The lifting and thrusting technique proposed by Zhenren Qiongyao is most profound, allowing Qi to circulate freely through the twelve meridians of the body, encompassing sixty-six specific acupoints and meridian systems, applicable for needling at any of the twelve two-hour periods (Shichen) in a day.
For cold bi-syndrome (Cold Bi), one may needle the Shenshu (BL23) and Zusanli (ST36) acupoints and apply the lifting method; Yinlingquan (SP9) is suitable for pain in the umbilical and abdominal regions. Jingwell (GB21) should only be needled with Hand Sanli (LI10) to treat neck and shoulder disorders; after the Jiaji (EX-B2) points, Zhongzhu (TE3) can be treated with scraping; Yinlingquan (SP9) undoubtedly treats hypochondriac pain; Daling (PC7) can primarily manage chest and heart pain; Weizhong (BL40) must be known for treating waist and leg conditions.
Before needling, warm the needle with one's mouth to make it warm, following the patient's breathing rhythm to guide the Qi. Press the acupoint with fingernails to facilitate insertion; this is the secret to painless needle entry. If the patient feels pain, apply the draining method; if they feel itching or numbness, apply scraping; plucking/acupoint stimulation should be performed first during exhalation and later during inhalation. The key to applying lifting and thrusting lies here: recognizing the meridians yields extraordinary efficacy. In spring and summer, when the body is lean, shallow needling is appropriate; in autumn and winter, when the body is fat, deep needling is appropriate.
Rubbing, advancing, and rotating techniques must distinguish between Yin and Yang (Zi and Wu hours); the rotation method for the abdominal region must be understood.
Cold conditions require heat methods, and heat conditions require cold methods; three lifts and three thrusts have distinct mechanisms: tight lifting and slow thrusting feels like ice water, while slow lifting and tight thrusting feels like a fire pool. One must recognize the difference between rubbing/advancing and lifting/thrusting: following the meridian direction tonifies the left side for warmth suitability; opposing the meridian direction drains the right side for cold drainage. When Qi arrives, it feels like a boatman steering the rudder, powerful as a bow releasing an arrow; when Qi arrives, it feels like a fish swallowing bait; when Qi has not arrived, it feels deep and secluded. Use fingers to press around the acupoint to await Qi; hand movements should be cautious like holding a tiger, respectful towards the patient. Fast Qi arrival brings fast results; slow Qi arrival brings slow results. The lifting, thrusting, and retention techniques of Zhenren Qiongyao are true secrets, revealing the divine mechanism of rapid efficacy. While *Suwen* and *Nanjing* differentiate tonification and drainage, few know this lifting and thrusting method. Throughout the four seasons, when humans contract pathogens, the decision on following/opposing movement stems from inquiry. Observe form and complexion to determine depth; apply needles slowly. Advancing three cun allows for lifting and thrusting; those using five cun should deduce carefully. Rare secrets of the world are little known; Ying and Wei Qi are retained first and used subsequently.
Lifting and thrusting are essential in spring, summer, autumn, and winter; effectiveness on left and right depends on mental focus. Divine practitioners first circulate Qi up and down; sitting needling must suit all appropriate areas. Upon reaching the skin layer, distinguish depth; know the sequence and do not apply needling recklessly. Whether twisting, plucking, or rubbing, one must carefully deliberate with intent. Following exhalation according to method constitutes tonification; following drainage according to method leaves no doubt. During tonification, the thumb pushes forward; during drainage, the thumb moves backward. For Yang persons, the thumb pushes forward to the nail; for Yin persons, pushing backward is similar. Tonification and drainage timing are thus established; male left and female right are known since ancient times. To perceive Qi ascent and understand lifting/thrusting, effects become miraculous shortly after needling; when lifting the needle, Qi arrival causes trembling; sudden heaviness indicates this mechanism. Exiting Yang and entering Yin should follow respiration; exiting and entering Yin and Yang must not be violated. This is the Ten-Section Brocade of Qiongyao, a lifting and thrusting secret worth more than a thousand gold.
