This book is called a "forbidden book"? Because it is forbidden, it is kept secret. It is said to be closed and secreted, not wanting to be transmitted. But books should be the public use of the Great Dao (the Great Way), why destroy them by burning! This is incorrect! Calling it "forbidden" means it originated from within the imperial palace, not that its use is prohibited. It was likely due to the aversion to the long-standing filth of the Yuan Dynasty in China, that our Taizu Gao Emperor arose according to destiny, sweeping away the barbaric stench, eliminating cruelty, and punishing falsehood, personally braving arrows and stones. At that time, military generals did not spare their lives, breaking cauldrons and sinking boats (making retreat impossible). Amidst the lack of weapons, after sinking boats and breaking cauldrons, it was inevitable to suffer deaths and injuries from arrowheads, enduring the calamity of wounds and pain. Furthermore, extraordinary talents like Zhongshan were produced to assist him. Formulas were created according to the severity of injuries, responding to diseases with every application, truly possessing the miracle of bringing the dead back to life. Taizu could not bear to see the people's brains and entrails smeared on the ground, so he selected formulas that proved effective repeatedly. He ordered them to be collected and recorded in the palace, to establish life for the common people. At that time, Liu Qingtian participated in major military and state affairs. The ruler and minister were as one body, and thus he also gathered these formulas, having previously recorded them. Hence, it is called a forbidden book and kept secret at home; few in the folk world could obtain copies. Fortunately now, a descendant of civil and military officials, respecting the current Emperor's benevolence as vast as heaven, engages in the profession of traditional medicine (Qingnang), discussing the teachings of Xuanyuan (Yellow Emperor) and Qibo, seeking to test former secret miraculous formulas. I therefore casually recorded them, counting each one obtained, accumulating day by day and month by month, and carefully completed this work. This marks the beginning of the external transmission of forbidden formulas. Alas, it is not the complete book, but merely a few spots on a leopard's skin (a small part of the whole). I thus became deeply fond of this subject, fearing that future generations might have similar uses, so I wrote this at the beginning of the volume, to enter the eyes first upon opening, making it a secret text, not to be lightly disseminated, and to let others know the origin and flow of these formulas.
Step 1: Wash with decoction; Step 2: Assess the injured area; Step 3: Traction/Reduction; Step 4: Or incise with a knife to reach bone; Step 5: Press and align; Step 6: Apply Heilong San (Black Dragon Powder) to unblock; Step 7: Fill the wound with Fengliu San (Wind-Flow Powder); Step 8: Splint and bind; Step 9: Take oral medicine; Step 10: Wash again; Step 11: Reapply Heilong San to unblock; Step 12: Or refill the wound with Fengliu San; Step 13: Re-splint and bind; Step 14: Continue treating with oral medication as before.
If the cranial bones are fractured and crushed, gently flatten and align them by hand. If the skin is intact, apply Heilong San (Black Dragon Powder). If broken, fill the wound with Fengliu San (Wind-Flow Powder), wrap with silk cloth, keep away from wind, and avoid water to prevent Tetanus (Po Shang Feng). If water and wind pathogens enter the brain causing Tetanus, there will inevitably be scalp rupture and headache, which becomes incurable. For injuries within the hair, the hair must be cut off before applying medicine.
For cranial bone fractures depressed inward, decoct four liang of fresh tiger fat and five qian of Chuanxiong Rhizoma (Chuanxiong) in good wine, heat, and drink. This will relieve dizziness and pain.
For abdominal contusions with blood stasis causing pain, decoct five qian of Angelica Sinensis Radix (Danggui), five qian of Rheum Palmatum Rhizome (Dahuang), seven peach kernels (Taoren), and a pinch of Carthamus Flower (Honghua) in one bowl of wine. Take early in the morning at wu hour (3-5 AM).
For traumatic urinary retention, decoct two liang of Small Toona Root (Xiao Tongzi Shugen) chopped up, using half water and half wine. Urination will resume immediately.
For severed fingertips, align them precisely. Wrap externally with herbal water and candle inner membrane (herbal name) securely. Wait for skin and flesh to reconnect, then apply plaster and rub with wound-healing and muscle-regenerating medicine.
For severed throat or neck, suture with silver wire, apply herbal paste externally, changing once daily. After two changes, when skin and flesh join, replace with plaster, sprinkle with muscle-generating powder, and take upper-body decoction internally.
For intestines protruding due to knife or axe injury, mix sesame oil with medicine and water, soak green cloth, place on intestines. Wait until soft, then push edges back inside. For dislocated limb bones, if only one bone is broken, it may be treatable; if both are broken, it is incurable.
For severe injuries, generally traction and alignment are required, or surgical reduction followed by binding and splinting. Traction should be applied near the original injury site, not far from the affected bone segment.
When performing traction, observe how the bones on left and right are displaced. There is straight traction and oblique traction.
For left and right injuries, examine the bone gaps, carefully palpate and press to estimate the situation. The goal is to return the bone to its original position and press the skin against the bone.
To identify the injury site, simply palpate whether the bone is flat and aligned.
Traction can be performed by one person or two to three, depending on difficulty.
