The Hand-Shaoyin Heart Meridian originates within the heart, emerging to connect with the Xinxi (Heart System). Since the Xinxi is not suitable for direct needling, needles are inserted beside its meridian to harmonize the Fire Qi. Another point involves the Foot-Yangming Stomach Meridian, which originates at the side of the nose, descends along the lateral aspect of the nose, and enters the upper teeth; needling here serves to harmonize the Earth Qi. A third location is the Guangming (Brightness) acupoint on the lower limb, located five cun above the lateral malleolus, belonging to the Foot-Shaoyang Gallbladder Meridian; inserting three needles around Guangming harmonizes the Wood Qi. Epileptic and convulsive disorders arise from the failure of Yin and Yang to interact and the disharmony between Water and Fire; therefore, needling methods targeting the Zang-Fu organs and Five Elements are employed to reflect the principles of regulating these systems.
In general, treating Xiao-Dan disease (Wasting-Thirst syndrome), Pu-Ji disease (sudden collapse as if struck), Pian-Ku disease (hemiplegia), Wei-Jue disease (limb flaccidity with cold extremities), and Qi-Man Fa-Ni (chest fullness with rebellious Qi), which often affect wealthy and obese individuals, addresses diseases caused by excessive consumption of rich foods. Ge-Sai Bi-Jue disease (obstruction and blockage preventing communication between upper and lower body) results from sudden severe worry. Sudden syncope accompanied by deafness, or unilateral blockage, stems from a violent internal surge of Qi rather than external Wind-Cold-Damp invasion; this represents a condition where pathogenic factors linger due to emaciation. Limping is caused by external invasion of Cold, Dampness, and Wind.
"Bao" (Sudden) implies abruptness; "Cun" refers to stepping; "Bo" means walking irregularly. Therapeutic approaches vary according to internal, external, upper, and lower etiologies, which must be investigated. Xiao-Dan originates internally; Pu-Ji originates externally. Pathogens in the upper jiao cause hemiplegia; pathogens in the lower jiao cause flaccidity and cold limbs; pathogens in the middle jiao cause chest fullness and rebellious Qi. These conditions—Xiao-Dan, Pu-Ji, Pian-Ku, Wei-Jue, and Qi-Man Fa-Ni—present as either Deficiency or Excess syndromes. For obese, wealthy individuals, the excess arises from rich diet causing Qi obstruction and blockage, often triggered by sudden emotional turmoil. Sudden Jue-ni with blockage in the upper jiao leads to syncope and deafness; blockage in the lower jiao leads to dysuria and constipation/diarrhea; unilateral blockage indicates an internal etiology of sudden emotional stress, termed "violent internal Qi pressure." If not caused by internal emotional stress but by external Wind invasion, it relates to constitutional emaciation with lingering pathogens. Irregular gait is not due to emaciation but to external Cold, Dampness, and Wind. All these diseases differ in Deficiency/Excess nature and should be treated based on their etiology.
The Yellow Emperor stated: Jaundice with sudden pain, Dian-Ji (epilepsy), Jue-Kuang (manic states), are generated by long-term reversal of Qi. Disharmony of the Five Zang Organs arises from closure/blockage of the Six Fu Organs. Headache, tinnitus, and dysfunction of the nine orifices arise from disorders of the stomach and intestines.
Following Qibo’s explanation, the Yellow Emperor clarified that each Zang-Fu disorder has specific origins; understanding these origins allows one to discern Deficiency/Excess patterns and facilitates treatment. Although jaundice, sudden pain, epilepsy, and mania appear suddenly, they stem from prolonged Qi reversal. The Qi of the Five Zang Organs requires balance; imbalance results from disharmony of the Six Fu Organs, thus arising from their blockage. Headache and tinnitus leading to dysfunction of the nine orifices result from insufficient intake of Gu-Qi (Grain Qi) and deficiency of the Gu-Shen (Spirit of Grain) internally, thus originating from gastrointestinal dysfunction.
