(Mingjing [spirit and clarity] is lost, and evil qi acts recklessly and abnormally.) When the yin contracts and tendons spasm, (The Liver meridian passes through the genitalia, and the Liver governs the tendons.) The bones of both hypochondria cannot be lifted, (The hypochondria are the domain of the Liver; when the Liver fails, they cannot be lifted.) Hair becomes withered and complexion appears sallow, death occurs in Autumn. Excessive anger that does not subside injures Zhi [will/knowledge], (Anger is originally the emotion of the Liver, but it also injures the Kidney because the Qi of the mother [Water] and child [Wood] organs communicates.) When Zhi is injured, one tends to forget previous words, and the waist and spine cannot flex or extend. (The waist is the mansion of the Kidney, and the spine is the pathway of the Kidney.) Hair becomes withered and complexion appears sallow, death occurs in Late Summer. Unresolved fear injures Jing [essence], (Fear causes Qi to sink and collapse.) When Jing is injured, there is bone soreness, flaccidity, and cold limbs, (Both Wei [flaccidity] and Jue [cold limbs/reversal] symptoms indicate Kidney injury.) and essence leaks spontaneously. Therefore, the Five Zang organs store essence and must not be injured; if injured, they lose their hold, leading to Yin deficiency, (Each Zang organ has its own essence; injuring them leads to Yin deficiency, as the essence of all five Zang organs is Yin.) Yin deficiency leads to a lack of Qi, (Because essence can transform into Qi.) and without Qi, death ensues. (When Qi scatters, death occurs; however, the scattering of Qi results from the exhaustion of Yin essence, so those who practice health preservation should value this greatly.)
Diseases of the Heart manifest as a curled and short tongue, and red cheekbones. Diseases of the Lung manifest as panting and flaring nostrils. Diseases of the Spleen manifest as yellow lips. Diseases of the Liver manifest as a bluish complexion. Diseases of the Kidney manifest as black cheekbones and forehead. When Evil resides in the Heart and Lung, its Qi stagnates in the two axillae. (Heart and Lung belong to the Hand meridians.) When Evil resides in the Spleen, its Qi stagnates in the two thighs (Bi). When Evil resides in the Liver, its Qi stagnates in the two armpits. When Evil resides in the Kidney, its Qi stagnates in the two groins. (Liver, Spleen, and Kidney belong to the Foot meridians.)
Whenever Evil enters a deficiency, (i.e., enters the Xigu [valleys]. These refer to the gaps between sinews and bones, where Qi and Blood flow and circulate.) these are chambers of joints and mechanisms, where True Qi passes and Blood collaterals roam. Evil Qi and foul blood should not stay; if they do, they damage the collaterals, bones, joints, and mechanisms, preventing flexion and extension, thus causing spasms. Excess Wind causes movement, (Excess Wind refers to diseases characterized by tremors and shaking.) Excess Heat causes swelling, (Excess Heat refers to diseases such as erysipelas and carbuncles.) Excess Dampness causes loose stools, (Excess Dampness inevitably invades the Spleen and Stomach, causing indistinct separation of water and grain, resulting in loose stools.) Excess Dryness causes dryness, (Excess Dryness refers to diseases where body fluids are depleted, causing internal and external dryness.) Excess Cold causes edema. (Excess Cold means Yang Qi does not move, causing distension, fullness, and floating emptiness.)
Spring Qi diseases affect the head, (Due to the rising of Yang Qi.) Summer Qi diseases affect the Zang organs, (Referring to the Heart, which corresponds to Summer Qi and is the master of all Zang organs.) Autumn Qi diseases affect the shoulders and back, (This corresponds to the Lung.) Winter Qi diseases affect the four limbs. (The Qi of the four limbs is thin and easily invaded by Evil.)
