In winter, contracting cold pathogen (Han Xie), the body develops fever for only one day before delirium (saying nonsense) appears. People assume this is the pathogen transmitting to the Yangming channel (Yangming meridian syndrome). Little do they know that the patient has a pre-existing excess of stomach fire in the Yangming stage. When wind enters the Taiyang channel (Taiyang meridian syndrome), the stomach fire boils up and becomes restless. If treatment attempts to address both by purging stomach heat from the Yangming stage, the condition will not improve. However, since the Taiyang pathogen is currently vigorous, failing to specifically treat the Taiyang stage means the Wei-level (Defensive level) pathogen cannot be dispersed, nor can the Ying-level (Nutritive level) pathogen be resolved. Removing Yangming fire first might inadvertently draw the pathogen deeper into the Yangming stage, exacerbating the burning disaster. It is better to treat only the Taiyang stage, preventing the Taiyang pathogen from penetrating deeply, allowing the Yangming fire to dissipate spontaneously without direct treatment. The formula used is Pingyang Tang (Calm Yang Decoction): Guizhi (Cinnamon Twig, 3 fen), Mahuang (Ephedra, 1 qian), Gancao (Licorice Root, 1 qian), Qinghao (Sweet Wormwood, 3 qian), Tianhuafen (Trichosanthes Root, 1 qian). Decoct in water and take orally. After one dose, the fever subsides, and delirium ceases.
This formula uses less Guizhi and more Mahuang because the cold is light while the heat is heavy. Qinghao is used as the sovereign drug because it clears heat and disperses pathogens, entering the Bladder channel and moving towards the Stomach. It resolves the pathogen in the Bladder and the fire in the Stomach, not only avoiding drawing the pathogen into the Yangming stage but also helping to disperse pathogens out of the Yangming stage. Tianhuafen is added because delirium is often accompanied by phlegm; Tianhuafen effectively eliminates phlegm in the diaphragm without increasing heat. Combined with Qinghao, Guizhi, and Mahuang, it works from top to bottom, eliminating phlegm and thus eliminating the pathogen. With both phlegm and pathogen cleared, why would delirium persist? Hence, efficacy is achieved in one dose.
This condition can also be treated with Zhengxian Tang (Seize the Initiative Decoction): Guizhi (Cinnamon Twig, 5 fen), Mahuang (Ephedra, 5 fen), Shigao (Gypsum, 1 qian), Maidong (Ophiopogon Tuber, 5 qian), Fuling (Poria, 5 qian), Banxia (Pinellia Rhizome, 8 fen). Decoct in water and take orally.
In winter, contracting cold pathogen, the body develops fever for two days, presenting symptoms resembling malaria. People assume this indicates transmission to the Shaoyang channel (Shaoyang meridian syndrome). In fact, there was originally cold pathogen in the Shaoyang region, which manifests concurrently upon contracting cold. While Xiaochaihu Tang (Minor Bupleurum Decoction) may show some effect, it is not the correct method. The proper approach is to heavily treat the Yangming stage while simultaneously treating the Shaoyang stage. Since the fire-pathogen in the Yangming stage has not dissipated, even if Shaoyang symptoms appear, the pathogen remains in the Yangming stage. Pathogens lingering in Yangming can cause alternating chills and fever, delirium, and mania. By heavily treating Yangming, stomach fire resolves, preventing the pathogen from moving to Shaoyang. The original cold pathogen in Shaoyang, left isolated and unsupported, cannot reignite Yangming fire. Naturally, Yangming fire extinguishes, and Shaoyang pathogen resolves. The formula used is Puoxie Tang (Break Evil Decoction): Shigao (Gypsum, 3 qian), Chaihu (Bupleurum Root, 1 qian), Banxia (Pinellia Rhizome, 1 qain), Fuling (Poria, 3 qian), Gancao (Licorice Root, 1 qian), Maidong (Ophiopogon Tuber, 1 liang), Xuanshen (Scrophularia Root, 3 qian), Chenpi (Aged Tangerine Peel, 1 qian). Decoct in water and take orally. After one dose, fever resolves, and malaria-like symptoms heal.
