Pulse: In cases of the five types of jaundice presenting as excess-heat, the pulse must be surging and rapid; if it is faint and choppy, the condition belongs to deficiency.
Jaundice (Huang Dan): Although there are five types of jaundice, they all result from damp-heat steaming and fermenting within the Spleen and Stomach due to food and fluids, causing yellowing of the body. When symptoms include fullness and oppression in the chest and abdomen, yellowing of the face and whole body, and scanty dark-red urine resembling soap pod juice, it is akin to fermentation by yeast, indicating jaundice generated by damp-heat. Use Yinchen Siling San (Yinchen Four-Ingredient Powder) with modifications. If the patient presents with yellowing, thirst, and solid stool, use Yinchendahuang Tang (Yinchen Rhubarb Decoction) to purge, after which the yellow color will naturally recede. Recovery is indicated when the urine becomes clear and white. Many cases of jaundice resolve into drum distension (Gu Zhang).
The five types of jaundice arise from damp-heat stagnating and steaming within the Spleen.
Yinchen San (Yinchen Powder): Treats yellowing caused by damp-heat.
Ingredients: Yin Chen Hao (Artemisiae Scopariae Herba), Zhi Zi (Gardeniae Fructus), Chi Fu Ling (Red Poria), Zhu Ling (Polyporus), Ze Xie (Alismatis Rhizoma), Cang Zhu (Atractylodis Rhizoma), Zhi Shi (Aurantii Fructus Immaturus), Huang Lian (Coptidis Rhizoma), Hou Po (Magnoliae Officinalis Cortex), Hua Shi (Talcum) (all in equal parts).
Preparation and Administration: Coarsely crush the above ingredients for one dose, add a bunch of Juncus effusus (Deng Cao), and decoct in water for oral administration. Add Chai Hu (Bupleuri Radix) if there is body heat; add Huang Bo (Phellodendri Cortex) if urine is scanty and dark-red; add Luo Bo Zi (Raphani Semen) and Fu Ling (Poria) if there is fullness and oppression in the chest and diaphragm; for those who consume alcohol, add Gua Lou Ren (Trichosanthis Semen), Ge Gen (Puerariae Radix), and Sha Ren (Amomi Fructus), and remove Hua Shi (Talcum). This pattern often corresponds to Wine Jaundice (Jiu Dan).
Yinchendahuang Tang (Yinchen Rhubarb Decoction): Treats jaundice accompanied by constipation.