If the Sea of Water and Grain [Stomach] is in excess, there is abdominal fullness; (Water and grain stagnate in the middle.) If the Sea of Water and Grain is deficient, there is hunger but an inability to accept food. (Stomach deficiency fails to receive, Spleen deficiency fails to transport.) If the Sea of Marrow is in excess, the body feels light and strong with excessive strength, exceeding normal limits; (Naturally possessing superior capacity.) If the Sea of Marrow is deficient, there is dizziness and tinnitus, (Empty brain causing dizziness, feeling like rotation. Marrow deficiency implies Essence decline, Yin deficiency causes tinnitus.) leg soreness, vertigo, and fainting, (Empty marrow causes weakness and leg soreness; Essence decline causes Qi loss, leading to vertigo and unconsciousness.) inability to see, lassitude, and a desire to lie down. (All due to the decline of Essence and Qi.)
If Upper Qi is insufficient, the head feels empty and full, ears suffer from ringing, the head feels heavy and difficult to keep upright, (Heavy and unable to remain stable,) eyes feel dizzy; if Middle Qi is insufficient, urination and defecation become abnormal, (Water metabolism relies on Qi transformation. Urine may be dark yellow or scanty and astringent, often caused by excessive sexual activity, fatigue, and over-injury to Essence and Qi; regarding it all as Fire is incorrect.) intestines suffer from painful rumbling; if Lower Qi is insufficient, Wei [flaccidity] and Jue [reversal] occur, along with chest oppression. (The Classic states that if Kidney Qi is hot, the waist and spine cannot lift, marrow decreases, and feet cannot support the body, developing into Bone Wei. Jue is divided into Cold and Heat; Yin decline below leads to Heat Jue, Yang decline below leads to Cold Jue. Failure of ascending and descending to interact causes Heart Qi to be unsmooth, resulting in oppression.)
Why does Spleen disease cause the four limbs to be unusable? It is said: The four limbs rely on Stomach Qi but cannot reach the meridians directly; they must rely on the Spleen to receive it. Now, Spleen disease prevents the transmission of fluids from the Stomach, so the four limbs do not receive Water and Grain Qi. Qi declines daily, meridians become obstructed, sinews, bones, muscles, and flesh have no Qi to generate, thus becoming unusable.
Those attacked by thieving winds and empty evils are affected first by Yang; those with irregular diet and起居 (daily routine) are affected first by Yin. Yang invasion enters the Six Fu organs, Yin invasion enters the Five Zang organs. Entering the Six Fu organs causes fever and inability to sleep at proper times, (Inability to sleep on time,) upper manifestations include panting; entering the Five Zang organs causes chest fullness and obstruction, lower manifestations include undigested diarrhea, and prolonged cases develop into Chang Pi (dysentery). Yang diseases ascend to the extreme then descend; Yin diseases descend to the extreme then ascend. Those injured by Wind are affected first above; those injured by Dampness are affected first below.
Yang deficiency leads to exterior cold, (Yang receives Qi from the Upper Jiao to warm the skin and muscle intervals; Yang deficiency lacks the ability to defend the exterior, so even without pathogenic invasion, there is aversion to cold.) Yin deficiency leads to interior heat, (Loss of control by the Five Zang injures Essence; Essence injury leads to Water deficiency failing to control Fire, generating interior heat.) Yang excess leads to exterior heat, (Interstices close, pores block, Defensive Qi cannot vent, meaning "when humans are injured by Cold, the disease becomes Heat," which is an exterior contraction syndrome.) Yin excess leads to interior cold. (Warm Qi departs, Cold remains alone, hence interior cold.)
Blood and Qi prefer warmth and dislike cold; cold makes them astringent and unable to flow, warmth makes them dissipate and move. (Dissipating and circulating.) Cold Qi lodging in the Small Intestine prevents proper accumulation, (Yang Qi fails to transform, water and grain cannot stay,) thus causing diarrhea and abdominal pain; Heat Qi lingering in the Small Intestine causes intestinal pain, stifling heat, scorched thirst, then hard dry stool cannot pass, thus causing pain and obstruction.
