Mafan Shigao (Wheat Meal Stone Paste) treats the disease of back carbuncle (back furuncle/abscess), with effects so miraculous that they are beyond comparison. It is a pity that few people in the world know it. However, the dosage and processing methods recorded in ancient prescriptions are brief and not detailed, leading to impure medicinal materials and rough, simplified preparation. This causes ineffectiveness on one's own part, rather than an error in the prescription itself. I personally witnessed a nobleman suffering from this disease. The physician applied Mafan Shigao for topical application, which was not only ineffective but also increased pain; furthermore, caustic drugs were used to corrode it, yet pus did not discharge, causing day-and-night pain and preventing rest. The wound expanded daily, spreading to both flanks, and several carbuncles appeared all over the throat, feet, and knees. Physicians all considered it untreatable. Although I had prepared this medicine, carefully selected and strictly processed it to be excellent and refined, I dared not use it immediately. Waiting until other medicines failed, I tried using it. Within one night, the pain ceased completely, pus and blood discharged profusely like rushing water, and the patient slept peacefully. Everyone was astonished, believing it to be another divine medicine, without realizing it was merely Mafan Shigao. For carbuncle diseases, it is necessary to have abundant discharge of pus and blood, so that even if toxic factors exit with the pus and blood, internal damage to the Zang-Fu organs is avoided, allowing the patient to recover. A mediocre physician, seeing that pus did not discharge, drilled two tin tubes intending to insert them into the carbuncle and suck out the pus with his mouth. I stated: Using this method, the patient certainly cannot be saved! I strongly opposed this idea. Another suggestion was to make a hole in a straw mat, allowing the patient to lie supine to drain the pus. This advice could not be dissuaded, so I allowed him to try, but no pus came out. Later, my prepared Mafan Shigao finally achieved efficacy. Since then, villagers suffering from this disease came to ask about it. Therefore, I recorded this prescription, urging careful selection and compounding to achieve complete success, which is why I have detailed it here.
1. When a carbuncle or furuncle first appears, Mafan Shigao should be applied around the periphery to protect its base/root, preventing spread, while leaving a coin-sized opening in the center to allow toxic factors to exit. If the swelling gradually subsides, adjust the application size accordingly. Continue until after the carbuncle ruptures and pus discharges, when the wound shrinks to about one cun in diameter. At this point, apply Shenyi Gao (Divine Miracle Ointment) topically to promote healing and contraction, followed by Mafan Shigao.
Shenyi Gao (Divine Miracle Ointment) Treats all types of skin sores and scabies (Note: This formula was originally missing; added now from Wei Yilin's *Defang Fang*).
Scorpio (Quanxie, 全蝎) (7 pieces, remove venom)
Gleditsia Sinensis Fruit (Zaojiao, 皂角) (one piece, crushed)
Croton Seed (Badou, 巴豆) (7 kernels, remove shell)
Cnidium Monnieri Fruit (Shechuangzi, 蛇床子) (3 qian)
Clear Sesame Oil (Qingyou, 清油) (1 liang)
Beeswax (Huangla, 黄蜡) (0.5 liang)
Calomel (Qingfen, 轻粉) (half spoon)
Realgar (Xionghuang, 雄黄) (separately ground, 3 qian)
First, decoct Gleditsia Sinensis Fruit, Scorpio, and Croton Seed in oil until the color changes, then remove these three ingredients. Add Beeswax and melt it. Take out and place in a cool area. Add Realgar, Cnidium Monnieri Fruit powder, and Calomel, mix evenly to form an ointment. First wash with Sophora Flavescens Decoction (Kushen Tang, 苦参汤) warm, then rub the medicine onto the sores/scabies. It is highly effective.
Additionally: Shenyi Gao (This formula added from *Chuishui Yuanzhu*)
Phellodendron Bark (Huangbai,
Apricot Kernel (Xingren,
Scrophularia Root (Yuanshen/Xuanshen, 元参/玄参) (each 1 liang)
Use 2 jin of Sesame Oil, decoct until dark black. Add 5 qian of Snake Slough (Shetuo, 蛇蜕), 1 liang of Honeycomb (Fengfang, 蜂房), and a ball of human hair (from a male) the size of a chicken egg. Decoct again until black. Remove dregs. Slowly add Lead Tetroxide (Huandan, 黄丹), amount unspecified, until consistency is moderate. Simmer over mild and strong heat to collect the ointment.
