen, 5 qian). Decoct in water and take orally.
This formula is named Jiu Ku Dan (Save the Withered Pill). Xuan Shen dissipates floating fire of Spleen and Stomach; Gan Ju eliminates stomach pathogen; Mai Dong moistens Lung fluid; assisting Shu Di to generate Kidney Water. Like torrential rain falling from heaven, parched earth is instantly nourished, defeating drought demons. To prevent excessive flooding, Qian Shi strengthens Earth qi, remaining a Kidney meridian herb, thus harmonizing Spleen and Kidney. It provides irrigation without invasion. One dose stabilizes delirium; two doses relieve thirst; three doses stop cold limbs and cool the body. Worldly people do not know this treatment; even Zhang Zhongjing did not discuss it. Chen Shiduo transmitted this miraculous method at the request of Tian Shi, calling it a life-saving pill for Shanghan.
Qing Tu San (Clear Earth Powder) is also effective for this condition.
Shi Gao (1 liang), Mai Dong (1 liang), Sheng Di Huang (Rehmanniae Radix, 1 liang), Gan Cao (1 qian), Jin Yin Hua (Lonicerae Japonicae Flos, 5 qian), Bai Zhu (3 qian). Decoct in water and take orally.
In winter, contracting cold pathogen, profuse sweating with unresolved heat, and abdominal pain intolerable to pressure. People think external pathogen is incomplete and internally knotted in abdomen, a Yang syndrome transforming into Yin. I disagree. Profuse sweating indicates pathogen resolving with sweat; there should be no pathogen left, why abdominal pain? This is total loss of Yang Qi and exhaustion of Yin, leaving abdomen without Yin nourishment, resembling internal knotting pain, actually an emergency of dual loss of Yin and Yang. Generally, pain relievable by pressure is deficiency; untouchable pain is excess. Why is this untouchable yet deficiency? Dual loss of Yin and Yang signifies imminent death. Even without pressure, pain is unbearable; pressing further damages intestines and stomach, increasing suffering, hence untouchable. For such cases, urgency permits no delay. Use Ji Jiao Yin Yang Tang (Urgently Rescue Yin and Yang Decoction):
Ren Shen (2 liang), Huang Qi (Astragali Radix, 3 liang), Dang Gui (Angelicae Sinensis Radix, 1 liang), Shu Di Huang (2 liang), Gan Cao (3 qian), Bai Zhu (2 liang). Decoct in water and take orally. One dose stops abdominal pain instantly, resolves body heat, and halts sweating.
This formula uses Ren Shen and Huang Qi to tonify Qi, returning Yang within Yin; Dang Gui and Shu Di tonify Blood, holding Yin within Yang; Bai Zhu and Gan Cao harmonize intestines and stomach, unblocking waist and umbilicus, guiding Yin and Yang to Qi Hai and Guan Yuan points. Thus, lost is not lost, severed is not severed. Mistaking this for Yang transforming to Yin and using purely warming agents like Rou Gui (Cinnamomi Cortex), Gan Jiang (Zingiberis Rhizoma), or Fu Zi (Aconiti Lateralis Radix Praeparata) might restore Yang momentarily, but without Yin to hold it, Yang returns only to be lost again.
Jiu Wang San (Save the Dying Powder) works easily for this condition.
Ren Shen, Dang Gui, Shu Di (each 1 liang), Gan Cao (2 qian), Fu Zi (1 slice). Decoct in water and take orally.
In winter, contracting cold pathogen, profuse sweating with resolved heat, slight abdominal pain, and inability to bend or extend the waist. People think pathogen remains in Kidney meridian, wanting to use pills with Fang Ji (Stephaniae Tetrandrae Radix), which is incorrect. This results from sweating causing Yang loss; Yang deficiency prevents Yin from assisting. Yin and Yang root each other. Excessive sweating leaves little Yang; Yin cannot guide Yang inward, leaving Yang homeless, wandering abdomen, causing pain due to lonely Yang without master. Kidney Yin, lacking Yang companion, dares not ascend via He Che road, causing waist stiffness. Treat with Yin Yang Tang (Guide Yang Decoction):
Du Zhong (Eucommiae Cortex, 1 qian), Shan Yao (Dioscoreae Rhizoma, 5 qian), Gan Cao (1 qian), Fu Ling (2 qian), Qian Shi (3 qian), Ren Shen (3 qian), Rou Gui (3 fen), Bai Zhu (5 qian). Decoct in water and take orally. One dose stops abdominal pain; two lightens waist; three restores flexibility.
This formula boosts vigorous Yang without boosting weak Yin. Yin obstructs Yang because it bullies Yang's weakness; thus, I boost Yang without boosting Yin. Using Fu Zi and Fang Ji would severely damage Yin and Yang, likely resulting in lifelong disability.
Ji Yang Tang (Rescue Yang Decoction) is also usable.