Where Evil gathers, Qi must be deficient. (Evil enters only due to deficiency.) Wind is the chief of a hundred diseases; upon changing, it becomes other diseases. (Thus it is said Wind is the beginning of a hundred diseases.) It has no fixed formula, yet its origin is always Wind Qi. (Speaking of its many changes, the cause of all is Wind Qi.)
Lao Feng [Labor-induced Wind], (Wind contracted due to labor.) Its method lies below the Lung, (When humans labor, they pant; labor moves the Lung, this is known.) Its disease makes people stiff neck, blind vision, (Evil below the Lung causes panting and counterflow, making the neck stiff and unable to bow; Wind-Heat congests upward, causing aversion to wind and photophobia, making people squint.) expectorate phlegm-like mucus, (Wind-Heat injures Yin, making body fluids thick and turbid.) aversion to wind and chills. (Lung governs skin and hair; Defensive Qi is injured.)
It is asked: How to treat? Qibo says: Rescue the ability to bend and extend. (Warming the Lung disperses Wind, bending and extending become safe; if stagnation turns to Heat for a long time, then clear and resolve.) If Taiyang draws Essence upward, three days; middle-aged persons, five days; those lacking Essence, seven days. (Essence decline makes drawing Essence slow.) Cough up green-yellow sputum, resembling pus, size of a pill, exiting from mouth and nose; if not expelled, it injures the Lung, injuring the Lung leads to death. (This is what is called dry cough, difficult to treat.)
There is a disease with body heat, (Damp-Heat injures sinews.) sweating like bathing, (Damp-Heat steaming.) aversion to wind and shortness of breath, (Sweating makes Defensive Qi deficient, hence aversion to wind; Defensive Qi deficiency causes Qi leakage, hence shortness of breath.) What disease is this? It is named Jiu Feng [Wine Wind]. (Disease caused by wind after drinking.)
Facial swelling is called Feng [Wind], (Yang receives Wind Qi.) Swelling of calves is called Shui [Water]. (Yin receives Dampness.)
What is meant by changes after disease formation? (Formation refers to the root of disease, change refers to the branch; different root and branch is called change.) It is said: Wind becomes Cold and Heat, (Wind is Yang Evil, good at moving and changing; if merging inward, Yang deficiency leads to exterior cold; if merging outward, Yang excess leads to exterior heat. Also: Exposure to wind generates cold and heat.) Zhang [stifling heat] becomes Xiao Zhong [excessive eating/thirst], (Zhang Heat accumulates in the Stomach, causing easy hunger.) Jue [reversal] becomes Dian Ji [head diseases], (Jue means reversal. Qi reverses upward, causing pain, vertigo, and falling, forming head diseases.) Long-term Wind becomes Sund Xie [undigested diarrhea], (Long-term Wind means Wood overacts on Earth, causing diarrhea with undigested food.) Mai Feng [Pulse Wind] becomes Li [leprosy], (Cold-Wind lodges in blood vessels, staying long, causing internal decay of skin and flesh, becoming Leprosy.) Changes of disease are countless. (Not just these few points.)
A hundred diseases arise from Qi: Joy makes Qi slacken, (Slacken gradually leads to dispersion, hence sudden joy injures Yang. Also: Joy causes spirit and essence to scatter and not store.) Grief makes Qi consume, (Grief injures Heart and Lung, Ying and Wei do not disperse, heat stays in the middle, consuming Qi.) Thought makes Qi knot, (Thought concentrates Qi, causing it to stay and not disperse.) Anger makes Qi rise, (Qi counterflows, Blood rises, hence vomiting blood due to anger.) Fear makes Qi sink, (Fear injures Kidney, essence leaks spontaneously, is this not a sign of Qi sinking?) Startle makes Qi chaotic, (Great startle and sudden fear cause spirit and will to wander, Blood and Qi to scatter.) Labor makes Qi exhaust, (Labor causes panting and sweating, internal and external escape, exhausting Qi.) Cold makes Qi gather, (Cold binds externally, Yuan gates close tightly, Yang Qi converges internally and cannot disperse.) Heat makes Qi leak. (Heat opens interstices, sweat pours out, Qi escapes with sweat.)
