Whenever a person falls ill, it is due to the gathering of pathogenic factors (Xieqi). Even if the patient is emaciated and weak, one must not exclusively employ tonification methods. The Classics state: "Where pathogenic factors gather, Qi must be deficient." For instance, in cases such as red eyes, it is clearly caused by pathogenic heat, and one may exclusively employ purgation methods for treatment. For other various diseases, only balanced tonification and purgation are suitable; one must first purge and then tonify. This is called first eliminating pathogenic factors and subsequently replenishing true Qi. This is a secret not readily transmitted by former masters.
When performing tonification now, instruct the patient to take a breath in, and rotate the needle along with the inhalation. For example, when needling the left side, turn the needle tip to the right. Hold the needle with the thumb and index finger of the right hand, pushing the index finger forward and pulling the thumb backward, continuing to twirl the needle deeper by one or two fen. In this way, True Qi can penetrate deeply into the muscle layers. If multiple acupoints are involved, apply this method to all. After completion, pause briefly, then lightly tap the needle head three times with a finger, repeating this three times. Next, hold the needle with the thumb and index finger of the left hand, rub the needle three times continuously with the thumb (this is called "Fei" or Flying), advance the needle deeper by one or two fen, directing the needle tip to the left (this is called "Three Advancements and Three Flights"). Perform this method five or six times until a heavy and tight sensation is felt under the needle, or warmth is perceived, indicating that Qi has arrived sufficiently. At this moment, have the patient inhale, withdraw the needle rapidly upon inhalation, and immediately press and seal the acupoint with the hand. This is what is called the tonification method (from *Zhenjiu Dacheng*).
Shufu stated: Regarding tonification methods, suppose an acupoint requires insertion to a depth of five fen. One must first insert two fen, wait for Deqi (arrival of Qi) under the needle, then insert another two fen. Upon reaching four fen, wait again for Deqi, then insert the final one fen, totaling five fen. Then, withdraw the needle rapidly and immediately press the puncture hole firmly with the thumb of the left hand to prevent bleeding; this constitutes tonification. Regarding purgation methods, suppose an acupoint requires insertion to five fen. Insert directly to five fen, wait for approximately Deqi, then withdraw the needle by two fen. Pause briefly, then withdraw another two fen, totaling a withdrawal of four fen. Wait briefly again for Deqi, then slowly withdraw the needle completely without closing the puncture hole with the hand, allowing pathogenic Qi or blood to exit; this constitutes the purgation method.