This formula uses Shigao and Xuanshen to treat Yangming fire, and Maidong to moisten lung dryness. Lung dryness fails to control excessive Liver-Gallbladder activity. Moreover, dry lungs draw fluids from the Stomach, drying Stomach earth further and intensifying fire. Using ample Maidong moistens Lung metal, reducing reliance on Stomach earth. Nourished Lung qi naturally controls Liver and Gallbladder wood, preventing Shaoyang pathogen from colluding with stomach fire. Chaihu soothes Shaoyang qi, while Fuling, Gancao, Banxia, and Chenpi harmonize between Yangming and Shaoyang. Without support, the pathogen collapses.
This condition is also well-treated with Bagong Heyang Tang (Eight Gentlemen Harmonizing Yang Decoction): Shigao (Gypsum, 1 qian), Chaihu (Bupleurum Root, 2 qian), Fuling (Poria, 3 qian), Baizhu (Atractylodes Macrocephala Rhizome, 2 qian), Gancao (Licorice Root, 1 qian), Chao Zhizi (Fried Gardenia Fruit, 1 qian), Qingpi (Green Tangerine Peel, 3 fen), Tianhuafen (Trichosanthes Root, 1 qian). Decoct in water and take orally.
In winter, contracting cold pathogen, the body develops fever for three days, accompanied by abdominal fullness and spontaneous diarrhea. People assume yang has transmitted to yin. However, yin syndromes feature abdominal fullness and diarrhea, but so do yang syndromes. Failing to distinguish between yin and yang and indiscriminately using methods for Taiyin syndrome often leads to death. How to distinguish? Taiyin spontaneous diarrhea involves extreme cold and pain; Shaoyang spontaneous diarrhea involves extreme heat and pain. Although pain is similar, the nature differs. Pressing the abdomen: increased pain indicates a yang syndrome; no pain indicates a Taiyin yin syndrome. Therefore, treating yang syndromes requires harmonizing Shaoyang pathogen, not mistreating Taiyin. The formula used is Jiajian Chaihu Tang (Modified Minor Bupleurum Decoction): Chaihu (Bupleurum Root, 1 qian), Baishao (White Peony Root, 5 qian), Fushen (Poria with Hostwood, 2 qian), Gancao (Licorice Root, 1 qian), Zhizi (Gardenia Fruit, 2 qian), Chenpi (Aged Tangerine Peel, 1 qian), Danggui (Angelica Sinensis Root, 3 qian), Zhiqiao (Bitter Orange Fruit, 5 fen), Dahuang (Rhubarb Root, 5 fen). Decoct in water and take orally. Abdominal fullness resolves after one dose; spontaneous diarrhea stops after two doses.
This formula combines harmonization with mild purgation and light tonification within separation strategies, dispersing fire-pathogen while preserving healthy qi. Da Chengqi Tang (Major Order the Yellow Decoction) is too harsh; Da Chaihu Tang (Major Bupleurum Decoction) is too focused on separation. Thus, Jiajian Chaihu Tang is designed for Shaoyang abdominal fullness with spontaneous diarrhea.
This condition can also use Hegong San (Harmonize and Attack Powder): Chaihu (Bupleurum Root), Zhizi (Gardenia Fruit), Danpi (Moutan Cortex, 2 qian each), Baishao (White Peony Root, 5 qian), Fuling (Poria, 3 qian), Gancao (Licorice Root), Chenpi (Aged Tangerine Peel), Dahuang (Rhubarb Root, 1 qian each). Decoct in water and take orally.
In winter, contracting cold pathogen, the body develops fever for four days with persistent aversion to cold. People assume Taiyin has transformed into Shaoyin. However, it remains Taiyin. Taiyin belongs to Spleen Earth; Shaoyin belongs to Kidney Water. They seem different, but Spleen Earth is damp earth. Damp earth contains water imagery; thus, Spleen cold is Earth cold, and Earth cold is Water cold. Aversion to cold appears before pathogen transmits to the Kidney. Treatment should focus solely on the Spleen, resolving cold spontaneously. The formula used is Lizhong Tang (Regulate the Middle Decoction) modified: Baizhu (Atractylodes Macrocephala Rhizome, 1 liang), Renshen (Ginseng Root, 3 qian), Fuling (Poria, 3 qian), Rougui (Cinnamon Bark, 1 qian), Fuzi (Prepared Aconite Root, 1 qian). Decoct in water and take orally. Aversion to cold and fever resolve after one dose.
Using Rougui and Fuzi seems to treat Shaoyin Kidney, but with Renshen and Baizhu as sovereigns, it treats the Spleen. Although Spleen and Kidney can be treated together, and Renshen/Baizhu enter the Kidney, Rougui and Fuzi penetrate all channels. Treating the Spleen effectively treats the Kidney, making this formula miraculous.