Du Zhong (2 qian), Shan Yao (1 liang), Gan Cao (1 qian), Ren Shen (5 qian), Bai Zhu (5 qian), Po Gu Zhi (Psoraleae Fructus, 1 qian). Decoct in water and take orally.
In winter, contracting cold pathogen, profuse sweating, panting unable to breathe, face red like vermilion, inability to speak, calling for water to save self, but drinking only one mouthful, unwilling to drink more. People think extreme heat, wanting to use Bai Hu Tang to clear Yangming fire, unaware this is Dai Yang syndrome (Floating Yang), upper heat and lower cold. Using Bai Hu Tang, even with extra Ren Shen, would cause death upon ingestion. Prescription:
Ba Wei Di Huang Wan (Eight-Ingredient Rehmannia Pill, 0.5 jin). Decoct in large pot, allow free drinking. Must sleep soundly for half a day. Upon waking, sweating stops, breathing normalizes, face becomes pale and clear, thirst ceases.
This condition originally should not have been sweated, but mistaken sweating caused profuse perspiration. With massive sweating, Yang pathogen dissipates, Yang Qi scatters, Yin rises with it, attempting to escape via throat. Skin pores sweat, but surging Yin cannot fully exit with sweat, rushing straight to throat, unstoppable. Yin rising, Yang leaking, unable to guide Yin back to Qi Hai, Yang follows Yin upward, Yin forces Yang down, causing panting. Yang above, Fire above is natural. Yin rising means extreme cold in Kidney Palace; lower lacks fire, upper fire cannot return to source, flaming face red. Upper fire不散, mouth thirsty, calling for water saves throat heat, not stomach heat. Excess heat usually stems from Stomach Fire, characterized by screaming madness. Here, breathing is calm despite panting, speechless, indicating deficiency heat. Truly upper false heat and lower true cold. Ba Wei Di Huang Wan supplements water while supplementing fire. Swallowed, fire resolves with water; in stomach, water calms with fire. Harmonizing upper and lower, irrigating Lung and Kidney, truly miraculous. Sweating causing Yang loss injures Qi; why treat Kidney to succeed? Yang loss means no fluids, boiling internal fire. Greatly tonifying True Yin quenches Stomach flame. Once Stomach fire rests, Kidney gate closes. Closed gate generates Stomach Earth qi. Generated Earth qi nourishes Lung Metal. Nourished Lung Metal executes clearing function. Mother calls son, same qi attracts, inevitably drawing Kidney Qi back to its home. Kidney Qi returned, warmed by spring-like warmth and nourished by spring water, Fire born from Water, Water pleased by Fire, achieving swift efficacy.
Fan Huo Tang (Return Fire Decoction) treats this condition miraculously.
Shu Di Huang (3 liang), Shan Zhu Yu (Corni Fructus, 1 liang), Rou Gui (3 qian). Decoct in water and take orally.
Winter Shanghan causing cold limbs, blue face, cold hands, hot feet. People think direct invasion of Yin Cold, suggesting Li Zhong Tang, which is incorrect. This is Liver Qi stagnation and dispersion failure, Wind pathogen between exterior and interior. Using Li Zhong Tang would cause madness and death. Direct Yin Cold invasion always starts with cold feet. Hot feet prove not direct Liver meridian invasion. Pathogen not inside Liver meridian suggests not treating Liver directly. Yet, pathogen outside meridian affects owner. Neighbor's shock brings vigilance; thief outside door causes panic. Using Li Zhong Tang is attacking fire against thief; house burns before thief caught, thief enters courtyard, kills owner. Treat with modified Xiao Chai Hu Tang (Minor Bupleurum Decoction) to scatter exterior-interior pathogen, calming Liver Qi, dissolving external evil. Prescription:
Chai Hu (2 qian), Bai Shao (5 qian), Gan Cao (1 qian), Dang Gui (1.5 qian), Huang Qin (1 qian), Ban Xia (1 qian). Decoct in water and take orally. One dose warms hands; two stops cold limbs, removes body heat, face turns white.
Qi Xian Tang (Seven Worthies Decoction) is very effective for this condition.
Bai Shao, Bai Zhu (each 5 qian), Gan Cao (1 qian), Rou Gui (3 fen), Chai Hu (1 qian), Dan Pi (Moutan Cortex, 3 qian), Tian Hua Fen (Trichosanthis Radix, 2 qian). Decoct in water and take orally. Calms after one dose.
Winter Shanghan, body heat, spontaneous sweating, aversion to cold but not heat. People think Yangming disease, wanting Shi Gao Tang, which is wrong. Sweating resembles Yangming, but Yangming always hates heat. Aversion to cold, not heat, indicates severe Yang deficiency, pathogen wanting to exit but failing, internal heat resolved, internal cold unresolved. Caused by mistaken sweating. Use Bu Zhong Yi Qi Tang (Tonify Middle and Augment Qi Decoction):
Ren Shen (3 qian), Huang Qi (3 qian), Bai Zhu (2 qian), Dang Gui (2 qian), Chai Hu (1 qian), Sheng Ma (Cimicifugae Rhizoma, 4 fen), Chen Pi (1 qian), Gan Cao (1 qian). Add Gui Zhi (5 fen). Decoct in water and take orally. One dose stops sweating, cools body, relieves aversion to cold.
