Postpartum "Erzhen" (a mass formed by retained blood clots after childbirth) fails to dissipate, which is due to the hard accumulation of Xuejia (abdominal masses). It feels grasping upon palpation, worsens in the late afternoon (Dibang), and presents with lingering pain and discomfort. The pathogenic factor attacks upward to the chest and heart, causing extreme exhaustion and distress. It is appropriate to administer Heilong Dan (Black Dragon Pill), Danggui Tang (Angelica Decoction), and Duoming Dan (Life-Rescuing Pill) for treatment.
To treat postpartum blood disorders nearing death. Administering this medicine is invariably effective; its miraculous efficacy cannot be described in words.
Danggui (Radix Angelicae Sinensis), Wulingzhi (Trogopteri Dung), Chuanxiong (Rhizoma Chuanxiong), Gaoliangjiang (Rhizoma Alpiniae Officinarum), Gan Dihuang (Radix Rehmanniae Preparata - use raw form, each 1 liang).
Finely chop all ingredients. Place them in a sand box the size of a roof tile. Seal the seams with Chishizhi mud (Halloysitum Rubrum) reinforced with paper fibers, and secure tightly with salt mud. Seal the lid, and roast over ten jin of charcoal until red hot. Remove from fire, allow to cool, open the box, and observe that the contents have turned into black dregs. Take out and grind finely, then add the following herbs:
Baicaoshuang (Soot from Kitchen Chimney, separately ground, 5 liang), Liuhuang (Sulfur), Ruxiang (Resina Frankincense, each 1 qian half), Huarushi (Lapis Lazuli), Hupo (Amber, each 1 qian).
Grind all into a fine powder. Use rice vinegar to make a paste and form pills the size of a bullet. Take one pill per dose. Roast the pill in charcoal until red hot, then crush it with fresh ginger juice and unadulterated wine (each 1 he) and half a cup of urine. Mix well and take at once for immediate effect.
To treat postpartum failure of defeated blood (retained lochia) to dissipate, hardness and pain of Erzhen, which may come and go; as well as cases where new mothers, taking advantage of deficiency, suffer from internal invasion of wind-cold, leading to sluggish lochia and hard, distended umbilical and abdominal regions.
Danggui (Radix Angelicae Sinensis, slightly stir-fried), Guijian (Ramulus Corni, use the feather-like parts), Honglanhua (Flos Carthami Tinctorii, each 1 liang).
Chop into coarse powder. Take 3 qian per dose, decoct in one large cup of wine until reduced to 70%, remove dregs, and take warm before meals.
Duoming Dan (Life-Rescuing Pill): See the formula in the previous second discussion.
Postpartum care is the most difficult to regulate. Therefore, it is taboo to ask about the baby's gender, and contact with filth is prohibited. These are ancient precautions against delusional thoughts. It is often seen that women rejoice if they bear a boy but worry if they bear a girl. Such premature joy or sorrow leads to heart deficiency and vexation, often starting here. Since excessive blood loss occurs, blood deficiency leads to yin deficiency. Yin deficiency generates internal heat. This internal heat is called "Fan" (vexation). Its symptoms include fullness and vexation in the chest and heart, shortness of breath, headache and confusion, bone and joint pain, worsening in the late afternoon, resembling the deficiency vexation seen after severe illness.
To treat postpartum vexation and restlessness.
Renshen (Radix Ginseng), Danggui (Radix Angelicae Sinensis), Maimendong (Radix Ophiopogonis, remove the heart), Gan Dihuang (Radix Rehmanniae Preparata), Guixin (Cortex Cinnamomi), Shaoyao (Radix Paeoniae Alba, each 1 liang); Shaoyao (2 liang).
Chop into coarse powder. Take 4 da qian per dose. Add two cups of water, first decoct one he of Jingmi (Oryza Sativa Glutinosa) and ten leaves of Danzhuye (Herba Lophatheri) until reduced to one cup. Remove rice and leaves, add the herbal powder and three jujubes. Decoct until reduced to 70%, remove dregs, and take before meals. Gan Dihuang should preferably be used in its raw dried form; if deficiency is severe, use prepared (cooked) Rehmannia.
