Prescription: Baizhu (Atractylodis Rhizoma, 白术) 18g, Dangshen (Codonopsis Radix, 党参) 20g, Huangqi (Astragali Radix, 黄芪) 35g, Guizhi (Cinnamomi Ramulus, 桂枝) 13g, Danggui (Angelicae Sinensis Radix, 当归) 15g, Chuanxiong (Ligustici Chuanxiong Rhizoma, 川芎) 12g, Bajiitian (Morinda Officinalis Radix, 巴戟天) 10g, Yizhiren (Alpiniae Oxyphyllae Semen, 益智仁) 9g, Shichangpu (Acori Tatarinowii Rhizoma, 石菖蒲) 9g, Yuanzhi (Polygalae Radix, 远志) 6g, Chenxiang Powder (Aquilariae Lignum Resinatum, 沉香粉) 3g, Hangjuhua (Chrysanthemi Flos, 杭菊花) 10g, Guangmuxiang (Costi Rhizoma, 广木香) 6g, Gancao (Glycyrrhizae Radix et Rhizoma, 甘草) 7g, Dazao (Ziziphi Jujubae Fructus, 大枣) 12 pieces, Shengjiang (Zingiberis Rhizoma Recens, 生姜) 3 slices. Take five doses, one dose per day, divided into three servings.
Rationale of the Formula: Baizhu, Dangshen, and Gancao are used to supplement the Qi of the Spleen and Lung in the right Yin meridians. Huangqi and Guizhi are used to supplement the defensive Yang Qi of the Foot-Taiyang Bladder Meridian. Danggui and Chuanxiong assist the circulation of the left Yin meridians to raise blood. Shichangpu, Yuanzhi, and Chenxiang serve as counter-agents to guide the Qi of the left Yang meridians downward. Bajiitian and Yizhiren warm and tonify the Kidney to restrain frequent urination. Hangjuhua and Guangmuxiang clear the brain and awaken the spirit. Gancao, Shengjiang, and Dazao harmonize the various herbs.
Follow-up on December 28, 1968: After taking three doses of the above medicine, the hallucinated image of the "female ghost" disappeared. After completing the five doses, the patient's appetite increased, urination decreased, and mental state improved. Re-examination of the Yin and Yang pulses showed increased pulse force in both left and right positions, but the imbalance among left, right, Yin, and Yang persisted as described previously. Since the main symptoms had subsided, treatment was adjusted gradually. The patient took Guipi Wan (Spleen-Restoring Pill) for over a month and subsequently recovered health.
Reflections: In medical cases from ancient times to the present, many strange and unusual syndromes have been observed. Because the changes of Yin and Yang in the human body are extremely complex, people often suffer from diseases with bizarre phenomena. Since the manifestations vary from person to person, physicians should determine the treatment based on the etiology and syndrome, combined with the findings of the "Four Diagnostic Methods."
In treating internal injuries caused by emotional factors, the Seven Emotions—Joy, Anger, Worry, Thought, Grief, Fear, and Shock—must be used as the basis for differentiation and treatment. For example, in this case, relying on superstition regarding ghosts and gods is useless. Diseases are mostly formed by the dysfunction of Yin and Yang circulation within the body. Therefore, treatment can correct these imbalances by understanding the mechanisms of Yin and Yang, thereby restoring equilibrium.
This case resulted from excessive grief injuring the interior, leading to an excess of Yin Qi and deficiency of Yang Qi. Yin Qi is like night or rainy weather, while Yang Qi is like daytime or clear weather. Thus, using herbs that tonify Yang Qi allows the "sunlight to shine brightly," causing the darkness and haze to disappear.
Applying Traditional Chinese Medicine theory to treat diseases emphasizes understanding the principles of cosmic Yin and Yang creation and the mechanism of Yin and Yang transformation within the human body. The Classic states: "Carefully master Yin and Yang, do not consult with the multitude," which refers to this meaning.