This condition is also effectively treated with Jiawei Guifu Tang (Added Cinnamon and Aconite Decoction): Baizhu (Atractylodes Macrocephala Rhizome, 1 liang), Rougui (Cinnamon Bark), Ganjiang (Dried Ginger, 1 qian each), Fuzi (Prepared Aconite Root), Gancao (Licorice Root, 5 fen each). Decoct in water and take orally.
In winter, contracting cold pathogen, the body develops fever for five days, followed by limb coldness (Jue). People assume cold pathogen has entered Jueyin. In reality, it is due to dryness of Kidney Water failing to nourish the Liver. Limb coldness is typically a Jueyin symptom, but here it occurs before pathogen entry because Liver blood is extremely dry, drawing from depleted Kidney Water. The Liver seeks its Mother (Kidney); the Kidney, invaded by wind, suffers. This mother-son distress causes limb coldness without Jueyin pathogen entry. Treatment should focus on the Kidney, stabilizing the Liver. The formula used is Zimu Liangkuai Tang (Mother and Son Both Comfortable Decoction): Shudi (Prepared Rehmannia Root, 5 qian), Maidong (Ophiopogon Tuber, 5 qian), Danggui (Angelica Sinensis Root, 2 qian), Shanzhuyu (Cornus Fruit, 3 qian), Fuling (Poria, 2 qian), Qianshi (Euryale Seed, 2 qian), Shanyao (Dioscorea Rhizome, 2 qian), Xuanshen (Scrophularia Root, 5 qian). Decoct in water and take orally. Limb coldness stabilizes after one dose; fever resolves after two doses.
This formula purely tonifies the Kidney, with Danggui nourishing Liver blood. Treating the Kidney inherently treats the Liver, negating the need for Baishao (White Peony Root) to calm Liver qi. Baishao is contraindicated here because its sour astringency, while stopping heat-induced limb coldness quickly, may trap fire when combined with hydrating herbs during active pathogen presence. Danggui, being mobile rather than astringent, assists Shudi and Shanzhuyu in generating Water to nourish the Liver, thereby controlling the Liver via Kidney tonification.
In winter, contracting cold pathogen, the body develops fever for six days. Sweating does not resolve the pathogen, and Taiyang symptoms persist. People assume pathogen has returned to Taiyang. Actually, the pathogen wants to return to Taiyang but cannot. If untreated, the pathogen may mutate. Since it cannot fully return, it peeks at the Taiyang gate. Using Taiyang herbs guides the pathogen back along its previous route, facilitating dispersion. The formula used is Guizhi Tang (Cinnamon Twig Decoction) in small amounts to disperse. One dose resolves all pathogen. Excessive use causes severe damage. This illustrates medical flexibility.
This condition is also well-treated with Jiexie Tang (Dispel Evil Decoction): Guizhi (Cinnamon Twig, 3 fen), Fuling (Poria, 5 qian), Danggui (Angelica Sinensis Root, 3 qian), Shengdi (Rehmannia Root, 5 qian), Baizhu (Atractylodes Macrocephala Rhizome, 3 qian), Chenpi (Aged Tangerine Peel, 3 fen), Gancao (Licorice Root, 1 qian), Maidong (Ophiopogon Tuber, 5 qian). Decoct in water and take orally.
In winter, contracting cold pathogen, by the seventh day, fever persists, and delirium continues. People assume transmission back to Yangming. Actually, the pathogen seeks Yangming but is rejected. Having been previously invaded, Yangming resists new intrusion. Fear of re-invasion triggers delirium, distinct from previous excess-type delirium. Treatment should not block Yangming but disperse Taiyang pathogen. Once Taiyang pathogen is gone, it won't re-enter Yangming. The formula used is Guizhi Tang (Cinnamon Twig Decoction). Delirium stops after one dose. Avoid Shigao Tang (Gypsum Decoction) to prevent damaging Stomach qi.
This condition is also miraculously treated with Heying Tang (Harmonize Ying Decoction): Mahuang (Ephedra, 3 fen), Fuling (Poria, 3 qian), Danggui (Angelica Sinensis Root, 3 qian), Xuanshen (Scrophularia Root, 5 qian), Gancao (Licorice Root, 1 qian), Maidong (Ophiopogon Tuber, 5 qian), Zhuye (Bamboo Leaf, 30 pieces), Banxia (Pinellia Rhizome, 5 fen). Decoct in water and take orally.