If skin is broken and bone is displaced, and traction fails to reduce it, press close to the gap. Use a sharp knife to remove some tissue to allow reduction without deviation. If manual traction fails, cutting rotting flesh may allow reduction. Once reduced, apply Heilong San (Black Dragon Powder) around the swelling, leaving the wound open to be filled with Fengliu San (Wind-Flow Powder). The knife used must be very sharp; excision knives or carving knives are suitable.
For comminuted fractures, assess the local alignment. Generally, depression indicates no fracture; check laterally for displacement. Perform traction and alignment, apply medicine and bandage, ensuring flatness and correctness.
Splints should be removed after three days (four to five days in winter). Soak in hot medicinal water to wash off old medicine without disturbing the injury. Reapply Heilong San (Black Dragon Powder) and splint. This is generally for severe injuries.
Swelling is caused by blood stasis. Wash with hot medicinal water and apply Heilong San (Black Dragon Powder).
For severe open wounds, fill with Fengliu San (Wind-Flow Powder) and apply more. For non-open areas, apply Heilong San (Black Dragon Powder) and splint with fir bark. Have someone hold cold water in their mouth and spray it suddenly to cause shock, helping reduction, then suture with silver wire and apply herbs (take internal medicine for the head/upper body).
For severe skull fractures in adults or children, do not use raw herbs; apply Hemostatic Powder (Zhixue San) to stop bleeding. (Take internal powdered medicine for safety).
For initial facial/head abrasions with bleeding, apply four-herb paste. Internally take powdered medicine plus three fen of Jiegu Dan (Bone-Knitting Pill).
For initial mid-body injuries, first take powdered medicine plus five fen of Jiegu Dan (Bone-Knitting Pill). Mild cases take decoction after seven days; severe cases take the previous medicine after fourteen days.
For initial lower body injuries, treat initially with powdered medicine as for the mid-body.
For sudden sprains, apply external plaster and take powdered medicine.
Cranial fractures are treatable on the main skull bone, but fractures at the temporal region (Taiyang acupoint) are fatal and incurable. Another method for treating head injuries: apply powdered medicine to the mouth exterior, stick umbrella paper to the head, and tightly bind the injury with fir bark.
For shoulder blade dislocation, assess the treatment method. Use a chair to encircle the ribs, cover with soft clothes and cotton. One person holds steady, two perform traction, drop the wrist, flex the arm and wrist, and bind with silk cloth.
For Jinjing bone (rib/costal cartilage) below the ribs, if injured, do not splint. Must press flat and stable, apply Heilong San (Black Dragon Powder) evenly, and bind with silk cloth. The same applies to bilateral rib injuries.
For hip dislocation posteriorly, two people can stabilize the leg for traction, then use foot pressure to reduce. If dislocated anteriorly into the groin, it cannot be managed.
For hand bone dislocations, observe direction. If displaced left, pull right; if right, pull left.
When performing traction and alignment, cushion with soft material like silk cloth.
For embedded bone fragments under the skin, apply only Heilong San (Black Dragon Powder). Later, skin and flesh will necrose, and the bone will naturally expel itself, leading to healing.
For broken bones or severed tendons with open wounds, fill with Fengliu San (Wind-Flow Powder). Suture the surrounding skin with needle and thread, then apply Heilong San (Black Dragon Powder).
For splinting, use fir bark strips finger-width wide, arrange evenly around, and tie tightly with rope for one or two rounds. Ensure tight binding.
For flat bone fractures with intact skin, apply medicine patches. Avoid splinting curved joints to prevent stiffness; instead, use black silk wrapping to allow movement. Patients should maintain voluntary motion. For severed fingers, bind with ramie hemp; for legs, use ramie rope coin-sized.
For applying medicine, spread Heilong San (Black Dragon Powder) mixed with water on leather or oiled paper over a board, then roll onto the injury.
Soak fir bark strips finger-width wide, arrange sparsely, and tie tightly with small cord three times around. Change dressing and wash every three days as before.
For joints requiring flexion/extension (e.g., wrist, fingers), apply medicine, wrap in silk, and move frequently. Movement allows extension, and extension allows natural flexion.
Injuries within one month can still be treated; after that, they are incurable.
Avoid eating certain herbs during injury recovery, as they may prevent proper bone realignment.
For intestinal/abdominal blood stasis from falls, take dispersing powders such as Siwu Tang (Four Substances Decoction).
Do not immediately take Jiegu Dan (Bone-Knitting Pill) for constipation/dysuria after trauma, as these formulas are often warm/hot and taken with wine, exacerbating dryness. Try Siwu Tang (Four Substances Decoction) first. Also take Dacheng Tang (Great Achievement Decoction) plus Mu Tong (Akebia Caulis). If bowels remain blocked, add Poxiao (Mirabilite). After passage resumes, take Jiegu Dan (Bone-Knitting Pill).
For severe injuries, before taking trauma medicine, first take Qi-regulating medicines like Hejun Qi San (Harmonizing Qi Powder).

Store pill and powder medicines in jars in the kitchen, dried over fire.
Avoid moxibustion/fire therapy for injuries; doing so renders medicine ineffective. Be cautious! Dahuoxue Dan (Major Blood-Invigorating Pill) is better with them, but usable without. Can substitute with Liquidambar Resin (Fengxiang) for Frankincense.
