Taiyang disease with a floating and tight pulse, where the exterior syndrome remains unresolved, accompanied by fluid retention in the epigastrium (fluid accumulation below the heart), vomiting with fever, or coughing, thirst, dysentery, frequent urination, hiccups, or slight wheezing, should be primarily treated with Xiaolongdan Decoction (Minor Dragon Decoction).
If the rebellious Qi and wheezing are severe, it may transform into Jie Xiong (Chest Binding); if the exterior syndrome is not resolved, one should carefully differentiate the preceding symptoms and appropriately administer Guizhi Tang (Cinnamon Twig Decoction), Mahuang Tang (Ephedra Decoction), or Xiaolongdan Decoction.
Furthermore, in Taiyang syndrome, if sweating was previously administered but did not resolve the condition, and purgation was mistakenly used without success, a floating pulse indicates the pathogen remains on the exterior, hence the failure to heal. If the exterior syndrome persists, the pathogenic factor should first be released via diaphoresis; Guizhi Tang (Cinnamon Twig Decoction) is appropriate. Quack doctors often misapply this precedent.
Furthermore, with a floating and tight pulse, absence of sweat, and fever, accompanied by pain persisting for eight or nine days, diaphoresis should be induced to slightly alleviate symptoms. If irritability and photophobia worsen at this stage, nasal bleeding will likely occur to resolve the condition. This occurs because Yang Qi is excessively constrained.
Furthermore, in cold damage, if headache is not severe and the pulse is not tight and solid, purgation must absolutely be avoided. Purgation will inevitably exacerbate the condition or lead to Jie Xiong (Chest Binding), which becomes difficult to cure.
The application of medical prescriptions requires precision; the key lies in examining the pulse, respiration, and rate, and differentiating the physical signs of deficiency and excess. There is no treatment that does not employ cold herbs to treat heat syndromes, or hot herbs to attack cold syndromes; this is the principle of sages in treating diseases.
Wheezing is generally regarded by the world as a critical and dreaded disease. Some suffer from wheezing for decades, reaching a critical state upon each flare-up yet recovering spontaneously; others suddenly develop wheezing due to another illness, leading to untreatable conditions. Various formulas discussing diarrhea and dysentery only attribute it to Spleen and Stomach disorders, stating merely that wind-cold-damp-toxins invade, or dietary injury causes stagnation, leading to diarrhea when the intestines are deficient.
