紧而滑，脾不能为胃行其津液，津液无以上承，所以口干不欲饮也。金匮云∶痰饮之病，宜以温药和之。拟苓
桂术甘合真武意，温肾运脾，降气纳气，俾阳光一振，则阴霾自除矣。

前方连服三剂，咳嗽脓血均减，身热亦退大半，原方去桃仁及薄荷叶，加轻马勃（八分）、通草（八分）。
崔左 咳呛已延月余，胸膺牵痛，痰味腥臭，临晚潮热，脉数苔黄，烦劳过度，五志化火，平素嗜酒，酒湿
生热，肝火湿热互蒸于肺，肺脏生痈也。急拟千金苇茎汤加味。

阴分素亏，木火刑金，湿热互蒸，肺痈早成，肺叶已伤，输转无权，惟虑由痈而痿，致入不治之条。

蠲化痰浊，清肃肺气，毋使过之，伤其正也。

Patient Sun, Left side: The Spleen is the source of phlegm generation, and the Lung is the vessel storing phlegm. Lung deficiency fails to descend Qi, and Kidney deficiency fails to grasp Qi, resulting in coughing, rapid breathing, and difficulty lying flat. The tongue coating is white and greasy; the pulse is wiry, tight, and slippery. The Spleen cannot transport fluids for the Stomach, so fluids fail to ascend to moisten the mouth, causing dryness of the mouth without desire to drink. The *Jingui Yaolue* states: "For diseases of Phlegm-Fluids, it is appropriate to harmonize with warm herbs." We propose combining the principles of Linggui Zhugan Tang (Poria, Cinnamon Twig, Atractylodes Macrocephala and Licorice Decoction) with Zhenwu Tang (True Warrior Decoction) to warm the Kidney and transform the Spleen, descending and grasping Qi. Once Yang energy is revitalized, Yin-cold pathogens will naturally dissipate.

This is a severe case. Urgently prescribe Acrid-Cool clearing of Warmth to resolve Phlegm and stasis.

After taking the previous prescription for three doses consecutively, coughing and purulent blood decreased significantly, and body heat subsided by more than half. Remove Tao Ren and Bohe Ye from the original formula, add Qing Mabo (Marasmus Mushroom) [8 fen] and Tongcao (Tetrapanax Pith) [8 fen].
Patient Cui, Left side: Coughing has persisted for over a month, with pulling pain in the chest and hypochondrium, foul-tasting sputum, evening tidal fever, rapid pulse, and yellow tongue coating. Due to excessive fatigue, the Five Emotions transformed into Fire; habitual alcohol consumption generates Damp-Heat from wine. Liver Fire and Damp-Heat mutually steam into the Lung, causing Lung Abscess (Fei Yong). Constitutionally deficient in Yin, Wood-Fire insults Metal (Liver Fire invading Lung), Damp-Heat mutually steams, Lung Abscess (Fei Yong) has formed early, Lung lobes are damaged, transport function is lost. We fear progression from Lung Abscess to Lung Flaccidity (Fei Wei), leading to an incurable state.

Second visit: After taking the previous prescription for twenty doses, coughing and foul sputum have greatly reduced. Add Ge Fen Chao Ejiao (Donkey-hide Gelatin fried with clam powder) [2 qian] and Mi Zhi Doulou (Sonchus Flower head honey-fried) [1 qian].
Patient Ju, Left side: Lung Abscess (Fei Yong) has persisted for two months. Coughing produces much pus and little blood, sputum is thick, turbid, and foul-smelling. Appetite is reduced, body is thin, spirit is fatigued, pulse is rapid and weak. Lung lobes are already corrupted, toxic heat lingers. The condition is dangerous. Tentatively prescribe methods to support the interior and expel pus, clear the Lung and resolve Phlegm. It remains to be seen if there will be a turning point.

Patient Wen, Left side: External contraction of Cold-Wind invading Lung and Stomach. Rich and fatty foods brew Phlegm-Turbidity. Blood stasis coagulates and obstructs between Lung lobes, forming Lung Abscess (Fei Yong). Hence, rough breathing due to coughing, filth-like purulent sputum, hypochondriac pain making turning difficult, aversion to cold with fever, thick white greasy tongue coating, floating, tight, and slippery pulse. The disease onset is sudden and urgent; only potent medicines can achieve success. Urgently imitate Shegan Mahuang Tang (Belamcanda and Ephedra Decoction) from *Yizong Jinjian* combined with Zaojiao Wan (Soap Pod Pill) from *Jingui Yaolue*. One disperses exterior pathogens, the other sweeps away Phlegm-Turbidity.

Second visit: The previous prescription dispersed Lung pathogens and swept away Phlegm-Turbidity. Sweating occurred, cold and fever resolved, coughing and rapid breathing alleviated. However, sputum remains thick, foul, and turbid. Chest fullness, hypochondriac pain, loss of appetite, short dark urine, greasy coating, and slippery-rapid pulse persist. Sticky Phlegm-Turbidity and accumulated Damp-Stasis are knotted between Lung lobes, difficult to clear instantly. Now, reduce the dosage slightly, eliminate Phlegm-Turbidity, clear and purify Lung Qi, avoiding excess to prevent damaging healthy Qi.
