The formula from *Waitai Mi Yao* (Secrets of the Outer Palace): Treats Shanghan (Cold Damage) disease persisting for eight or nine days, up to ten or more days, presenting with severe restlessness, thirst, and fever; sanjiao (Triple Burner) has sores and hidden pathogens causing diarrhea; or symptoms such as opening the mouth and protruding the tongue, ulcerated eyes, oral ulcers, and inability to recognize people. This formula is used to eliminate heat toxins and stop dysentery.
Longgu (Fossilized Bones, 30g), water 10 dou (approx. 2L), decoct until 4 sheng (approx. 800ml) remain. Submerge in a well to cool, take 5 he (approx. 100ml) cold, gradually increasing the dose.
The formula from *Zhouhou Beiji Fang* (Handbook of Prescriptions for Emergencies): Treats heat diseases causing severe diarrhea bordering on death.
Longgu (Fossilized Bones, 30g), powdered, water 10 dou (approx. 2L), decoct to obtain 5 sheng (approx. 1L). Wait until extremely cold, then drink in small amounts. Recovery occurs with sweating.
Also treats chronic intermittent dysentery that does not cease.
Longgu (Fossilized Bones, 15g), crushed, water 5 sheng (approx. 1L), decoct to obtain 2.5 sheng (approx. 500ml). Divide into five doses, drink after freezing. Additionally, mix Longgu powder with rice water to form pills, taking ten pills per dose.
Commentary: All three formulas use Longgu (Fossilized Bones) decocted in water and taken cold, presumably using cold to counteract heat and astringency to prevent collapse.
Zhuche Wan (Park-the-Cart Pill): Treats all types of dysentery, regardless of cold or heat patterns.
Ejiao (Donkey-Hide Gelatin, 75g), crushed, stir-fried with clam shell powder until granular, then simmered with 4 sheng (approx. 800ml) of vinegar to form a paste. Danggui (Angelicae Sinensis Radix, 75g). Huanglian (Coptidis Rhizoma, 150g), remove rootlets. Ganjiang (Zingiberis Rhizoma Praeparatum, 50g), processed.
Grind the above herbs into fine powder, mix with the Ejiao paste, and pound into pills the size of wutong seeds. Take thirty pills per dose before meals, administered with rice water. Three times daily.
In cases of bloody dysentery caused by accumulated blood-heat, characterized by tenesmus and severe pain, if the pain persists despite purgation and cleansing, it is neither due to heat nor accumulation. It results from deficiency of ying-blood (nutritive blood) where yang-gang overcomes yin. Medication should be assisted with blood-nourishing herbs; once ying-blood is regulated, the pain will cease immediately.
Nocturnal emission and spermatorrhea, although both attributed to fire, differ in their involvement of the heart and kidney. When originating from the heart, spirit fluctuates upward, causing essence to leak downward. There is no need to treat the kidney; simply clear the heart, and the dreams will cease naturally. Although essence is stored in the kidney, its governance lies in the heart. Therefore, clever youths often suffer from this condition, while rustic peasants do not. It ultimately depends on the movement and stillness of the heart. When originating from the kidney, vigorous young men with abundant qi, who have long abstained from sexual activity, may experience congestion and heat in the meridians, leading to insecure essence. This corresponds to the classic saying: "When yang is excessive and cannot secure, yin qi is exhausted." This condition further divides into two types: one arising from wood (liver) and one from water (kidney), because both zang-organs contain ministerial fire. They should be treated separately. Another type arises from damp-heat in the spleen and stomach flowing downward, causing unclear essence in the kidney meridian, resulting from excessive consumption of rich wine and heavy flavors. As stated in *Zhizhi Fang*: Besides the heart and kidney, there is also involuntary leakage of spleen essence, cloudy urine, weakness in hands and feet, and soreness/pain in the lower back. One should use formulas containing Cangzhu (Atractylodis Rhizoma) to astringe spleen essence. What is meant by astringing spleen essence? Essence is generated from grain.
