Patients exhibiting speech confusion have two types of syndromes: deficiency and excess. Deficiency manifests as Zheng Sheng (repetitive, low-voiced muttering due to mental confusion), while excess manifests as Chan Yu (delirious speech). Stomach excess in Shang Han (Cold Damage) and combined disease of the Three Yang meridians, heat entering the Blood Chamber in women, and Chan Yu accompanied by diarrhea all belong to excess syndromes; clear urination and defecation, cold extremities, faint and fine pulse, and earnest/repetitive speech all belong to deficiency-cold syndromes. Treatment varies by formula: for deficiency-cold, apply warming methods such as Si Ni Tang (Four Reversal Decoction); for stomach excess, apply purgative methods using Cheng Qi Tang (Ordering Qi Decoction). For heat entering the Blood Chamber in women, administer Xiao Chai Hu Tang (Minor Bupleurum Decoction). If Chan Yu is accompanied by four-reversal (cold extremities) and a deep, fine pulse, it is untreatable.
Si Ni Tang (Four Reversal Decoction) (see Liang Gan Men for formula)
Chai Hu Tang (Bupleurum Decoction) (see Shao Yang Jing for formula)
Da Cheng Qi Tang (Major Order Qi Decoction) (see Yang Ming Jing for formula)
After a major illness, heart deficiency causes irritability and restlessness with fever. This resembles Shang Han (Cold Damage) but differs by the absence of aversion to cold and headache. If sweating occurs, use Ren Shen Zhu Ye Tang (Ginseng and Bamboo Leaf Decoction); if vomiting occurs, use Ju Pi Tang (Tangerine Peel Decoction). Many post-illness conditions present similarly; see respective sections for details.
Treats internal and external deficiency and irritability following diaphoresis or purgation, where attacking methods are contraindicated.