In winter, contracting cold pathogen, by the eighth day, tidal fever persists. People assume transmission to Shaoyang. Actually, the pathogen is in Yangming, attempting to exit but stuck. Yangming is a fu-organ rich in qi and blood, harboring significant pathogens like phlegm in the stomach/diaphragm, causing tidal fever independently of Shaoyang. The pathogen eyes Shaoyang, but Shaoyang, having suffered from Yangming previously, resists. Thus, tidal fever appears, but the pathogen hasn't entered Shaoyang. Treatment should clear Yangming heat alone; Shaoyang pathogen will dissipate. The formula used is Jiewei Tang (Clear Stomach Decoction): Qinghao (Sweet Wormwood, 5 qian), Fuling (Poria, 2 qian), Gancao (Licorice Root, 5 fen), Maidong (Ophiopogon Tuber, 5 qian), Xuanshen (Scrophularia Root, 3 qian), Zhuye (Bamboo Leaf, 50 pieces). Decoct in water and take orally. Stomach heat clears after one dose; tidal fever subsides after two doses.
This formula extinguishes Yangming fire and resolves Shaoyang atmosphere, treating both. Treating only Shaoyang intensifies Yangming fire; treating only Yangming dries Shaoyang. Balancing both prevents severity escalation. Single-focus on Yangming implicitly treats Shaoyang, ensuring complete success.
This condition is also effectively treated with Fayue Tang (Ventilate and Rise Decoction): Gegen (Kudzu Root, 3 qian), Fuling (Poria, 5 qian), Gancao (Licorice Root, 5 fen), Maidong (Ophiopogon Tuber, 3 qian), Xuanshen (Scrophularia Root, 1 liang), Shengdi (Rehmannia Root, 3 qian), Chaihu (Bupleurum Root, 5 fen). Decoct in water and take orally.
In winter, contracting cold pathogen, by the ninth day, diarrhea persists. People assume transmission to Taiyin, transforming yang to yin. Actually, it is yang departing yin. Transforming to yin involves transmission into yin (causing diarrhea); departing yin involves transmission out of yin. Yin-transmission diarrhea involves abdominal pain; yin-departure diarrhea does not. Persistent diarrhea without pain on day nine indicates departure from yin. Do not use Taiyin antidiarrheals, as this traps pathogen in yin, causing critical conditions. Treat Shaoyang to resolve exterior-interior pathogen; diarrhea and chills/fever will cease. The formula used is Xiaochaihu Tang (Minor Bupleurum Decoction) modified: Chaihu (Bupleurum Root, 1 qian), Fuling (Poria, 3 qian), Gancao (Licorice Root), Huangqin (Scutellaria Root, 1 qian each), Chenpi (Aged Tangerine Peel, 5 fen). Decoct in water and take orally. Diarrhea stops after one dose; chills/fever resolve.
This formula resolves half-exterior/half-interior pathogen and separates dampness, exiting yang efficiently.
This condition is also effectively treated with Heyin Tang (Unite Yin Decoction): Chaihu (Bupleurum Root, 8 fen), Fuling (Poria, 5 qian), Gancao (Licorice Root, 5 fen), Tianhuafen (Trichosanthes Root, 1 qian), Zhiqiao (Bitter Orange Fruit, 3 fen), Shenqu (Medicated Leaven, 5 fen), Baishao (White Peony Root, 3 qian). Decoct in water and take orally.
In winter, contracting cold pathogen, by the tenth day, aversion to cold and vomiting occur. People assume transmission to Shaoyin. Actually, the pathogen refuses to enter Shaoyin. Why aversion to cold and vomiting? Transmission to Taiyin depletes Middle Jiao qi, weakening Spleen qi. Weak Spleen consumes Kidney fire. The Kidney, previously invaded, is vulnerable. The "son" (Spleen) bears the burden to protect the "mother" (Kidney). Thus, symptoms appear in Taiyin before reaching Shaoyin. Treat Taiyin Spleen to stop vomiting. However, treating Spleen alone without Kidney fails to generate Spleen earth with Kidney fire, leaving aversion to cold unresolved and vomiting temporary. The formula used is Shirui Liangwen Tang (Warm Spleen and Kidney Decoction): Renshen (Ginseng Root, 3 qian), Baizhu (Atractylodes Macrocephala Rhizome, 5 qian), Rougui (Cinnamon Bark, 1 qian), Bajitian (Morinda Root, 3 qian), Dingxiang (Clove, 3 fen), Roudoukou (Nutmeg, 1 piece), Qianshi (Euryale Seed, 3 qian), Shanyao (Dioscorea Rhizome, 3 qian). Decoct in water and take orally. Aversion to cold stops after one dose; vomiting ceases after two doses.
