Pulse: The pulse of hernia (Shan) is wiry and rapid, resulting from the accumulation of pathogenic factors; observing the location of the pulse, the Liver is the primary organ involved in hernia. A slippery pulse at the Heart position and a deep pulse at the Lung position indicate that Wind Hernia (Feng Shan) is prone to dissipate. A floating and slow pulse at the Guan position indicates Wind Deficiency syndrome. Spasm of the Yang sinews forms a mass (Jia), while spasm of the Yin sinews presents as hernia symptoms. A deep-slow or floating-choppy pulse indicates cold pain associated with hernia and masses. When pain is severe, the pulse may become hidden (Fu), thready, or erratic. A firm and rapid pulse suggests a favorable prognosis, whereas a weak and rapid pulse suggests an unfavorable one.
Hernia Qi (Shan Qi): Hernia originates from the Liver Meridian; treatment should focus on unblocking rather than obstructing, and it is not directly related to the Kidney Meridian. Some cases present without a visible mass but with audible sounds; others present with a visible mass resembling a melon, accompanied by sounds like a frog croaking. This constitutes Hernia Qi disease. Initially, damp-heat stagnates in the meridians for a long time, followed by external cold constriction, preventing dispersion, thus causing pain. One must not rigidly classify this solely as a Cold syndrome; instead, herbs with both cold and hot properties should be combined. Shen Xiao Tang (Miraculous Efficacy Decoction) with modifications should be used: Chuan Wu (Prepared Aconite Root) to disperse cold, and Shan Zhi (Gardenia Fruit) to clear damp-heat. These are primary herbs for treating the Lower Jiao, yielding rapid results.
Gas moving in the intestines producing sound or pain indicates Pan Chang Qi (Coiled Intestine Qi). Pain and audible sounds upon palpation of the small intestine and scrotum indicate Bladder Qi (Pang Guang Qi). A knot rising from beside the umbilicus in the small intestine region causing hook-like pain indicates Small Intestine Qi (Xiao Chang Qi). Gas rushing downward from the lower abdomen and upward to the heart and abdomen causing acute pain indicates Kidney Deficiency (Shen Xu). Asymmetry in size of the testicles indicates Pian Zhui (Unilateral Descent). Hardness with swelling but no pain in the testicles indicates Shui Shen Qi (Water Kidney Qi). All hernia conditions are mostly caused by heat stagnating internally and cold constricting externally. (The following formulas are suitable for all the above.)
Shen Xiao Tang (Miraculous Efficacy Decoction): Treats all types of hernia.
Mu Xiang (Aucklandiae Radix, ground separately) 7 fen, Wu Zhu Yu (Evodiae Fructus) 7 fen, Hui Xiang (Foeniculi Fructus, stir-fried with wine), Yan Hu Suo (Corydalis Rhizoma), Yi Zhi Ren (Alpiniae Oxyphyllae Fructus), Cang Zhu (Atractylodis Rhizoma, soaked in rice water), Xiang Fu (Cyperi Rhizoma), Dang Gui (Angelicae Sinensis Radix), Chuan Wu (Aconiti Radix Praeparata, processed, skin removed, dose halved), Shan Zhi (Gardeniae Fructus, stir-fried) 1 qian, Sha Ren (Amomi Fructus Rotundus) 7 fen, Gan Cao (Glycyrrhizae Radix et Rhizoma) 3 fen.

If distension and pain occur, add Ru Xiang (Boswelliae Resina) and Zhi Shi (Aurantii Fructus Immaturus); if blood stasis causes distending pain, add Tao Ren (Persicae Semen) and Chuan Xiong (Ligustici Chuanxiong Rhizoma), removing Yi Zhi Ren and Shan Zhi. If Kidney Qi rushes upward causing severe chest pain and suffocation, add Chen Xiang (Aquilariae Lignum Resinatum) and Zhi Shi, removing Yi Zhi Ren and Shan Zhi.
Wu Ling Tong Qi San (Wu Ling Gas-Unblocking Powder): Treats all types of hernia, regardless of duration, cold-heat nature, or wind-damp-cold pathogens.
Wu Yao (Linderae Radix), Dang Gui, Shao Yao (Paeoniae Radix), Xiang Fu, Tang Qiu (Juglandis Semen, i.e., Walnut Shell/Kernel contextually often refers to Ju He here or similar fruit seed, WHO standard uses Juglandis Semen but text says Tang Qiu which is colloquial for walnut or similar, strictly translating as per text "Tang Qiu" but noting it likely refers to a seed/herb, here translated as Juglandis Semen/Walnut based on common usage in such contexts or kept as Tang Qiu if uncertain, however standard TCM herb list doesn't have Tang Qiu, it's likely a typo for Ju He or similar. Given "Ju He" appears later, Tang Qiu might be a specific local name. I will translate as "Tang Qiu (Walnut Kernel/Juglandis Semen)" for clarity or stick to literal if unsure. Actually, Tang Qiu is often a folk name for Juglandis Semen or similar. Let's use Juglandis Semen.), Chen Pi (Citri Reticulatae Pericarpium) 1 qian each, Fu Ling (Poria), Bai Zhu (Atractylodis Macrocephalae Rhizoma, remove rhizome head), Bing Lang (Arecae Semen), Yan Hu Suo, Ze Xie (Alismatis Rhizoma) 5 fen each, Mu Xiang, Gan Cao 3 fen each.

Remove residue, add half a bowl of good wine, mix well. Add one teaspoon of roasted salt. Take hot on an empty stomach. For severe pain, one dose brings immediate cure.
For hernia triggered by overexertion, this involves deficiency. It occurs upon exertion; the pulse is deep-tight, wide, and forceless, indicating mixed deficiency. The pain is mild, characterized mainly by heaviness and dragging sensation.
He Qi Yi Rong Tang (Harmonizing Qi and Nourishing Blood Decoction): Treats hernia pain with mixed deficiency.
Ren Shen 5 fen, Dang Gui, Chuan Xiong, Qing Pi (Citri Reticulatae Pericarpium Rubrum, remove pith), Hui Xiang (salt and wine stir-fried), Yan Hu Suo, Cang Zhu (rice water soaked) 1 qian each, Mu Xiang (ground separately), Chen Xiang (ground separately), Chuan Wu (processed, skin removed) 5 fen each, Shan Zhi, Sha Ren, Wu Zhu Yu (stir-fried) 7 fen each, Gan Cao 2 fen.

Case 1: A boatman suffered from Small Intestine Hernia with unbearable swelling and pain. He also had swollen, painful eyes with discharge, tearing, and itching. Pulse: Cun positions were flooding and rapid; Chi positions were faint. This indicated Excess above and Deficiency below. Cooling herbs for the eyes worsened the hernia pain; warming herbs for the hernia worsened the eye pain. Physicians were helpless. I administered Mu Xiang Jin Ling Wan orally on an empty stomach to treat Lower Jiao deficiency-cold, and Tui Xue San (Blood-Dispersing Powder) before bed to treat Upper Jiao wind-heat. Three doses of each resulted in complete recovery.
Case 2: Zhao Xueshan developed unbearable hernia pain and swelling, with aversion to cold and shivering, after sexual activity followed by rising early at dawn and exposure to cold. Pulse: All six positions were faint and forceless. I prescribed Wu Ji San modified with Wu Zhu Yu and Xiao Hui Xiang, and Pan Cong San, but neither was effective. Later, I applied moxibustion. The patient’s feet were brought sole-to-sole and bound. Moxa cones the size of wheat grains were applied to the gap between the middle fingers. After 7 cones, the pain ceased. The effect was miraculous.
