(With supplementary additions)
Hernia (Shan) belongs to the Liver Meridian. Damp-heat, phlegm, and blood stasis flow downward due to deficiency, causing disease. Additionally, external cold constrains the Qi, preventing its free flow, leading to contraction and traction of the tendons and vessels, resulting in pain. Or, due to unregulated alcohol consumption and sexual activity, turbid Qi flows into the lower body. It may be triggered by overwork or exposure to cold, with attacks occurring at specific times. Some present as palpable masses in the lower abdomen that cannot dissipate immediately. This condition takes damp-heat as the branch (manifestation) and Kidney deficiency as the root. Symptoms may include a mass shaped like a melon or sounds like a frog croaking. There are cases where lower abdominal pain radiates to the testicles, and cases where pain is located on one side of the lower region. Treatment for damp-heat must distinguish the proportion of dampness versus heat. Exposure to heat causes the penis to stand erect and flaccid without retracting; exposure to cold causes traction and pain; exposure to dampness causes swelling and a sensation of bearing down.
Acupoints: Ganshu (BL18), Qihai (CV6), Guanyuan (CV4), Zhongji (CV3), Sanyinjiao (SP6), Wailing (EX-LE7) (located 1.5 cun left and right of the umbilicus; moxibustion here cures hernia instantly and prevents recurrence permanently), Guilai (ST29), Dadun (LR1), Xingjian (LR2), Taichong (LR3), Lanmen (EX-LE10, also known as Quanyin).
One method: Moxibustion on the blue vein three cun lateral to Guanyuan (CV4), seven moxa cones (zhuang), results in immediate cure (if the left side is affected, treat the right side; if the right side is affected, treat the left side).
Another method: Instruct the patient to close their mouth. Measure the distance between the corners of the mouth horizontally with a straw as one fold. Add two more folds of this length to the measurement. Bend it into a triangle shape resembling the symbol "△". Place the upper apex at the center of the umbilicus, and place the two lower apices at both sides below the umbilicus. The locations of these two lower apices are the acupoints. If the left side is affected, treat the right side; if the right side is affected, treat the left side; if both sides are affected, treat both points. Use moxa cones the size of a wheat grain. Moxibustion of fourteen zhuang, or twenty-one (three times seven) zhuang, yields miraculous effects.
Swelling and severe pain of the glans penis constitute acute hernia (Cu Shan). Pain in the female genitalia should be treated by needling Dadun (LR1) and Xingjian (LR2).
Spermatorrhea: Emission of semen during sexual dreams is called dream spermatorrhea (Meng Yi). Emission of semen without dreaming is called slipperiness spermatorrhea (Hua Jing).
Acupoints: Gaohuangshu (BL43), Shenshu (BL23), Zhongji (CV3) (Moxibustion dosage follows the patient's age in years), Sanyinjiao (SP6), Ququan (LR8) (effective for accompanying cold pain in the knees and shins), Zhongfeng (LR4).
Additionally, there are two Jinggong points, located 1.3 cun lateral to the lower border of the fourteenth thoracic vertebra. Moxibustion of seven zhuang is effective.
White Turbidity (Bai Zhuo):
Acupoints: Shenshu (BL23), Guanyuan (CV4), Zhongji (CV3).
Impotence (Yang Wei): This is a syndrome of deficiency-cold in the Kidney and Bladder.
Acupoints: Shenshu (BL23), Qihai (CV6) (Extensive moxibustion is beneficial).
Incontinence of Urine: This refers to frequent urination without awareness. It is generally caused by reckless movement of Fire pathogenic factors in the Bladder, disturbing the water, leading to incontinence and frequency. Treatment should tonify Kidney Yin and nourish Blood, focusing on draining Fire pathogenic factors. Nocturnal enuresis in sleep is a deficiency syndrome. Infants have immature Bladder Qi, and the elderly have insufficiency of the Lower Yuan; both suffer from this. However, infants often have accompanying Heat, while the elderly often have accompanying Cold; this distinction must not be overlooked.
Acupoints: Qihai (CV6) (Effective for pediatric nocturnal enuresis), Guanyuan (CV4), Yinlingquan (SP9), Dadun (LR1).
Five Types of Lin Syndrome:
Qi Lin involves difficult urination with residual urine.
Shi Lin (Stone Lin) involves pain in the urethra and urination resembling sand/gravel (also known as Sha Lin).
Xue Lin (Blood Lin) involves hematuria, triggered by heat.
Gao Lin (Paste Lin) involves discharge resembling paste/grease.
Lao Lin (Labor Fatigue Lin) is triggered by physical fatigue.
(Supplementary addition: Pain radiating to the Qichong point).
Acupoints: Jianshi (PC5), Qihai (CV6), Guanyuan (CV4), Shimen (CV5), Yinlingquan (SP9).
One method: Fill the umbilicus with hot-fried white salt, apply moxa cone, seven zhuang. Alternatively, moxibustion on Sanyinjiao (SP6) leads to cure.
Urinary Retention (Bi Long): Bi means obstruction/blockage. Long refers to dribbling/straining.
Acupoints: Xiaochangshu (BL27), Yinjiao (CV7) (Located above the Bladder, hence moxibustion here), Yinlingquan (SP9).
Constipation:
Acupoints: Zhangmen (LR13), Taidai (SP4), Zhaohai (KI6).
Constipation due to Spleen Deficiency:
Acupoints: Sanyinjiao (SP6) (Moxibustion thirty zhuang), Shangqiu (SP5).
Rectal Prolapse: This is caused by deficiency of Qi and Blood leading to sinking.
Acupoints: Shenque (CV8) (Moxibustion dosage follows the patient's age in years), Changqiang (GV1) (Three zhuang), Shuifen (CV9) (Moxibustion hundred zhuang; treats diarrhea and rectal prolapse).
If accompanied by damp-heat, use five times the amount of Chinese gallnut (Wubeizi) and alum (Mingfan), each three qian, ground into powder. Boil in two bowls of water. Wash with the hot solution for immediate retraction. For prolapse of three to five cun, after washing, apply powder of red kaolin (Chishizhi), support the anus with oiled paper, and sprinkle around the perimeter for excellent results.
Blood in Stool (Bian Xue): The pathology lies in the Stomach and Large Intestine, hence named Wind in the Intestines (Chang Feng) or Internal Toxicity (Zang Du). Blood appearing before stool is called proximal blood (Jin Xue); blood appearing after stool is called distal blood (Yuan Xue). Both are caused by damp-heat pouring downward.
Acupoints: Zhongwan (CV12), Qihai (CV6) (For all cases of blood loss with pale complexion, poor appetite, weak/soggy pulse, and cold extremities, moxibustion on these two points is excellent).
One method: For any case of blood in stool unresponsive to other treatments, locate the prominent, tender spot on the spine level with the umbilicus. Moxibustion of seven zhuang stops it. If it recurs, moxibustion of another seven zhuang roots out the problem permanently. For epistaxis and all other bleeding disorders unresponsive to treatment, moxibustion ensures no recurrence.
Another method: Moxibustion following the patient's age in years at the lower border of the twentieth thoracic vertebra in the midline.
Hemorrhoids and Fistula (Zhi Lou): Hemorrhoids that ulcerate are called hemorrhoidal fistulas. Constipation inevitably causes severe pain.
Acupoints: Erbai (EX-UE2) (Four cun posterior to the wrist crease), Changqiang (GV1), Chengshan (BL57), Fuliou (KI7), Shangqiu (SP5).
Additionally, moxibustion one cun lateral to the lower border of the fourteenth thoracic vertebra is effective for treating intestinal wind and various hemorrhoids.
