Hongjiao (Gout/Charcot Foot) ○ This disease arises from infiltration by wind, cold, summerheat, and dampness, or from damage to the Spleen and Stomach due to excessive alcohol consumption and rich flavors, leading to damp-heat pouring downward into the Liver and Kidney. The disease typically begins with hidden nodular pain at the Qichong (ST30) acupoint.
If dampness prevails, the sinews and vessels become flaccid and relaxed, accompanied by edema. There is a sensation of heaviness without upward movement, and all four limbs are cold. Treatment should employ pungent-warm dispersing methods, followed by draining and eliminating dampness.
If heat prevails, the sinews contract and curl, muscles become withered and thin without swelling, all four limbs are hot, and sometimes there is upward rushing. Treatment involves clearing fire and descending heat.
When dampness and heat contend, prevailing dampness causes aversion to cold, while prevailing heat causes high fever. If accompanied by headache and body pain resembling Taiyang disease (Exterior Syndrome), it differs in that it initially presents with hot swelling in the feet and knees, or weakness preventing movement.
If Hongjiao attacks upward to the chest and diaphragm, causing panting, incessant vomiting, spontaneous sweating, and a short, choppy pulse, the prognosis is fatal. If the pathogen enters the Heart, causing mental confusion, lower abdominal bi-sensation and distension, and a left Cun pulse that fluctuates between large and small, the prognosis is fatal. If it enters the Kidney, causing swelling of the waist and feet, urinary retention, blackening of the forehead, and chest fullness, with an absent left Chi pulse, the prognosis is fatal. If critical signs appear but the pulse has not yet ceased, prepare Fuzi (Aconiti Lateralis Radix Preparata) powder, mix with saliva, and apply to Yongquan (KI1).
Another method: Needle Jianjing (GB21), Zusanli (ST36), and Taichong (LR3).

Tendon Cramp
For tendon cramps in the ten fingers of the hand, moxa seven cones above the ankle bone of the hand.
For tendon cramps in the tibia, moxa three cones on the tendon below the knee joint.
For abdominal distension with tendon cramps, moxa fourteen cones one cun above the umbilicus.
Foot tendon cramps: Chengshan (BL57), above the ankle. (For internal tendon urgency, moxa the medial malleolus; for external tendon urgency, moxa the lateral malleolus.)
Wei Syndrome (Flaccidity Syndrome)
Weakness of sinews and bones, without pain or itching, is called Wei; weakness of the feet preventing walking is called Fei. It is caused by the depletion of the Five Viscera, leading to weakness of blood vessels, sinews, bones, and muscles, resulting in an inability to move. The presentation resembles Roufeng Hongjiao (Soft Wind Gout). That condition is due to excess wind-cold pathogens, thus presenting with pain and swelling. Wei syndrome belongs to deficiency of blood and qi, characterized only by inability to use without pain. Do not confuse it with treatment for wind syndromes (with supplementary additions).
Acupoints: Huantiao (GB30) (Retain needle until Qi arrives, wait two shi periods before removing), Zhongdu (GB39), Zusanli (ST36).
