The parturient lies supine. The midwife examines carefully with a lamp to see where the umbilical cord is entangled around the fetus, and gently unties and removes it by manual manipulation. Then, she administers medicine and food according to the previous method. Supporting the parturient upright and urging her to push forcefully will result in immediate delivery.
Foshou San (Buddha's Hand Powder) (see preceding text) is the most stable and effective formula.
Another prescription: Take earth from the center of the stove (earth taken from the sides is ineffective), grind it into fine powder, mix with wine, and take one qian. Simultaneously, apply it to the parturient's umbilicus; this is extremely effective.
Another prescription: Use a finger to dip soot from the bottom of a cooking pot, draw an "X" on the sole of the infant's foot, and the baby will be delivered smoothly immediately.
Another prescription: Take half a zhong of honey and half a zhong of pure sesame oil, boil until reduced by half, then administer; the baby will be delivered instantly.
During labor, if the uterus prolapses first, coiling and exposing outside the body, while the fetus is born subsequently, and the uterus fails to retract after birth, this is called "Pan Chang Sheng" (Coiled Intestine Birth). This is generally due to chronic Qi deficiency failing to consolidate and bind, Blood heat causing easy flow, instability of the Lower Yuan (lower metabolic foundation), and laxity of the key gates, leading to such conditions. The rescue method: After the fetus and placenta are expelled, have the parturient lie supine. The midwife should first wash the prolapsed uterus with warm water, then lift and gently push it back inside, pushing upwards. Subsequently, instruct the parturient to clench her legs tightly and contract the anus, and the uterus will naturally retract. Alternatively, take forty-nine castor beans (Ricini Semen), remove the shells, pound them into a paste, and apply it to the Baihui point (GV20) on the vertex. Remove the application urgently once the uterus has fully retracted. Secondly, one may mix cold water with vinegar and spray it onto the parturient's face; with each spray, the uterus contracts gradually until fully retracted. This method is excellent. Or, treat according to the various prescriptions for uterine prolapse listed later. To avoid this suffering, it is advisable to take Dihuang Wan (Rehmannia Pill) added with one liang of Wuweizi (Schisandrae Fructus) and one liang of Rougui (Cinnamomi Cortex) during intervals between pregnancies to consolidate the keys of the Lower Yuan. Upon becoming pregnant again, take Hulian Wan (Berberis Pill) added with one liang of Taidangshen (Codonopsis Radix) to tonify Qi, and also take Sanbu Wan (Three Tonics Pill) to cool the Blood. Medicines that induce premature birth or thin the fetus should not be taken lightly. If at the eighth month, take Bawu Tang (Eight Substance Decoction) added with Kezi (Terminaliae Chebulae Fructus), Maidong (Ophiopogonis Radix), and Suke (Papaveris Pericarpium) for more than ten doses, which can prevent this occurrence. All other prescriptions are mentioned previously. If the uterus is dry and cannot retract, and all medicines fail, sprinkle a little water prepared by grinding a knife onto the uterus to moisten it. Additionally, decoct genuine magnetite (Magnetitum) in one cup of water, mix with wine, and drink; the uterus will naturally retract. This is the divine prescription of Bian Que.
