There are two situations where diaphoresis (sweating therapy) can be employed: one is when wind-cold pathogen invades from the exterior, and the other is when fire qi is constrained internally.
Cases of deficiency of Yin with hyperactive deficient fire are difficult to treat. For constrained fire, dispersion should be applied according to the affected meridian; for mild cases, descending methods may be used, while for severe cases, dispersal should follow its ascending nature. Excess fire can be purged. Promoting urination is a very rapid method to descend fire.
Whenever Qi is in excess, it transforms into Fire.
In cases where Fire is urgent and severe, it must be moderated. Raw Gancao (Radix Glycyrrhizae) can be used, as it both purges Fire and moderates urgency; Renshen (Radix Ginseng) and Baizhu (Rhizoma Atractylodis Macrocephalae) may also be used.
For individuals with robust constitution and sufficient Zheng Qi (Upright Qi), presenting with excessive Fire leading to mania or madness, direct treatment (using cold/cooling herbs to directly subdue heat) can be applied, or they may drink water mixed with Niao Xiao (Sal Nitri) and Bing Pian (Borneolum Syntheticum). For those with weak constitution but excessive Fire causing mania, Shengjiang Tang (Fresh Ginger Decoction) can be given. If Ice Water were administered for direct treatment in such weak cases, death would ensue immediately. In some instances, tonifying Yin causes Fire to descend spontaneously, using herbs such as Chao Huangbai (Fried Cortex Phellodendri) and Dihuang (Radix Rehmanniae Preparata).
Shanzhiren (Semen Gardeniae) has a powerful ability to descend Fire, excreting it through urination. Its medicinal property allows it to curve and descend downward, a mechanism of descending fire that people often do not realize.
In cases of excessive Fire, cooling medicines should not be used abruptly; instead, warm dispersing methods must be employed first.
Another prescription: Zuojin Wan (Left Metal Pill) treats Liver Fire.
Huanglian (Radix Coptidis, 6 liang) and Zhuyu (Fructus Evodiae, 1 liang or half liang) are processed into pills with water. Take 50 pills with plain boiled water.
Dai Yun stated: New-onset cough accompanied by nasal congestion and heavy voice belongs to this category.
It mostly pertains to the Lung; dispersing agents should be pungent-warm or pungent-cool in nature.
(Attributed to Wind-Heat.)
Dai Yun stated: Ban (Macules/Patches) are characterized by colored spots without granular elevation. If there are granular elevations, it is Zhen (Papules/Rash). This arises from Wind-Heat carrying Phlegm, emerging from the interior to the exterior. One should refer to Fengwang Tongsheng San (Wind-Blocking Unblocking Powder) and disperse it via slight sweating. Using purgation is contrary to medical principles.
Ban caused by internal injury results from extreme deficiency of Stomach Qi, causing Fire to wander on the exterior of the body. Tonification to guide Fire downward is appropriate. Ban resembling Taiyang disease (Cold Damage) is caused by Phlegm-Heat manifesting externally; it should be dispersed via slight sweating. Using purgation is contrary to medical principles.
Dai Yun stated: Zhen (Papules/Rash) are characterized by superficial, small lesions with granular elevation. They appear and disappear quickly, appearing again after receding. This differs from Ban which lacks granular elevation. Careful differentiation is required.
This pertains to Heat and Phlegm in the Lung; clearing Lung Fire and descending Phlegm, or dispersing via sweating, are appropriate. There are also cases where purgation can be used.
This refers to the situation where everyone suffers from the same illness, also known as Tianxing Shi Yi (Epidemic Disease of Heavenly Transmission). There are three therapeutic methods: tonification, descending, and dispersing.
Another prescription:
Dahuang (Radix et Rhizoma Rhei), Huangqin (Radix Scutellariae), Huanglian (Radix Coptidis), Renshen (Radix Ginseng), Jiegeng (Radix Platycodonis), Fangfeng (Radix Saposhnikoviae), Cangzhu (Rhizoma Atractylodis), Huashi (Talcum), Xiangfu (Rhizoma Cyperi), Renzhonghuang (Human Manure Yellow).
Grind the above into powder and form pills with Shenqu (Massa Medicata Fermentata). Take 50–70 pills per dose. Administer as decoctions based on Qi, Blood, or Phlegm imbalances: for Qi deficiency, use Sijunzi Tang (Four Gentlemen Decoction); for Blood deficiency, use Siwu Tang (Four Substances Decoction); for excessive Phlegm, use Erchen Tang (Two Aged Decoction). If Heat is severe, Tongzi Xiaosui (Child's Urine) can be used to wash down the pills.
Datou Tianxing Bing (Great Head Epidemic), Dongyuan (Li Dongyuan) has a prescription:
Qianghuo (Rhizoma et Radix Notopterygii), Jiu Huangqin (Wine-processed Radix Scutellariae), Dahuang (Radix et Rhizoma Rhei, wine-steamed).
Dongwen Bing (Winter-Warmth Disease) occurs when non-seasonal climate conditions prevail during winter. In winter, the gentleman should store and conceal Yang Qi, but here it leaks outward. Treatment should exclusively use tonifying herbs combined with exterior-releasing agents.
Another prescription: Take a bamboo tube retaining nodes at both ends, drill a hole in the middle, place Gancao (Radix Glycyrrhizae) inside, seal the hole with a bamboo/wood peg, soak it in a fecal pit for one month, take it out, dry it, and use it specifically to treat Yi Du (Epidemic Toxins).
