During the stage of pustule formation (pustulation), if the areola around the pox appears red and purple, this indicates heat in the blood level. It is essential to use Xiaodu Huoxue Tang (Detoxifying and Blood-Activating Decoction). Although the formula contains Huangqin (Scutellariae Radix, 黄芩), one should additionally add Baizhu (Atractylodis Macrocephalae Rhizoma, 白术) five to six fen to stabilize the fetal origin (Taiyuan, 胎元), ensuring the fetus remains stable and unmoved. If the areola is pale red or pale white, Qianjin Neituo San (Thousand-Gold Internal-Supporting Powder, 千金内托散 - see previous text) must be used, with the dosage of Renshen (Ginseng Radix et Rhizoma, 人参) doubled and Rougui (Cinnamomi Cortex, 肉桂) removed.
For pregnant women developing smallpox, securing the fetus should be the primary principle. If prior treatment did not follow proper methods, leading to internal steaming of heat and subsequent miscarriage, such cases are common. In such situations, instruct an experienced midwife to support the parturient woman, wash her lower body with hot water, help her onto the bed, prevent her from lying supine, and require her to sit upright for about fifteen minutes. Then, prepare a mixture by adding one portion of Baijiu (Baijiu, 白酒) to boiling water, mix with half a cup of Tongbian (Human Urine, 童便), and administer orally to promote the circulation and expulsion of blood stasis. If the pox is dense, use Jiawei Baoyuan Tang (Modified Baoyuan Decoction, 加味保元汤); if Qi and Blood are extremely deficient, use Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction, 十全大补汤). If there is acute pain in the lower abdomen, it indicates incomplete expulsion of blood stasis; Heishen San (Black God Powder, 黑神散) is suitable to cleanse and remove it. Cold-natured herbs must strictly be avoided.
Jiawei Baoyuan Tang: Taidangshen (Codonopsis Radix, 台党), Huangqi (Astragali Radix, 黄芪), Danggui (Angelicae Sinensis Radix, 当归), Chuanxiong (Ligustici Chuanxiong Rhizoma, 川芎), Baizhi (Angelicae Dahuricae Radix, 白芷), Jiegeng (Platycodonis Radix, 桔梗), Shang Nongpo (Magnoliae Officinalis Bark, 上浓朴), Zicao (Arnebiae seu Lithospermi Radix, 紫草), Fangfeng (Saposhnikoviae Radix, 防风), Niuzi (Arctii Fructus, 牛子), Baishao (Paeoniae Radix Alba, 白芍), Rougui (Cinnamomi Cortex, 肉桂).
Heishen San: Shudi (Rehmanniae Radix Praeparata, 熟地), Danggui (Angelicae Sinensis Radix, 当归), Chuanxiong (Ligustici Chuanxiong Rhizoma, 川芎), Gancao (Glycyrrhizae Radix et Rhizoma, 甘草), Rougui (Cinnamomi Cortex, 肉桂), Paojiang (Zingiberis Rhizoma Praeparatum, 炮姜), Puhuang (Typhae Pollen, 蒲黄 - stir-fried), Heidou (Sojae Semen Nigrum, 黑豆), Xiangfu (Cyperi Rhizoma, 香附 - vinegar-processed), Tongbian (Human Urine, 童便).
For women developing smallpox postpartum, the treatment should focus primarily on greatly tonifying Qi and Blood. Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction, 十全大补汤) is the crucial formula at this time. The Baishao (Paeoniae Radix Alba, 白芍) in the formula should also be stir-fried until cooked with good Baijiu (Baijiu, 白酒). One must not recklessly use cold-natured herbs, as this would damage the intent of generating and dispersing vitality.
At the initial stage of measles (Mazheng, 麻症), symptoms often include coughing, sneezing, clear nasal discharge, tearing, and swelling of the eyelids. The body temperature gradually rises, and spots appear on the skin after two to three days or four to five days. The lesions resemble hemp seeds in shape and look like elm flowers in color. Occasionally, some may appear larger, similar to smallpox. However, after the eruption phase begins, the lesions become pointed and sparse, gradually becoming denser. They have granules but lack the root areola. They rise slightly and appear pale but do not form pus, which is significantly different from smallpox. Although it is said to be slightly milder than smallpox, the changes occur rapidly. Careful attention is required throughout the entire course of treatment. In summary, at the onset, thorough exterior-releasing methods should be used first; cold-natured herbs are most taboo. When the rash has erupted, clearing and draining methods should be employed. After the resolution and shedding phases, nourishing Blood is prized. For complicated symptoms, treatment should be adjusted according to the specific manifestations. Warm-rash syndrome presenting with cold-strike symptoms: if it occurs before smallpox eruption, it is called Saozhen (Itching Rash, 瘙疹); within one month, it is called Lanyi Chuang (Rotting Clothes Sore, 烂衣疮); within one hundred days, it is called Bairi Chuang (Hundred-Day Sore, 百日疮). Upon eruption, red spots cover the body like millet grains, caused by heat accumulated in the womb during gestation. With careful nursing and management, rapid recovery will naturally occur. If rashes erupt immediately after smallpox heals, it is called Gaidouzhen (Covering Smallpox Rash, 盖痘疹). The rashes spread over the whole body, are red and itchy, starting like millet grains and gradually forming cloud-like patches. This is due to residual toxins remaining after smallpox, combined with excessive diet and external exposure to wind-cold. Treatment should primarily focus on dispersing wind and clearing heat. Regarding Yinzhen (Urticaria, 瘾疹), where rashes are very itchy, red, and hidden within the skin, this is caused by Heart Fire scorching the Lung Meridian, compounded by external wind-dampness. Treatment should first disperse wind-dampness, then clear heat and toxins. This approach is correct. These conditions are not measles (Mazhen, 麻疹) but rather toxins inherited from the fetal origin (Taiyuan, 胎元).