Twenty-Four: Analysis of Famous Formulas Through the Ages
Based on the principles of Yin, Yang, Positive, and Negative in the "Twelve Meridians," combined with clinical experience, I briefly analyzed the indications, herb actions, and functions of nineteen famous formulas from past dynasties in adjusting the movements of San-Yin and San-Yang, Left, Right, Positive, and Negative within the patient's body.
(1) Sijunzi Tang (Four Gentlemen Decoction) [Lantai Guidfan Fang]: Treats sallow complexion, weak voice, weakness in the four limbs, and a weak pulse. If there is internal injury with deficient heat or difficulty digesting food, Paojiang (Prepared Zingiberis Rhizoma, 炮姜) must be added. Renshen (Panacis Ginseng Radix, 人参), Baizhu, Fuling (Poria, 茯苓), and Gancao each 2 qian. Add Jiang (Ginger, 姜) and Zao (Jujube, 枣). Decoct in water and take orally. Xu Lingtai said: "This is the main formula for tonifying the Spleen."
"Sijunzi Tang" treats weakness in the four limbs and indigestion. I believe the primary action of this formula is to support the normal transport function of the right Foot-Taiyin Spleen Meridian. It is suitable for those with Qi deficiency due to Spleen Earth failing to generate Lung Metal.
Baizhu is a key herb for strengthening the stomach; its nature is bitter, sweet, and warm. Bitterness dries dampness, and warmth tonifies the Spleen. Gancao is sweet and neutral; sweetness enters the Spleen. Renshen is sweet, slightly bitter, and slightly warm; it enters the Spleen and reaches the Lung, greatly tonifying Yuan Qi. Fuling is sweet and bland; it leaches out dampness and strengthens the Spleen.
All four herbs in this formula possess a sweet flavor. Sweetness enters the Spleen, making this the main formula for tonifying the Spleen. Together, their properties are "sweet, warm, and peaceful," hence the name "Four Gentlemen." It has significant effects on tonifying the Spleen, benefiting the Lung, and strengthening the Stomach.
(2) Lizhong Wan (Regulate the Middle Pill) and Tang (Decoction) [Lantai Guidfan Shanghan Fang]: Treats cholera with headache, fever, body pain, and aversion to cold/water. Lizhong Tang. If after a severe illness, the patient frequently salivates and cannot recover for a long time due to Cold in the Stomach, use Lizhong Wan. Renshen, Zhigancao (Honey-fried Glycyrrhizae Radix et Rhizoma, 炙甘草), Baizhu, and Ganjiang (Zingiberis Rhizoma, 干姜) each 3 liang. Sieve the above four ingredients into powder. Make pills the size of a chicken egg yolk with honey. Crush one pill and mix with a few ounces of boiling water. Take warm. Three doses daily, two at night. If the abdomen does not feel warm, increase to three or four doses. However, the decoction is superior. To prepare the decoction, cut the four ingredients according to weight. Use eight sheng of water, boil to obtain three sheng, remove dregs, and take one sheng warm, three times daily. Xu Lingtai said: "Sini Tang warms the Lower and Middle Jiao; Lizhong warms the Upper and Middle Jiao; they target different areas."
Lizhong Wan and Tang treat Spleen-Stomach Deficient-Cold, Cold Cholera with abdominal pain and vomiting, or chronic Deficient-Cold abdominal pain and loose stools. I believe its primary action is warming the Middle and dispersing Cold, functioning to regulate the right Spleen Meridian.
Lizhong Wan is suitable for chronic Spleen-Stomach Deficient-Cold; Lizhong Tang acts quickly and is suitable for acute Spleen-Stomach Deficient-Cold. Renshen, Gancao, and Baizhu are excellent herbs for tonifying the Spleen and strengthening the Stomach, assisted by the hot and pungent Ganjiang to warm the Middle and disperse Cold. For those with abdominal pain and diarrhea due to Spleen Qi deficiency and functional collapse, this formula lifts the central Spleen Earth, hence named "Regulate the Middle Pill."
