Stool yellow is due to internal heat; red, crimson, and black are viewed similarly. Green, white, and cyan all indicate cold patterns; if accompanied by food stagnation, the odor is sour (37).
Prolonged diarrhea leads to flaccidity of the four limbs; sudden fright or restless sleep indicates instability. Heat-related malnutrition (Re Gan) presents with hair clumping like tufts; phlegm-cough suggests Taiyang Cold Damage (Shanghan).
Liver cold transmitting to the Spleen causes green/cyan stool with a foul odor; scorched yellow is the manifestation of Spleen Earth heat. Lung and Intestine cold present with sticky, white pus; redness and heat result from Heart and Kidney heat.
Song on Body Heat and Cold Extremities (Prolonged diarrhea often involves deficiency-heat)
Qi deficiency frequently induces cold extremities (Jue); Blood deficiency must manifest as heat. If both Qi and Blood are deficient, the body feels hot while the hands and feet are cold.
Vomiting and diarrhea combined are called Huoluan (Cholera-like syndrome), presenting in three types: Cold, Heat, and Food Stagnation.
When there is vomiting and diarrhea, reddish cheeks, generalized fever, thirst, frequent episodes with scanty discharge, this belongs to Heat. Treat with Wuling San (Five-Ingredient Powder with Poria) plus calcined Ginger, or administer Yi Yuan San (Benefit Prime Powder) cooled in calcined Ginger Decoction; the effect is miraculous.
When there is vomiting and diarrhea, pale complexion, cold feet, abdominal pain, frequent crying, no thirst, infrequent episodes with abundant discharge, this belongs to Cold. Treat with Lizhong Tang (Regulate the Middle Decoction). For severe cases, adding Fuzi (Aconite Lateral Root) and Wumei (Mume Fruit) is effective. In these two conditions, breast milk and food intake are not obstructed.
When vomit and stool are sour and foul-smelling, the face is yellow, appetite is poor, and the abdomen is often painful, this is Food Stagnation. Use Dingxiang Pi Ji Wan (Clove Spleen Accumulation Pill) to expel the stagnation, followed by Jisheng Wan (Accumulated Sage Pill) for regulation.
For prolonged vomiting and diarrhea with emaciation and heat, use only Jisheng Wan.
Ancestral treatment for vomiting and diarrhea, regardless of Cold, Heat, Deficiency, or Excess, uses Weiling Wan (Stomach Regulating Pill) delivered with calcined Ginger Decoction, bringing immediate relief.
Yi Yuan San (Also known as Liuyi San / Six-to-One Powder, also known as Tianshui San)
Huaishi (Talcum) six liang, Gancao (Licorice Root) one liang, ground into fine powder. Can be decocted and taken, or mixed with cold water.
If vomiting and diarrhea do not cease and thirst persists, it is incurable. If accompanied by convulsions, it is incurable. Mouth and tongue ulcers, cold hands and feet, and body heat indicate Yin descending and Yang ascending, with the interruption of Spleen Qi; it is incurable.
Xijiang Yue (Poem)
Generally, for male or female vomiting and diarrhea, one must understand the smoothness or reversal of Yin and Yang. In men, severe diarrhea depletes Yin below; in women, excessive vomiting is abnormal. Abundant discharge but infrequent frequency indicates severe Cold invasion. Infrequent discharge but frequent frequency requires careful assessment for Fire Excess.
When vomiting and diarrhea occur together, it is named Huoluan involving Yin-Yang imbalance. One dose of Lizhong Tang can resolve upper vomiting and lower diarrhea. If this is ineffective, add Fuzi and calcined Ginger, using Wumei as a guide; this is a good prescription, do not boast to laymen.
If vomiting and diarrhea cause constant thirst, and various decoctions fail, drinking fluids leads to abdominal distension, facing death helplessly. Urgently use Fulong Yinzi (Dragon's Kiln Mud Drink), administered intermittently; once vomiting stops and Fire Evil calms, then treat appropriately with Lizhong Tang.
Vomiting and diarrhea are divided into three syndromes: Food Stagnation, Cold, and Heat. Yellow face, foul feces, and aversion to milk/food indicate Food Stagnation. If body heat and thirst are present, it should be treated as Heat Syndrome. Pale face, cold body, and abdominal pain indicate Deficient-Cold disease.
Food Stagnation requires moving Qi and resolving accumulation; Lingying Quji Wan is suitable. For Heat Syndrome, Yi Yuan San and Wuling San are primary. For Cold Syndrome, Lizhong Tang is usable, adding Fuzi and Wumei for severe cases. For both Cold and Heat, Weiling Wan is unique, taken with dry ginger decoction.
