1. In cases where a patient with vomiting has an abscess and pus, do not treat the vomiting directly; once the pus is discharged, the condition will resolve spontaneously.
2. Vomiting followed by thirst indicates that the disease is about to resolve; thirst followed by vomiting indicates water retention in the epigastrium (stomach region), which belongs to the category of fluid retention disorders (Yin Jia).
Patients who vomit should normally feel thirsty; if they do not feel thirsty now, it is due to Zhi Yin (fluid retention supporting in the chest/diaphragm) in the epigastrium, which belongs to Zhi Yin disorder.
3. Question: A patient has a rapid pulse (Shu Mai). Rapid pulse indicates heat, which should lead to digestion of food and increased appetite, yet the patient vomits instead. Why? The Master said: This is because sweating was induced previously, causing Yang Qi to become weak and Diaphragm Qi to be deficient, resulting in a rapid pulse. This rapidness represents guest heat (Ke Re, floating empty heat), which cannot digest food; the cause is deficiency-cold in the Stomach.
A wiry pulse (Xian Mai) indicates deficiency. With no surplus Stomach Qi, morning intake leads to evening vomiting, transforming into Wei Fan (Stomach Reversal). Cold is located in the upper jiao, but the physician incorrectly used purgatives. Now the pulse is wiry, hence it is named deficiency.
4. At the Cun Kou (radial artery) position, the pulse is faint (Wei) and rapid (Shu). Faint indicates lack of Qi; lack of Qi leads to Ying (Nutritive) deficiency; Ying deficiency leads to blood insufficiency; blood insufficiency leads to cold in the chest.
5. At the Zu Yang (dorsalis pedis artery) position, the pulse is floating (Fu) and choppy (Se). Floating indicates deficiency; choppy indicates Spleen injury. When the Spleen is injured, it fails to grind food. Morning food is vomited in the evening, and evening food is vomited in the morning; undigested food remains, termed Wei Fan. If the pulse is tight (Jin) and choppy, the disease is difficult to treat.
6. If a patient desires to vomit, do not use purgation.
7. For hiccups accompanied by abdominal fullness, observe the anterior (urination) and posterior (defecation) orifices. Whichever pathway is obstructed, unblock it, and the patient will recover.

10. For vomiting accompanied by borborygmus and epigastric stuffiness (Pi), Ban Xia Xie Xin Tang (Pinellia Decoction to Drain the Epigastrium) is the primary formula.
Ban Xia Xie Xin Tang Formula
Ban Xia (Pinelliae Rhizoma Preparatum, half sheng, washed) Huang Qin (Scutellariae Radix) Gan Jiang (Zingiberis Rhizoma) Ren Shen (Ginseng Radix) (each 3 liang) Huang Lian (Coptidis Rhizoma, 1 liang) Da Zao (Jujubae Fructus, 12 pieces) Gan Cao (Glycyrrhizae Radix et Rhizoma, 3 liang, honey-fried)
The above seven ingredients are decocted in 1 dou of water to obtain 6 sheng of liquid. Strain, then re-decoct to obtain 3 sheng. Take 1 sheng warm, three times daily.

The above six ingredients are decocted in 1 dou of water to obtain 3 sheng of liquid. Strain. Take 1 sheng warm, twice during the day and once at night.
12. For various types of vomiting where food cannot go down, Xiao Ban Xia Tang (Minor Pinellia Decoction) is the primary formula. (See Phlegm-Fluids chapter for formula.)
13. For vomiting with pathology above the diaphragm, if the patient subsequently desires water, it indicates resolution; give water immediately.
If the patient desires water, Zhu Ling San (Polyporus Powder) is the primary formula.
Zhu Ling San Formula
Zhu Ling (Polypori Sclerotium) Fu Ling (Poria) Bai Zhu (Atractylodis Macrocephalae Rhizoma) (equal parts)
The above three ingredients are pounded into powder. Take one fang cun bi (square-inch scoopful) with water, three times daily.
14. For vomiting with a weak pulse, frequent urination, slight fever, and cold extremities (Jue), the prognosis is poor; Si Ni Tang (Frigid Extremities Decoction) is the primary formula.
Si Ni Tang Formula