(3) Siwu Tang (Four Substance Decoction) [Lantai Guidfan Ju Fang]: Treats all syndromes of Blood Heat, Blood Deficiency, and Blood Dryness. Danggui and Shudi (Rehmanniae Radix Praeparata, 熟地) each 3 qian; Chuanxiong 1.5 qian; Baishao (Paeoniae Radix Alba, 白芍) 2 qian, stir-fried with wine. Decoct the above four ingredients in water and take orally. Xu Lingtai said: "This is the main formula for Blood diseases."
"Siwu Tang" treats Liver Meridian Blood Deficiency and Heart Blood Deficiency on the left side of the body, or irregular menstruation and dysmenorrhea in women. I believe the primary action of this formula is to support the normal movement function of the left Foot-Jueyin Liver Meridian.
Shudi is sweet and warm, nourishing and tonifying Blood. Danggui is pungent and sweet; combined with Baishao entering the Liver channel, it tonifies Liver Blood and promotes Blood circulation. Chuanxiong has an aromatic, pungent, and warm nature; it disperses and unblocks Blood stasis, acting as the Qi herb within Blood herbs.
The combination of these four herbs collectively tonifies Blood and nourishes the Liver. When Liver Wood Blood is abundant, Heart Blood also thrives. Thus, "Siwu Tang" is the main formula for regulating all Blood diseases.
(4) Foshou San (Buddha's Hand Powder) [Puji Ben Shi Fang]: Treats postpartum Blood Deficiency headache, fetal restlessness with bleeding, and is effective for inducing labor. Danggui 3 qian, Chuanxiong 1.5 qian, made into powder. Lightly decoct and take in one or two doses.
"Foshou San" treats postpartum Blood Deficiency headache, fetal restlessness with bleeding, and is effective for inducing labor. I believe this formula excels at tonifying Blood and promoting circulation, raising Blood flow, and regulating the left Liver Meridian.
1. Postpartum blood loss easily leads to insufficient blood circulation and headache. Chuanxiong ascends to treat headaches, and Danggui tonifies and moves Blood; thus, this formula treats postpartum Blood Deficiency headache.
2. Excessive movement may disturb the fetus, or insufficient Blood may fail to nourish the fetus, leading to heavy bleeding from the uterus and potential miscarriage. Danggui and Chuanxiong tonify and raise Blood, transforming descending Blood upward, stopping bleeding and calming fetal restlessness.
3. During ten months of pregnancy, Blood nourishes the fetus. Blood Deficiency can lead to difficult labor due to scanty and stagnant Blood. This formula tonifies and moves Blood, enhancing circulation, transforming stagnation into smoothness, allowing the fetus to be born easily, hence its efficacy in inducing labor.
However, the combination of Danggui and Chuanxiong is pungent and warm. Warmth promotes circulation and dispersion, tending to ascend. Therefore, it is contraindicated for those with floating, slippery, or large pulses at the Cun position of the left wrist.
(5) Xiaochaihu Tang (Minor Bupleurum Decoction) [Lantai Guidfan Shanghan Fang]: Treats Shaoyang Wind-Strike, alternating chills and fever, fullness and distress in the chest and hypochondrium, irritability with nausea, abdominal pain, palpitations, sweating on the head, white tongue coating, and Heat entering the Blood Chamber in women. Chaihu (Bupleuri Radix, 柴胡) 8 liang, Huangqin (Scutellariae Radix, 黄芩), Renshen, Gancao each 3 liang, Banxia (Pinelliae Rhizoma, 半夏) half sheng washed, Shengjiang 3 liang sliced, Dazao 12 pieces split. Decoct the above seven ingredients in 12 dou of water to obtain 6 sheng, remove dregs, decoct again to obtain 3 sheng. Take 1 sheng warm, three times daily. If vexation in the chest without nausea, remove Banxia and Renshen, add one Gua Lou Shi (Trichosanthis Fructus, 栝楼实). If thirsty, remove Banxia, add Renshen (total 4.5 liang) and Gualou Gen (Trichosanthis Radix, 栝楼根) 4 liang. If abdominal pain, remove Huangqin, add Baishao 3 liang. If subcostal hardness, remove Dazao, add Muli (Ostrea Concha, 牡蛎) 4 liang. If palpitations and difficult urination, remove Huangqin, add Fuling 4 liang. If no thirst with slight external heat, remove Renshen, add Guizhi 3 liang. Cover warmly to induce slight sweat. If coughing, remove Renshen, Dazao, Shengjiang, add Wuweizi (Schisandrae Fructus, 五味子) half sheng and Ganjiang 2 liang. Xu Lingtai said: "The modifications of this formula require careful study."