There is a disease with chest and hypochondriac support-fullness, (Fullness like supporting隔板.) hindering food intake, upon onset first smelling fishy and musky odors, (Odors: Lung Qi is fishy, Liver Qi is musky; turbid Qi of Lung and Liver counterflows upward, hence smelling these odors.) flowing clear fluid, (Lung deficiency causes clear nasal discharge, Spleen deficiency causes clear saliva drooling.) losing blood from vomiting, (Kidney Water fails to control Fire.) cold limbs, (Cold, Spleen Qi cannot reach four directions,) dizzy eyes, (Liver Blood deficiency,) frequent bleeding from front and back orifices, (Heart, Spleen, Liver, Kidney deficiency, hence frequent bleeding from two yin orifices, and menstruation irregular.) What is the disease name? How is it obtained? It is named Xue Ku [Blood Exhaustion], (Menstrual cessation.) This is obtained from youth, having suffered major blood loss, (Such as multiple births, metrorrhagia, hematemesis, etc.) or intercourse while drunk, (Five Zang repeatedly shaken, hundred vessels agitated.) Middle Qi exhausted, (Essence consumption leads to Qi exhaustion.) Liver injured, (Due to Liver governing dredging and draining,) hence menstruation becomes scanty and stops. (One cause is major blood loss, one is drunken intercourse; treatment should tonify and nourish Yin Qi, allowing blood to fill and return naturally. Although stated for women, if men have this syndrome, it is also Jing Exhaustion disease, belonging to Lao Sun [labor damage].)
Generally treating Xiao Dan [wasting-thirst], Pu Ji [sudden fall/stroke], Pian Gu [hemiplegia], Wei Jue [flaccidity and reversal], Qi Man [fullness], Fa Ni [counterflow], these are diseases of fat and noble people from rich foods. (Fat and noble people likely consume rich flavors, heat accumulates internally, injuring Yin, hence these diseases.) Ge [obstruction] leads to closure, upper and lower不通 (unconnected), this is a disease of sudden sorrow. (Sorrow causes Qi blockage and non-flow, leading to upper or lower痞塞 [stuffiness], and water/grain cannot pass.) Sudden Jue with deafness, unilateral blockage and不通, (Sudden Jue is violent Qi reversal.) caused by internal Qi violently pressing. (Bo means press/intrude. This is due to internal Qi counterflow violently intruding.) Not from internal injury or external Wind strike, (Saying disease does not come from inside, but external Wind strikes,) hence emaciation remains lodged. (Evil stays and does not go,伏 [lies hidden] turning to Heat, causing burning and emaciation.)
Buo [limping], (Bu means inability to walk under foot, Buo means one leg disabled.) Cold, is a Wind-Damp disease. Jaundice, sudden pain, Dian Jue Kuang [mania], are generated by long-term reversal. (Due to long-term Qi reversal, Yin/Yang Ying/Wei are imbalanced, but these syndromes are not formed overnight.) Five Zang imbalance is generated by Six Fu obstruction. (Six Fu obstruction means water/grain cannot transform, fluids cannot move, Essence/Qi lose nourishment, hence Five Zang imbalance.) Headache, tinnitus, nine orifices unbeneficial, are generated by Chang Wei [intestines and stomach]. (Chang Wei includes Six Fu; Six Fu belong to Three Yang, all relate to nine orifices.) Re Zhong [interior heat] Xiao Zhong [wasting-thirst], (Drinking much and urinating frequently is Re Zhong; eating much and urinating frequently is Xiao Zhong, both diseases of rich people from fatty/sweet foods.) cannot take Gao Liang [rich foods], Fang Cao [fragrant herbs], Shi Yao [stone drugs]. (Rich flavors, fragrant items, refined mineral drugs; all three assist heat and consume fluids.) Yellow urine indicates heat in lower abdomen. Yin Qi is little, Yang Qi is abundant, hence heat and vexation/fullness. (Yin deficiency means Yang must crowd in; Yang Evil solidifies in Yin division, hence heat and vexation/fullness.)
