Symptoms of vomiting phlegm manifest in various ways. All stem from the disturbance of fire-evil in the Lung and Stomach. All should be treated by clearing the Lung, eliminating phlegm, and extinguishing fire. Practitioners must view them as a single category of disease.
There are two types of vomiting phlegm: one involves spitting out white phlegm, and the other involves spitting out clumped phlegm. Both arise because the fire in the Lung and Stomach has not been cleared. Treatment urgently requires clearing the Lung, eliminating phlegm, and descending fire. One may use Xingren Qingfei Tang (Apricot Kernel Decoction to Clear the Lung) (see Article 56), removing Gancao (Licorice Root) and Jiegeng (Platycodon Root), and adding Huangqin (Scutellaria Root). Alternatively, one may use Qingfei Yin (Lung-Clearing Decoction) (see Article 56), removing Wuweizi (Schisandra Fruit), Baijie Geng (White Platycodon Root), and Gancao (Licorice Root), and adding Maidong (Ophiopogon Tuber), Huangqin (Scutellaria Root), Zhimu (Anemarrhena Rhizome), and Shigao (Gypsum Fibrosum). However, when clearing phlegm, one must absolutely avoid using drying herbs such as Banxia (Pinellia Tuber) and Nanting (Arisaema Rhizome). It is appropriate only to use herbs like Tianhuafen (Trichosanthes Root), Beimu (Fritillaria Bulb), and Chenpi (Citrus Peel). To clear the Lung and descend fire, it is appropriate to use herbs such as Tiandong (Asparagus Tuber), Maidong (Ophiopogon Tuber), Xingren (Apricot Seed), Sangbaipi (Morus Bark), Huangqin (Scutellaria Root), and Huanglian (Coptis Rhizome). One must absolutely avoid using Jiegeng (Platycodon Root), which is an herb that raises Qi in the Middle Jiao. If there is phlegm after measles but no vomiting of phlegm, it is appropriate to use Jiawei Erchen Tang (Modified Two-Aged Phlegm Decoction), removing Banxia (Pinellia Tuber) and Gancao (Licorice Root), and adding Beimu (Fritillaria Bulb) and Gualou Shuang (Trichosanthes Powder) as the main treatment. If there is severe phlegm in the chest after measles, use Jingyan Fu Fang (Experience External Application Formula), applying externally or wiping for treatment.
Zhu stated: Phlegm generated due to fire is a mild form of measles syndrome. In treating the phlegm, avoiding acrid-drying herbs demonstrates precise judgment.
Jiawei Erchen Tang (Modified Two-Aged Phlegm Decoction)
Chenpi (Citrus Peel), Banxia (Pinellia Tuber), Bai Fuling (Poria with Hostwood), Fangfeng (Saposhnikovia Root), Tianma (Gastrodia Rhizome), Lianqiao (Forsythia Fruit), Gancao (Licorice Root). Decoct in water and take orally.
Jingyan Fu Fang (Experience External Application Formula) Treats severe phlegm in the chest.
Wuzhuyu (Evodia Fruit) (one liang, stir-fried), Congbai (Green Onion White) (more than ten cun long). Grind into powder. Mix with natural juice of fresh ginger and mashed green onion white to form a cake. Steam until warm. Apply to the chest. Immediate effect.
What is the cause of vomiting roundworms? It is due to deficiency-cold in the Stomach without grain nourishment, causing worms to ascend. Urgent treatment is required to prevent stomach rot; regulating the Spleen and Stomach will naturally resolve the symptoms.
Vomiting roundworms before the eruption of measles is caused by heat in the Stomach preventing the intake of grain food. The treatment method should involve clearing the Lung and resolving the exterior, using Miben Mendong Qingfei Tang (Secret Version Ophiopogon Lung-Clearing Decoction) (see Article 50), removing Gancao (Licorice Root) and Jiegeng (Platycodon Root) as the main treatment. If roundworms are vomited during the stage when the rash is about to emerge or is emerging, it is because there is heat in the Stomach and phlegm above the diaphragm, causing the worms to be unsettled. If the patient can eat, the condition will resolve on its own, and medication is unnecessary. If medication is desired, administer Shachong Dan (Roundworm-Killing Pill). If roundworms are vomited during the initial fever stage before the appearance of measles, or if too many roundworms are vomited leading to inability to eat, these are cases of deficient and failing Stomach Qi, which are incurable. However, vomiting roundworms mostly occurs during the resolution phase and after the resolution of measles. This is also due to intense burning of Stomach fire, resulting in reduced appetite. With no nourishment, the worms ascend and exit. Sometimes three or four, five or six, or more than ten are expelled and then stop. It is not advisable for too many to be expelled, as excessive expulsion is harmful. Treatment should urgently focus on calming the Stomach; if prolonged, stomach rot will occur and become difficult to save. Use Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder) added with Wumei (Mume Fruit), adjusting according to circumstances. Worms are calmed by sour flavors. If roundworms are vomited after measles when the heat has subsided and the patient can eat, it is appropriate to regulate the Spleen and Stomach to prevent other complications. Use Anhui San (Calming Roundworm Powder) removing Hufen (Lead Carbonate) and Baijian (Alum), or treat with Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder) added with Wumei (Mume Fruit). If roundworms are vomited after measles, accompanied by anorexia and spontaneous sweating, this is truly Stomach deficiency, which is most difficult to treat. Use Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder) added with Wumei (Mume Fruit), Muli Fen (Oyster Shell Powder), and KT Shihu (Dendrobium Stem). If it is Spleen heat, use Xiehuang San (Drain Yellow Powder) (see Article 57) added with Wumei (Mume Fruit), Muli Fen (Oyster Shell Powder), and KT Shihu (Dendrobium Stem).
Zhu stated: In the syndrome of vomiting roundworms, cases due to Stomach heat causing worms to be unsettled are common. For vomiting roundworms due to Stomach deficiency, Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder) added with Wumei (Mume Fruit) and Shihu (Dendrobium Stem) serves as an emergency rescue method.
Shachong Dan (Roundworm-Killing Pill)
Shengcong (Raw Green Onion) (one bunch). Stir-fry and eat. The worms will turn into water.
Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder) Treats thirst due to Stomach deficiency where Jin-Ye (Body Fluids) and Qi cannot circulate.
Renshen (Ginseng Root), Baizhu (Atractylodes Macrocephala Rhizome) (stir-fried), Bai Fuling (Poria with Hostwood), Zhi Gancao (Honey-fried Licorice Root), Huoxiang Ye (Agastache Leaf), Gegen (Kudzu Root) (roasted), Muxiang (Aucklandia Root). Decoct in water and take orally.
Anhui San (Calming Roundworm Powder) Treats vomiting of red, coiled, and live roundworms, belonging to heat syndromes.
Wumei Rou (Mume Flesh) (three qian), Huanglian (Coptis Rhizome), Shujiao (Sichuan Pepper), Huoxiang (Agastache), Binglang (Areca Seed) (each one qian), Hufen (Lead Carbonate), Baijian (Alum) (each five qian). Grind into powder. Take three qian each time, decoct in water until paste-like consistency, and take on an empty stomach.
Tu Mo (Spitting Saliva) (pronounced 'mo', referring to saliva in the mouth.)
