Postpartum shortness of breath and panting occur because excessive blood loss during childbirth leads to severe fatigue and injury. Qi loses its foundation for attachment, causing the breathing rhythm to become disordered and deviate from normal physiological states. The breath is short and rapid with panting, and speech cannot be continuous; this indicates an extreme state of Blood depletion and Qi deficiency. Immediately decoct Shenghua Tang (Life-Birth Decoction) for one to two doses, and add 2-3 qian of Renshen (Ginseng Radix et Rhizoma) for emergency rescue. Meanwhile, prepare vinegar, Chinese chive, and bottles to prevent fainting. Do not hesitate to use Renshen due to unfounded fears that it will exacerbate panting; failing to use it sufficiently may lead to untreatable conditions. All Qi-descending medications are strictly prohibited at this time. If the patient is too deficient to tolerate tonification, vomits upon smelling food, and the Stomach Qi is about to perish, it is a terminal condition.
Modified Shenghua Tang: Chuanxiong Rhizome (Chuanxiong) 1 qian, Angelica Sinensis Root (Danggui) 3 qian, Charred Ginger Rhizome (Jiangtan) 5 fen, Peach Kernel (Taoren) 10 grains, Renshen (Ginseng Radix et Rhizoma) 3 qian, Poria (Fuling) 1 qian (omit if sweating is profuse). Add Huangqi (Astragali Radix) if sweating is profuse. Add Maidong (Ophiopogonis Tuber) and Wuweizi (Schisandrae Fructus) if thirsty. For weak Stomach Qi, anorexia, vomiting upon smelling medicine, or vomiting after ingesting cold medicines/foods with inability to retain grain, use Dushen Tang (Single Ginseng Decoction): Renshen 3-4 qian, Fresh Ginger (Shengjiang) 3 slices, Rice (Jimi) a pinch.
Buzhi Yangrong Tang (Tonifying Qi and Nourishing Essence Decoction): Chuanxiong Rhizome (Chuanxiong) 2 qian, Angelica Sinensis Root (Danggui) 4 qian, Honey-fried Licorice Root (Zhigancao) and Charred Ginger Rhizome (Paojiang) each 5 fen, Renshen (Ginseng Radix et Rhizoma) 2 qian, Huangqi (Astragali Radix) 1 qian, Prepared Rehmannia Root (Shudi), Sour Jujube Seed (Zaoren), and Chinese Yam (Shanyao) each 1 qian, Tangerine Peel (Chenpi) 3 fen. Add Fuzi (Aconiti Lateralis Radix Praeparata) 3 fen if feet are cold; add Mahuanggen (Ephedrae Radix et Rhizoma) 1 qian and Floating Wheat (Fuxiaomai) a pinch if sweating is profuse; add Maidong (Ophiopogonis Tuber) and Wuweizi (Schisandrae Fructus) if thirsty. Add Congrong (Cynomorii Herba) and Haren (Semen Cannabis) for constipation.
Within ten days before or after delivery, if contracted by Wind-Cold, presenting with cough, nasal congestion, heavy body, aversion to cold, or accompanied by fever and headache, Jiashen Ningfei Tang (Ginseng-Augmented Lung-Calming Decoction) is appropriate. Do not use Mahuang (Ephedrae Herba) to induce sweating. If panting is accompanied by hypochondriac pain, do not use Xiaochaihu Tang (Minor Bupleurum Decoction) to disperse pathogens. If suffering from Fire Cough (cough due to Yin deficiency and fire) with audible sound, scanty red sputum, do not use cold-natured medicines to clear heat. Treatment should primarily focus on regulating Qi and Blood. If dry cough with sound and scanty sputum persists for half a month, Jiawei Siwu Tang (Modified Four Substances Decoction) can be used.
Jiashen Ningfei Tang: Chuanxiong Rhizome (Chuanxiong) 1 qian, Angelica Sinensis Root (Danggui) 3 qian, Renshen (Ginseng Radix et Rhizoma) 2 qian, Apricot Kernel (Xingren) 10 grains, Platycodon Root (Jiegeng) and Exocarpium Citri Grandis (Juhong) each 4 fen, Farflores Flos (Kuandonghua) 1 qian, Mulberry Root Bark (Sangpiai) 7 fen, Pinellia Tuber (Banxia) 8 fen, Anemarrhenae Rhizoma (Zhimu) 1 qian. If deficient with abundant phlegm, add Bamboo Sap (Zhuyou) one small cup, Ginger Juice (Jiangzhi) three spoons, Honey-fried Licorice Root (Zhigancao) 4 fen, Farflores Flos (Kuandonghua) 6 fen. Decoct in water and take orally.
Xue Lizhai's Renshen Suxue Yin (Renshen and Suxue Drink): Renshen (Ginseng Radix et Rhizoma) 3 fen, Suxie (Sappan Lignum) 1 fen. Decoct in water. This formula treats cough and urgent panting caused by postpartum blood stasis entering the Lung.
