When both urination and defecation are obstructed, it is due to the accumulation of heat in both the anterior and posterior orifices.
Diandao San (Inverted Scattering Powder): Treats excess heat in the Zang-Fu organs, manifesting as difficulty in urination, difficulty in defecation, or obstruction of both urination and defecation.
Ingredients: Radix et Rhizoma Rhei (Dahuang, 大黄) (6 qian), Talcum (Huashi, 滑石) (3 qian), Fructus Gleditsiae Sinensis (Zaojiao, 皂角) (3 qian).
Preparation and Administration: Grind the above ingredients into powder. Take with yellow wine. If only defecation is obstructed, take according to the aforementioned dosage; if only urination is obstructed, use 3 qian of Dahuang, 6 qian of Huashi, and Zaojiao as before; if both are obstructed, divide Dahuang and Huashi equally, and keep Zaojiao as before.
Qianglang San (Scarab Beetle Powder): Treats obstruction of both urination and defecation. In the sixth and seventh lunar months, locate scarab beetles (Qianglang, 蜣螂) in cow dung, string them together with thread, dry them in the shade, and store for use.
Usage: Take one complete beetle, place it on a clean brick, and bake it thoroughly with ash fire from all sides. Cut it in half horizontally with a knife. For constipation, use the upper half; for urinary retention, use the lower half. Grind each part into powder separately, mix with fresh well water, and take orally. If both urination and defecation are obstructed, use the whole beetle.
Bazheng San (Eight-Upright Decoction): See the section on Lin Syndrome (Strangury).
Tiejiao Wan (Iron-Foot Pill): Treats obstruction of both urination and defecation.
Ingredients: Fructus Gleditsiae Sinensis (Zaojiao, 皂角) (remove skin and seeds, stir-fry) as needed.
Preparation and Administration: Grind into powder, mix with wine and flour paste to form pills the size of Sophora japonica seeds. Take 30 pills per dose with wine.
Woniugao (Snail Ointment): Treats obstruction of both urination and defecation.
Take three snails, grind them with their shells into a paste, add a small amount of Moschus (Shexiang, 麝香), apply to the umbilicus, and massage with hands until immediate relief occurs. Alternatively, grinding field snails and applying them to the umbilicus is also effective. I have used this prescription to treat cases of obstruction of both urination and defecation and heat-closure with remarkable efficacy.
Yanqi Fa (Umbilical Covering Method): Treats obstruction of both urination and defecation.
Ingredients: Scallion with roots intact (one stalk, unwashed, with soil), Fresh Ginger (one piece), Fermented Soybean (Danshidou, 淡豆豉) (20 grains), Salt (two spoons).
Preparation and Administration: Grind all ingredients into a smooth paste, shape into a cake. Heat it slightly and apply to the umbilicus, securing tightly with a cloth. After some time, qi will flow naturally and relief will occur. If not relieved, replace with a new cake.
Another Prescription: Treats obstruction of both urination and defecation.
Take one cup of honey, add 2 qian of Natrii Sulfas (Pinxiao, 皮硝), and mix with one cup of boiling water. Take on an empty stomach.
For obstruction of both urination and defecation: Grind Akebiae Caulis (Mutong, 木通) into powder and take with yellow wine. Adults take a larger dose, children take a smaller dose.
Dingxiang San (Clove Powder): Treats obstruction of both urination and defecation with miraculous effect.
Ingredients: Bitter Clove Bud (Kudingxiang, 苦丁香) (5 qian), Prepared Aconitum carmichaelii (Chuanwu, 川乌), Aconitum kusnezoffii (Caowu, 草乌), Angelica dahurica (Xiangbaizhi, 香白芷), Prepared Fructus Gleditsiae Sinensis (Yaogao, 牙皂), Asarum (Xixin, 细辛) (3 qian each), Piper nigrum (Hujiao, 胡椒) (1 qian), Moschus (Shexiang, 麝香) (a little).
