This chapter discusses the critical points where needling is prohibited; all practitioners of acupuncture should review it repeatedly.
The Yellow Emperor asked, "I wish to hear about the specific prohibitions." Qibo replied, "The Zang organs have critical areas that must be carefully examined. The Liver Qi arises on the left, the Lung Qi stores on the right, the Heart Qi distributes on the exterior, and the Kidney Qi governs the interior. The Spleen acts as the transporter, and the Stomach serves as the market for food and drink. Above the diaphragm and huang (a region between the heart and stomach), there are the 'parents' (Heart and Lung). Beside the seventh vertebral segment, there is the 'small heart' (referring to the Mingmen or Pericardium). Following these rules brings fortune, while violating them brings disaster.
If the needle strikes the Heart, death occurs in one day, with belching (Yi) as the manifestation. If it strikes the Liver, death occurs in five days, with incoherent speech (Yu) as the manifestation. If it strikes the Kidney, death occurs in six days, with sneezing (Ti) as the manifestation. If it strikes the Lung, death occurs in three days, with coughing (Ke) as the manifestation. If it strikes the Spleen, death occurs in ten days, with abnormal swallowing (Tun) as the manifestation. If it strikes the Gallbladder, death occurs in one and a half days, with vomiting (Ou) as the manifestation. If the large vessel on the dorsum of the foot is pierced and bleeding does not stop, death ensues. If the flowing vessels on the face are pierced, blindness may unfortunately result. If the Naohu (DU16) point on the head is pierced and the needle enters the brain, death is immediate. If the vessels under the tongue are over-punctured and bleeding does not stop, aphonia results. If the collateral vessels distributed on the sole of the foot are pierced without allowing blood to escape, swelling occurs. If the large vessel in the abdomen is pierced, the person falls down with a pale complexion. If the large vessel at the Qichong (ST30) area is pierced and blood does not escape, swelling resembling a crouching rat occurs in the groin. If the marrow in the spinal interspace is pierced, kyphosis (hunchback) results. If the upper breast is pierced and the mammary gland is injured, root erosion and swelling occur. If the Quepen (LI12) point is needled too deeply, causing Qi leakage, wheezing, coughing, and counterflow Qi occur. If the Yuji (LU10) area on the hand is needled too deeply, swelling occurs.
Do not needle someone who is heavily intoxicated, as this causes Qi chaos. Do not needle someone who is extremely angry, as this causes Qi counterflow. Do not needle someone who is severely fatigued, newly full after eating, severely hungry, severely thirsty, or greatly startled. If the large vessel in the medial thigh is pierced and bleeding does not stop, death ensues. If the Chizhu (ST32) point is needled too deeply into the vessel, internal leakage and deafness result. If fluid escapes from the knee patella upon needling, limping results. If the vessel of the Hand Taiyin Lung Meridian is punctured with excessive bleeding, immediate death occurs. If the vessel of the Foot Shaoyin Kidney Meridian is punctured in a case of deficiency-emptiness with bleeding, tongue stiffness and difficulty in speech result. If the Yingzhong (chest) area is needled too deeply injuring the Lung, wheezing, counterflow, and仰息 (breathing with difficulty lying on the back) occur. If the elbow is needled too deeply, Qi gathers there, resulting in inability to flex or extend the arm. If the area three cun below the medial thigh is needled too deeply, urinary incontinence results. If the axillary and costal interspace is needled too deeply, coughing results. If the Bladder in the lower abdomen is pierced causing urine leakage, abdominal fullness results. If the intestines are needled too deeply, swelling occurs. If the vessel within the bone above the eye orbit is pierced, lacrimal fistula and blindness result. If fluid escapes from the joints upon needling, inability to flex and extend results.