Bu Zhong Yi Qi Tang treats internal injury, not Shanghan, invented by Li Dongyuan with divine merit. Why use it? Shanghan includes internal injury cases; one must not rigidly stick to Shanghan methods but seek variable treatments. This case resulted from mistaken sweating; pathogen in meridians is shallow, cold mild, constituting external contraction with internal injury. Bu Zhong Yi Qi Tang tonifies righteousness while expelling pathogen, hence successful, especially with Gui Zhi to disperse cold. Mistaking for Yangming and using Bai Hu Tang with Shi Gao would likely turn it into deficiency-cold disease, leading to death. How important accurate differentiation is!
Wen Zheng Tang (Warm and Correct Decoction) is also usable.
Ren Shen (5 qian), Huang Qi (1 liang), Dang Gui (5 qian), Chai Hu (1 qian), Gan Cao (5 fen), Shen Qu (1 qian), Gui Zhi (3 fen). Decoct in water and take orally.
Winter Shanghan, body heat unresolved for 5-6 days, delirium, thirst, free urination, desire to lie down. People think residual Yangming heat, I disagree. Delirium belongs to Stomach Heat, but Stomach Heat delirium features loud voice and anger when provoked. Here, delirium is low-voiced, not Stomach Heat. If not Stomach Heat, why thirst and desire to drink? Drinking water does not produce phlegm but goes straight to Bladder, causing free urination, proving not Stomach Heat. Yangming Fire excess causes madness; here, peaceful desire to lie down contradicts Stomach Heat. If not Stomach Heat, why persistent delirium and thirst for 5-6 days? This is Heart Deficiency. Heart deficiency causes Spirit not guarding residence (delirium); Heart deficiency causes Fire arising in Pericardium (thirst). Heart and Small Intestine are exterior-interior. Water entering Heart moves to Small Intestine, hence free urination. Treatment:
Fu Ling (5 qian), Mai Dong (1 liang), Dan Pi (2 qian), Chai Hu (1 qian), Gan Cao (5 fen). Decoct in water and take orally. One dose stops delirium; two relieves thirst; body heat resolves.
Named Qing Re San (Clear Heat Powder). Mai Dong tonifies Heart; Fu Ling disperses heat; Chai Hu, Dan Pi, Gan Hao harmonize pathogen. Strong Heart Qi repels pathogen; pathogen exits via Small Intestine, Heart calms, fluids generate, thirst ceases, Kidney Qi ascends to Heart, essence grows, sleepiness vanishes. Suspecting Stomach Heat and using Bai Hu or Qing Long Tang would likely cause nosebleeds.
Liang Jie Tang (Cool and Resolve Decoction) is also usable.
Fu Shen (Poria with Hostwood, 3 qian), Mai Dong (5 qian), Xuan Shen (1 liang), Chai Hu (1 qian), Gan Cao (3 fen), Chao Zao Ren (Ziziphi Spinosani Semen, 2 qian). Decoct in water and take orally.
Winter Shanghan, days 5-6 alternating chills and fever, fullness and distress in chest and hypochondrium, vomiting or spitting, thirst or no thirst, irritability or no irritability. People think Shaoyang disease, suggesting Xiao Chai Hu Tang for harmonization. Xiao Chai Hu Tang is the holy medicine for Shaoyang pathogen, seemingly suitable. Shaoyang lies between exterior and interior; pathogen entering Yin causes chills, exiting Yang causes fever. Phlegm knots in chest causing fullness, urge to vomit but not, urge to thirst but not, generating irritability. Chai Hu Tang harmonizes easily, but usable only once, not commonly. Shaoyang Gallbladder Wood loves Water most, then Wind. Chai Hu is a Wind herb; while it resolves resentment, daily use dries dampness, causing dryness. Rain is needed for lush growth. Like summer drought withering crops, sweet rain is needed for rescue. After Chai Hu Tang, must use water-supplementing agents. Prescription: Ji Sheng Tang (Generate Life Decoction):
Shu Di Huang (5 qian), Xuan Shen (5 qian), Mai Dong (3 qian), Shan Zhu Yu (1 qian), Shan Yao (3 qian), Fu Ling (2 qian), Bai Shao (3 qian), Chai Hu (5 fen), Shen Qu (3 fen), Zhu Ru (Bambusae Caulis in Taedia, 1 pill). Decoct in water and take orally. One dose removes irritability/fullness; two stops chills/fever; three eliminates prior symptoms.