"Xiaochaihu Tang" treats alternating chills and fever, dizziness, bitter taste, irritability with nausea, chest fullness, and hypochondriac pain. I believe its primary action is regulating the Middle Jiao, assisting the Left Liver and Right Spleen, and regulating the ascending/descending functions of the Gallbladder and Stomach meridians.
Chaihu is the chief herb in the heaviest dosage; its bitter-cool nature regulates the Qi of the left Liver Meridian. Renshen, Gancao, and Dazao are minister herbs; their sweet-warm nature tonifies Qi in the right Spleen, promoting the rise of energy in the Middle Jiao. Banxia, Shengjiang, and Huangqin are assistant/counter-agents, aiding the Gallbladder and Stomach meridians to descend and stop vomiting.
This formula rotates the ascent of Left/Right Yin meridians and descent of Left/Right Yang meridians, causing pathogens in the Half-Interior/Half-Exterior to vanish through Yin-Yang rotation. Hence, it is called the "Harmonizing Method." Only by deeply understanding the mechanism of Yin-Yang rotation can one create such formulas, which is why Zhang Zhongjing is revered as the "Sage of Medicine."
(6) Xiaoyao San (Free and Easy Wanderer Powder) [Lantai Guidfan Ju Fang]: Treats Liver Blood Deficiency with Fire Hyperactivity, headache, dizziness, red cheeks, bitter mouth, fatigue, thirst, depression, hypochondriac pain, alternating chills, lower abdominal heaviness, irregular menstruation, and wiry-large-deficient pulse. Baishao stir-fried with wine, Danggui, Baizhu stir-fried, Fuling, Zhigancao, Chaihu each 2 qian. Adding Mudanpi (Moutan Cortex, 牡丹皮) and Zhizi (Gardeniae Fructus, 栀子) makes Jiawei Xiaoyao San (Modified Free and Wanderer Powder). Add roasted ginger slices and mint leaves when decocting. Xu Lingtai said: "This is a formula to soothe and reach the Liver and Spleen."
"Xiaoyao San" treats Middle Jiao stagnation, hypochondriac distension, headache, and dizziness. I believe its primary action is soothing the Left Liver and Right Spleen, ensuring the smooth ascent of Left/Right Yin meridians to resolve Middle Jiao stagnation.
Chaihu and Danggui soothe the left Liver Meridian; Baishao softens the Liver. These three nourish Blood and soothe the Liver. Baizhu and Gancao tonify the right Spleen Meridian; Fuling leaches dampness. Shengjiang is pungent; Bohe (Menthae Haplocalycis Herba, 薄荷) is pungent-cool and light. They warm the Middle and soothe the Liver. Together, they soothe the Liver and strengthen the Spleen, clearing the Middle Jiao and resolving Liver-Spleen stagnation.
Stagnation disease involves emotions knotting internally without expression, easily injuring the Liver. Chronic emotional stagnation often involves excessive thinking, which injures the Spleen. This formula soothes the Liver and reaches the Spleen, making it effective for treating stagnation.