Disease with sweet taste is named Pi Dan [Spleen Zhang], (Spleen Heat.) This arises from rich/fatty foods. Such people must eat sweet/delicious and fatty foods frequently. Fatty foods cause interior heat, (Strong flavors assist Yang.) Sweet foods cause middle fullness, (Nature is slow and does not disperse.) Hence his Qi overflows upward, (Spleen Heat overflows upward as sweetness,) transforming into Xiao Ke [wasting-thirst]. (Over time transforms into wasting-thirst.) Treat with Lan [Orchid/Cypripedium], to remove Chen Qi [stale Qi]. (Sweet-cold clear fragrance, can generate fluids and stop thirst, can remove accumulated stale heat Qi.)
Disease with bitter taste is named Dan Dan [Gallbladder Zhang], (Gallbladder Heat.) Such people often deliberate without deciding, (Liver/Gallbladder both labored, labor leads to deficiency.) hence Gallbladder deficiency Qi overflows upward, mouth becomes bitter, (Deficiency means Qi not secure, overflowing upward as bitterness.) Gao Liang changes, Zu Sheng Da Ding [sufficiently produce large carbuncles], receiving [evil] like holding emptiness, (Zu means many. Rich flavors excessive, accumulate as interior heat, changes often produce large carbuncles. Heat invades Yang division, infection easiest, like holding an empty vessel to receive objects.) Ying Qi does not follow, (Does not flow smoothly,) counterflows in flesh and muscle, then produces Yong Du [carbuncles/toxins].
Li [Leprosy], has Ying Qi heat and rot, its Qi is not clear, hence nasal pillar breaks and color fails, skin ulcers, Cold-Wind lodges in vessels and does not go, named Li Feng [Leprosy Wind]. (Cold-Wind lodges in blood vessels, staying long, Ying Qi turns heat, skin rots/ulcers, Blood/Qi not clear, decays into Leprosy.)
Head is the mansion of Ming Jing [spirit/clarity], (Essence/Qi of Zang/Fu rises to head to form use of seven orifices.) Head tilting, eyes sunken, (Head droops unable to lift, eyes sunken without light,) Spirit will be snatched. Back is the mansion of Chest [organs], (Back is where Zang/Fu connect.) Back curved, shoulders following, Fu will be ruined. Waist is the mansion of Kidney, unable to turn/wag, Kidney will be exhausted. Knee is the mansion of Sinew, (Sinews mainly governed by Liver, but connecting joints to stand this body, only knee sinews are most important.) Unable to flex/extend, walking hunched and leaning, Sinew will be exhausted. Bone is the mansion of Marrow, unable to stand long, walking trembling, Bone will be exhausted.
Da Gu [Large Bones] withered and dry, (Shoulders droop, neck tilts, waist heavy, knees fail. Kidney governs bone, bone withered means Kidney fails.) Da Rou [Large Flesh] sunken, (Chi skin wasted, buttock flesh withered. Spleen governs flesh, flesh sunken means Spleen fails.) Chest Qi full, panting difficult, (Lung governs Qi, Qi full/panting means Lung fails.) Qi moves form, (Qi does not return to Yuan, form vibrates, True Yin deficient and Lone Yang floats,) expected death in six months. (One year cycle of Yin/Yang change.) Zhen Zang Mai [True Organ Pulse] seen, then give its date. (Can decide death date based on overcoming day, not limited to six months example.)
Da Gu withered, Da Ru sunken, Chest Qi full, panting difficult, internal pain pulling shoulder/neck, (Disease reaches Heart,) expected death in one month. Zhen Zang Mai seen, then give its date.
