Treatment for Kidney Accumulation (Shen Ji): Manifesting in the lower abdomen, extending upward to the heart, resembling the running of a pig, moving up and down without fixed timing. If prolonged and unhealed, it presents with wheezing and counterflow qi (Chuan Ni), bone flaccidity (Gu Wei), and shortness of breath. The pulse is deep and slippery.

The Six Fu Organs (Liu Fu) include the Large Intestine, Small Intestine, Stomach, Gallbladder, Bladder, and San Jiao (Triple Burner), belonging to the Three Yang meridians. Tai Yang (Greater Yang) facilitates the flow of clear qi; Yang Ming (Bright Yang) discharges turbid qi; Shao Yang (Lesser Yang) transforms essence qi. Like the capital city's administration, their normal function is transport and transformation. When the Six Fu organs malfunction, stagnation and accumulation occur, leading to obstruction. Thus, in excess conditions, there is no rotation; in deficiency conditions, there is no transmission. Qi moves back and forth, causing pain without a fixed location. In the upper part, there is regurgitation or blockage; in the lower part, there is distension. It attacks both hypochondriac regions, resembling a mass (Pi Kuai), and is prone to change, differing from the pathology of the Five Zang organs.

Treatment for Long-standing Qi Accumulation: Resembling a movable mass (Jia), moving up and down with qi, occurring at fixed times. Symptoms include severe colicky pain in the heart and abdomen, radiating to the waist and hypochondrium, suffocating qi, wheezing and coughing, chest and epigastric fullness, lower abdominal distension, difficulty in urination and defecation, or diarrhea with profuse discharge, spermatorrhea, or white turbid discharge, resembling consumptive disease (Xu Lao).

Ban Xia (Pinelliae Rhizoma, washed seven times with decoction) 3 fen; Bing Lang (Arecae Semen) 3 fen; Dang Gui (Angelicae Sinensis Radix) 3 fen; Ju Pi (Citri Reticulatae Pericarpium) 2 liang; Xing Ren (Armeniacae Semen Amarum, bran-fried, skin and tips removed) 2 liang; Gui Xin (Cinnamomi Ramulus/Core) 2 liang; Fu Ling (Poria) 2 liang; Zhi Gan Cao (Prepared Glycyrrhizae Radix et Rhizoma) 2 liang; Pao Fu Zi (Prepared Aconiti Lateralis Radix Praeparata, peeled and navel removed) 2 liang; Chuan Xiong (Ligustici Chuanxiong Rhizoma) 2 liang; Zhi Ke (Aurantii Fructus Immaturus, bran-fried, albedo removed) 2 liang; Hou Po (Magnoliae Officinalis Cortex, processed with ginger juice) 2 liang; Wu Zhu Yu (Evodiae Fructus, washed with decoction) 1 liang.

Crush the above ingredients into a coarse powder. Take 4 qian per dose, add one and a half zhan of water, boil until reduced to 7/10, strain, and take before meals. If there is difficulty in defecation, add Da Huang (Rhei Radix et Rhizoma).

The patient has fullness under the hypochondrium, counterflow qi, and difficult breathing, persisting for years without cure, named Xi Ji (Breathing Accumulation). This is caused by qi stagnation and癖 (pi, localized accumulation) under the hypochondrium, not located between the Zang-Fu organs or Ying-Wei (Nutritive-Defensive) systems. After long-term accumulation, the qi does not interfere with the Stomach, so appetite is not affected. Moxibustion causes internal burning heat; acupuncture disturbs the meridians. Although medications for resolving accumulation are taken, they cannot cure it alone. One should resolve the accumulation and assist treatment with Dao Yin (qigong exercises).
