The pustules are fully covered and red, filled with blood inside, hence named "Blood Blister Pox" (Xuepao Dou). This condition arises from intense heat-toxin in the blood level that fails to transform into pus, resulting in this symptom. Urgent treatment should focus on cooling the blood and resolving toxicity. The prescription uses Shengdi Huang (Rehmanniae Radix), Chishao (Red Peony Root), Honghua (Carthami Flos), Zicao (Arnebiae Radix), Danpi (Moutan Cortex), Niubangzi (Arctii Fructus), Huangqin (Scutellariae Radix), and Baizhi (Angelicae Dahuricae Radix); it is contraindicated for those with weak constitutions. It is essential to refer to the previous methods of observing appearance and color to deliberate and use this appropriately.
At the initial onset, if the sores already contain white pus but subsequently turn grayish-white, become itchy, and collapse, death will ensue. Normally, pus formation starting from the feet indicates a favorable course (Shun Zheng). Here, the toxin invades the Yang aspect, while Yin blood fails to transform it, causing the toxin to sink and lie latent shortly thereafter. However, at the very beginning, if only a few spots appear, one may puncture them to drain the pus; as new pus forms, puncture again. Constant monitoring is required without negligence. Internally administering Qianjin Neituo San (Thousand-Gold Internal Support Powder) to greatly tonify Qi and Blood ensures safety. If the entire body presents thus, it becomes difficult to treat. Exercise utmost caution! Alternatively, referring to the previous diagnostic methods, treating with Dabu Yuan Jian (Great Tonification Decoction) is even more excellent.
Qianjin Neituo San: Renshen (Ginseng Radix et Rhizoma), Zhi Huangqi (Astragali Radix prepared with honey) each 1.5 qian; Baishao (Paeoniae Radix Alba), Rougui (Cinnamomi Cortex) each 6 fen; Chuanxiong (Ligustici Chuanxiong Rhizoma), Zhi Gancao (Glycyrrhizae Radix et Rhizoma prepared with honey) each 4 fen; Baizhi (Angelicae Dahuricae Radix) 7 fen; Shanzha (Crataegi Fructus) 8 fen; Muxiang (Aucklandiae Radix), Fangfeng (Saposhnikoviae Radix), Houpo (Magnoliae Officinalis Cortex) each 3 fen.
Refer to the preceding chapters. When pox fail to form pus, holes appear on the apex, leaking fluid which accumulates and condenses, resembling tinea capitis (ringworm of the scalp), commonly known as "Leaking Pox" (Lou Dou). This occurs because exterior deficiency fails to nourish the pus, leading to spontaneous rupture without external injury. Urgent treatment requires tonifying Qi and strengthening the Spleen to consolidate the muscles for salvation. If the whole body ruptures, death is inevitable. One must use Renshen (Ginseng Radix et Rhizoma), Zhi Huangqi (Astragali Radix prepared with honey), Danggui (Angelicae Sinensis Radix), Guanzi Gui (Cinnamomi Ramulus), Chuanxiong (Ligustici Chuanxiong Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Baizhi (Angelicae Dahuricae Radix), Zhi Gancao (Glycyrrhizae Radix et Rhizoma prepared with honey), Dingxiang (Caryophylli Flos), Huaishan (Dioscoreae Rhizoma), adding Longyan Rou (Longan Aril) and Lianzi (Nelumbinis Semen) to decoct together.
Pox presenting small holes the size of a needle mouth, resembling eyes chewed by insects, are termed "Bitten Pox" (Zhu Dou). This results from loose interstices (Cou Li), and if not treated urgently, massive depletion of Primordial Qi will lead to grayish sinking and death. One must use Renshen (Ginseng Radix et Rhizoma), Zhi Huangqi (Astragali Radix prepared with honey), Danggui (Angelicae Sinensis Radix), Zhi Gancao (Glycyrrhizae Radix et Rhizoma prepared with honey), Rougui (Cinnamomi Cortex), Dingxiang (Caryophylli Flos), adding Lizhi (Litchi Chinensis pericarpium or fruit) as a guiding drug. This will surely close the pores, allowing the pox to swell and generate pus. It remains crucial to consult the preceding chapters.