Da Gu withered, Da Ru sunken, Chest Qi full, panting difficult, internal pain pulling shoulder/neck, body heat, (Yin Qi gone,) Tuo Rou Po [flesh shed skin broken], (Po means lying long, bones exposed, flesh/muscle ruined,) Zhen Zang seen, death within ten days. (Heavenly stems end, ten-day cycle changes.)
Da Gu withered, Da Ru sunken, Shoulder Marrow internally consumed, movement increasingly weak, Zhen Zang not seen, expected death in one year. (Although syndromes not complete, Zhen Zang not seen, Spleen/Kidney failure already signaled, supports only one year, year changes Qi new, cannot revive again.) Seeing its Zhen Zang, then give its date.
Da Gu withered, Da Ru sunken, Chest Qi full, abdominal pain, heart uncomfortable, shoulder/neck/body heat, skin broken flesh shed, eye sockets sunken, Zhen Zang seen, eyes do not see person, immediate death. (Five Zang failure syndromes seen, eye sockets sunken, Zhen Zang seen, eyes do not see person, Shen Qi already detached.) Those who see person, die at time of their overcoming element. (Shen Qi still present, must wait for overcoming time to die.)
Ji Xu [Urgent Deficiency], (Saying Yuan Qi suddenly injured and abruptly changed.) Body struck suddenly, (Evil strikes body, must suddenly die.) Five Zang closed, pulse channels不通 (unconnected), Qi does not go/come, like falling/drowning, cannot set period. (Like falling/drowning, morning/evening unpredictable, cannot discuss with usual period.)
Its pulse cuts off and does not come, (Pulse cuts off, does not arrive.) If one person exhales five, six beats, (All Zang Qi cut off, life ends,) form/flesh not shed, Zhen Zang although not seen, still dies. (Not like above gradual decline with predictable death date.)
Pulse vigorous, (Heart receives Evil.) Skin hot, (Lung receives Evil.) Abdomen full, (Spleen receives Evil,) Men Mao [vexation/blindness], (Blindness/vexation, Liver receives Evil. Pulse passes diaphragm, Qi counterflows upward.) Front/back不通 (unconnected), (Kidney receives Evil.) This is called Wu Shi [Five Excesses] death. Body sweats, (Exterior Evil resolves.) Obtains bowel movement, (Interior Evil removed.) Then Excess ones live. (Interior/exterior harmonious.)
Pulse fine, (Heart deficient.) Skin cold, (Lung deficient.) Qi little, (Liver deficient.) Food/drink not enter, (Spleen deficient.) Diarrhea front/back, (Kidney deficient.) This is called Wu Xu [Five Deficiencies] death. Porridge/gruel enters stomach, (Spleen gradually revives.) Diarrhea stops, (Kidney gradually consolidates.) Then Deficient ones live. (Root Qi returns.)
Speaking faintly, speaking all day then again, this is Duo Qi [Qi Snatching], (Qi very deficient, voice not continuous. Lung organ loses hold.) Clothes/quilt not gathered, speech good/bad, avoiding/not avoiding relatives/strangers, this is Shen Ming [Spirit/Mind] chaos. (Heart organ loses hold.) Cang Lin [Granary] not storing, is Hu Men [gate] not controlling. (Gate not secure, hence diarrhea uncontrollable, Spleen organ loses hold.) Shui Quan [water spring] not stopping, is Bladder not storing. (Urination uncontrollable, Kidney organ loses hold.) Qi Tuo [snatching] eyes not bright, (Essence/Yang Qi of Five Zang/Six Fu all pour into eyes to become Jing, hence Yang Qi Tuo, eyes not bright.) Blood Tuo color white, naturally lusterless, pulse empty. Jin Tuo [fluid snatching], interstices open, sweat pours out. Ye Tuo [liquid snatching], bone joints flex/extension difficult, color sallow, (Color withered.) Brain/Marrow consumed, leg sore, ears frequent ringing. Jing Tuo [essence snatching],