(7) Mahuang Tang (Ephedra Decoction) [Lantai Guidfan Shanghan Fang]: Treats Taiyang Cold Strike, headache and body pain, fever without sweat, aversion to wind with wheezing. Mahuang (Ephedrae Herba, 麻黄) 3 liang de-stemmed, Guizhi 2 liang de-barked, Gancao 1 liang honey-fried, Xingren (Armeniacae Semen Amarum, 杏仁) 70 seeds soaked in boiling water, peeled, tips removed. Decoct the above four ingredients in 9 sheng of water. First boil Mahuang to reduce by 2 sheng, remove foam. Add other herbs, boil to obtain 2.5 sheng, remove dregs, take 0.8 sheng warm. Cover to induce slight sweat. No need to drink porridge. Rest as with Guizhi Tang.
"Mahuang Tang" treats Exterior Excess Cold with floating-tight pulse and no sweat. I believe its primary action is lifting the Foot-Taiyang Meridian to penetrate and open the skin surface. Since the Foot-Taiyang is bound by Cold pathogens, causing muscle-surface closure, Mahuang and Guizhi (pungent-warm) open pores. Gancao harmonizes the Middle. Xingren (bitter-warm) acts as a counter-agent to descend and guide Qi, rotating Yin and Yang. Although simple, the appropriate combination of Yin-Yang herbs adjusts ascending/descending functions, yielding significant effects.
Mahuang Tang is suitable for Foot-Taiyang Cold Strike, headache/body pain, exterior excess with no sweat, aversion to wind with wheezing, and floating-tight pulses in both Yin and Yang arteries.
(8) Guizhi Tang (Cinnamon Twig Decoction) [Lantai Guidfan Shanghan Fang]: Treats Taiyang Wind Strike, headache, fever, sweating, aversion to wind. Guizhi 3 liang de-barked, Baishao 3 liang, Gancao 2 liang honey-fried, Shengjiang 3 liang, Dazao 12 pieces split. Chew the above five ingredients. Decoct in 7 sheng of water over low heat to obtain 3 sheng, remove dregs. Take 1 sheng warm. Immediately after, drink over 1 sheng of hot thin porridge to boost efficacy. Cover warmly for about an hour until the whole body sweats slightly. Do not let sweat pour like water, or the disease will not be cured. If one dose cures, stop subsequent doses. If no sweat, repeat. If still no sweat, shorten intervals to finish three doses in half a day. For severe cases, take day and night. If symptoms persist after one dose, make another. If no sweat, take up to 2-3 doses. Avoid cold, raw, sticky, greasy, meat, five pungents, alcohol, and foul-smelling foods. Xu Lingtai said: "When taking herbs for external Wind-Cold, administration must be as follows."
"Guizhi Tang" treats Taiyang Wind Strike, floating-slow pulse, headache, fever, sweating, aversion to wind. I believe this formula uses Baishao to enter Yin meridians and gather Yin Blood to regulate Ying; assists Guizhi to ascend via Foot-Taiyang to defend the exterior. Shengjiang, Dazao, and Zhigancao benefit the Spleen and Stomach to harmonize the Middle. Together, they harmonize Ying and Wei, release the muscle layer, support Zheng Qi to expel Evil.
(9) Gegen Tang (Kudzu Decoction) [Lantai Guidfan Shanghan Fang]: Treats Taiyang Disease with stiffness in neck/back, no sweat, aversion to wind. Also treats Taiyang-Yangming Combined Disease with spontaneous diarrhea. Gegen (Puerariae Lobatae Radix, 葛根) 4 liang, Mahuang 3 liang de-stemmed, Guizhi 2 liang de-barked, Baishao 2 liang sliced, Gancao 2 liang honey-fried, Shengjiang 3 liang sliced, Dazao 12 pieces split. Decoct the above six ingredients in 1 dou of water. First boil Mahuang and Gegen to reduce by 2 sheng, remove foam. Add other herbs, boil to obtain 3 sheng, remove dregs. Take 1 sheng warm. Cover to induce slight sweat. No need to drink porridge. Rest and taboos as with Guizhi Tang.