This formula tonifies Spleen with Renshen/Baizhu, tonifies Kidney with Bajitian/Qianshi/Shanyao, and expels cold with Rougui/Dingxiang. Strengthening Kidney fire generates Spleen earth, warming both mother and son.
This condition is also miraculously treated with Jiawei Renshen Zhu Fu Jiang Tang (Added Ginseng, Atractylodes, Aconite, and Ginger Decoction): Renshen (Ginseng Root, 5 qian), Baizhu (Atractylodes Macrocephala Rhizome, 5 qian), Roudoukou (Nutmeg, 1 piece), Fuzi (Prepared Aconite Root, 3 fen), Ganjiang (Dried Ginger, 1 qian). Decoct in water and take orally.
In winter, contracting cold pathogen, by the eleventh day, fever intensifies, with frequent limb coldness (three to four times daily). People assume transmission to Jueyin. Actually, pathogen cannot transmit to Liver. Shaoyin is Cold Water; pathogen in Shaoyin without Jueyin entry causes limb coldness and heat signs. This coldness is apparent heat but true internal cold forcing yang outward. Death is likely, but proper medication saves. Zhang Zhongjing prescribed moxibustion for Shaoyin, teaching against treating Jueyin directly. Yang herbs can also revive life. The formula used is Huisheng Zhishen Tang (Revive Life Supreme Divine Decoction): Renshen (Ginseng Root, 3 liang), Rougui (Cinnamon Bark, 3 qian), Baizhu (Atractylodes Macrocephala Rhizome, 2 liang), Shengjiangzhi (Fresh Ginger Juice, 1 he), Congbai (Scallion White, 10 stalks, juiced). Decoct in water and take together. Limb coldness stabilizes after one dose; fever resolves after two doses.
Large doses of Renshen/Baizhu require Ginger/Scallion juice to vent externally, drawing pathogen from Kidney. Guided outward by these juices, limb coldness resolves without involving Liver. This demonstrates clever treatment.
This condition is also effectively treated with Jiawei Rendi Tang (Added Ginseng and Rehmannia Decoction): Shudi (Prepared Rehmannia Root, 2 liang), Renshen (Ginseng Root, 1 liang), Baizhu (Atractylodes Macrocephala Rhizome, 1 liang), Fuzi (Prepared Aconite Root, 1 qian), Shengjiangzhi (Fresh Ginger Juice, 1 he). Decoct in water and mix.
In winter, contracting cold pathogen, by the twelfth day, fever persists without limb coldness. People assume arrival at Jueyin implies recovery. Actually, extreme deficiency prevents limb coldness despite deep heat. Deep heat usually causes deep coldness driven by sufficient yuan qi. Here, yuan qi is too exhausted to drive coldness. Disease enters Liver by day twelve; absence of coldness indicates exhaustion, not lack of heat. Treatment should tonify Liver qi and assist with heat-clearing herbs. This moistens Jueyin, relaxes Wood qi, prevents pathogen retention, resolves fever, and induces/resolves coldness. The formula used is Xiaojue San (Dispel Coldness Powder): Baishao (White Peony Root, 5 qian), Danggui (Angelica Sinensis Root, 5 qian), Danpi (Moutan Cortex, 3 qian), Shengdi (Rehmannia Root, 2 qian), Gancao (Licorice Root, 1 qian), Renshen (Ginseng Root, 1 qian), Chao Hei Jingjie (Fried Black Schizonepeta Spike, 3 qian), Chao Zhizi (Fried Gardenia Fruit, 1 qian), Tianhuafen (Trichosanthes Root, 2 qian). Decoct in water and take orally. Limb coldness appears after one dose; stabilizes after two doses.
This formula tonifies Liver and cools blood to treat transmitted cold. Liver dryness and internal heat cause latent coldness due to deficiency. Nourishing Liver blood allows deep heat to surface. Tonifying deficiency enables riding the induced coldness to dissipate heat. Physicians should dare to treat boldly based on this insight.
This condition is also highly effective with Zengjian Xiaoyao San (Modified Free and Easy Wanderer Powder).
