After delivery, if the patient experiences blurred vision, dizziness, and loss of consciousness, it is termed Blood Syncope (Xue Yun). There are three main causes: first, exhaustion from overwork leading to Qi depletion and spiritual confusion; second, excessive blood loss leading to a critical deficiency of Yuan Qi (Original Qi); third, Phlegm-Fire rising due to deficiency, causing clouded consciousness. In these three conditions, the Hun (Ethereal Soul) fails to accompany the Shen (Spirit) in its normal circulation, resulting in near-respiratory arrest. Immediately administer Jiawei Shenghua Tang (Modified Birth-Tonifying Decoction) for two or three doses to resolve blood stasis and alleviate pain. Externally, use the Vinegar and Chinese Chive method to stimulate the nasal cavity, allowing the parturient to smell vinegar, thereby gathering and stabilizing the Spirit. This is an emergency method for treating Blood Syncope. Generally, for weak parturients or during labor, Shenghua Tang should be prepared in advance, along with Chinese chives, vinegar, and bottles as a precaution. This ensures complete safety. After the infant is born, one must not focus solely on the joy of the child while neglecting the mother, nor should the mother ignore her fatigue due to concern for the child. Furthermore, lying down immediately after birth or experiencing anger leading to rebellious Qi can all induce syncope. Exercise utmost caution!
Jiawei Shenghua Tang (Modified Birth-Tonifying Decoction) consists of: Chuanxiong Rhizome (Chuanxiong) 3 qian, Angelica Sinensis Root (Danggui) 6 qian, Charred Ginger Rhizome (Jiangtan) 4 fen, Peach Kernel (Taoren) 10 grains, Honey-fried Licorice Root (Zhi Gancao) 5 fen, Schizonepeta Spike (Jingjie) 5 fen. Decoct in water, add wine, and take hot. If sweating is excessive, add Ginseng Radix et Rhizoma (Renshen) 2 qian, Astragali Radix (Huangqi) 1 qian; however, this is only indicated when there is no abdominal pain due to stasis. If pain is severe, add Cinnamon Twig (Rougui) 5 fen; if thirsty, add Ophiopogon Tuber (Maidong) 1 qian and Schisandra Fruit (Wuweizi) 10 grains. If phlegm is present, add Bamboo Sap (Zhuli) half cup and Ginger Juice (Jiangzhi) one spoon. If syncope is caused by Phlegm-Fire rising due to deficiency, add Exocarpium Citri Reticulatae (Juhong) 4 fen. If the pulse is hidden in both hands or absent in the left hand, add Ginseng Radix et Rhizoma (Renshen) 4 qian, Ophiopogon Tuber (Maidong) 1 qian, and Schisandra Fruit (Wuweizi) 2 qian. For these three types of Blood Syncope, strictly avoid herbs that break blood and deplete Qi. Even Hawthorn Fruit (Shanzha), despite its mild nature, should not be used. If there is pain due to uterine subinvolution (blood stasis), administer Yi Mu Wan (Motherwort Pill) and Cervus Cornu Pulveratum (Lujiaohui) alongside Shenghua Tang. If Qi collapse is imminent, with clenched jaw and inability to swallow medicine, insert a goose feather tube into the throat to instill the medication. If the body gradually warms up after instillation, continue administration regardless of the number of doses. Simultaneously, use warm hands to massage from the heart region down to the abdomen through a single layer of clothing, and frequently change into warm garments to maintain warmth. The patient will naturally regain consciousness gradually. If, upon waking, the pain from uterine subinvolution persists, reduce the dosage of Ginseng Radix et Rhizoma (Renshen) and Astragali Radix (Huangqi), and take only Shenghua Tang alone. Wait until the stasis resolves and pain ceases before resuming supplementation with Ginseng Radix et Rhizoma (Renshen) and Astragali Radix (Huangqi).