"Gegen Tang" treats Taiyang-Yangming Combined Disease with no sweat and aversion to wind. I believe its primary action rapidly opens the Foot-Taiyang skin/muscle blocked by Cold, addressing Wind-Cold entering the Foot-Yangming Stomach Meridian. Since Taiyang symptoms remain, Guizhi Tang herbs are used. Due to exterior excess with no sweat, Mahuang disperses Taiyang exterior pathogens. Due to involvement of Foot-Yangming, Gegen disperses Stomach Meridian exterior pathogens. All Foot-Yang meridians govern the exterior, but Foot-Taiyang is the outermost, while Foot-Yangming is inner relative to the exterior. The Classic states: "Taiyang is Opening, Yangming is Closing, Shaoyang is Pivot." Thus, Wind-Cold transmission through Six Meridians proceeds hierarchically from exterior to interior.
Comparing Mahuang, Guizhi, and Gegen Tangs in the Shanghan Lun, all address exterior pathogens. Their actions exert reverse movement functions via Yang meridians to expel exterior evils.
(10) Sini Tang (Four Counteraction Decoction) [Lantai Guidfan Shanghan Fang]: Treats deep pulse, body pain, mild nausea, diarrhea with undigested food, cold extremities, internal Cold with external Heat, faint pulse nearing extinction. Zhigancao 2 liang, Ganjiang 1.5 liang, Fuzi (Aconiti Lateralis Radix Praeparata, 附子) one piece raw, peeled, broken into eight slices. Decoct the above three ingredients in 3 sheng of water to obtain 1.2 sheng, remove dregs, divide into two warm doses. Strong individuals may use a large Fuzi and 3 liang Ganjiang. Xu Lingtai said: "Sini Tang should not be used lightly. If symptoms are incomplete, examine carefully. Only use if all symptoms are present."
"Sini Tang" treats deep pulse, body pain, nausea, diarrhea with undigested food, cold extremities, internal Cold/external Heat, faint pulse. I believe its primary action warms the Foot-Shaoyin Kidney and Foot-Taiyin Spleen, exerting positive movement functions. Three Yin meridians receive Qi from the Five Zang organs; Three Yang meridians receive Qi from the four extremities. The Five Zang are the source of life energy (Root), while the Six Fu organs are branches receiving energy from the Zang. Cold extremities, diarrhea with undigested food, deep/faint/slow pulses indicate the Mingmen Fire of the Foot-Shaoyin Kidney is about to extinguish. Urgently use "Sini Tang" to return Yang to the Lower Jiao and secure the root of life.
TCM rule: Treat Exterior first, then Interior. However, if seeing diarrhea with undigested food, cold extremities, faint pulse, life energy is failing. Even with exterior symptoms, urgently abandon Exterior to save Interior. When the Foot-Shaoyin Kidney recovers Yang and pulse strengthens, indicating sufficient Sheng Qi, then treat the Exterior.
(11) Zhenwu Tang (True Warrior Decoction) [Lantai Guidfan Shanghan Fang]: Treats Taiyang sweating without resolution, persistent fever, palpitations, dizziness, tremors, feeling of falling. Also treats Shaoyang days 4-5, difficult urination, heavy/painful limbs, spontaneous diarrhea, indicating Water Qi. Fuling, Baishao, Shengjiang each 3 liang sliced, Baizhu 2 liang, Fuzi one piece prepared, peeled, broken into eight slices. Decoct the above five ingredients in 8 sheng of water to obtain 3 sheng, remove dregs. Take 0.7 sheng warm, three times daily. If coughing, add Wuweizi half sheng, Xixin (Asari Radix et Rhizoma, 细辛), Ganjiang each 1 liang. If urination is free, remove Fuling etc. If diarrhea, remove Baishao, add Ganjiang 2 liang. If vomiting, remove Fuzi, add Shengjiang (total 0.5 jin). Xu Lingtai said: "This is the correct formula for treating Water Counterflow."
