Before the eruption of measles (Rash), there may be Stomach Fire, or after the eruption, residual toxic heat fails to dissipate. This heat-toxin gathers in the upper and lower gums, causing sores on the lips and mouth. In such cases, wash with warm rice-washing water more than ten times daily (or rinse with Raw Licorice Decoction), and urgently administer detoxifying herbs. If treatment is delayed, it may transform into "Zou Ma Gan" (Gangrenous Stomatitis).
Between one to five or six days after the initial fever of the rash, abdominal pain is common. This is due to Large Intestine Fire stagnating within the Spleen's orifice, causing abdominal pain. Do not mistake this for Food Stagnation; using digestive-promoting herbs or rubbing the abdomen can cause harm. Only by resolving the rash toxin will the abdominal pain cease upon its dissipation; utmost caution is required.
If residual toxins remain after the rash subsides, they should be cleared immediately. If left unresolved for a long time, it will inevitably lead to wheezing and coughing, phlegm rattling in the throat, coldness and numbness in the four limbs, lusterless eyes with a cyan-white complexion, nostrils flaring like bellows, or inability to produce a cough sound. If the pulse at the right hand is scattered when lightly palpated and completely imperceptible when heavily pressed, it becomes a difficult-to-treat condition.
After the rash recedes, if there is slight body heat, this is Deficiency Heat, which requires no treatment; once Qi and Blood flow smoothly, the heat will subside spontaneously. If the heat is severe or persists for a long time, Chai Hu Mai Men Dong San (Bupleurum and Ophiopogon Powder) is suitable; in severe cases, Huang Lian Jie Du Tang (Coptis Decoction for Relieving Toxicity) or Ren Shen Bai Hu Tang (Ginseng White Tiger Decoction) may be used.
If heat persists after the rash, accompanied by dry, upright hair, emaciation, and skeletal prominence, leading to gradual wasting, it constitutes Gu Zheng Lao Zhai (Bone-steaming Consumption Disease). Wan Shi Chai Hu Si Wu Tang (Wan’s Bupleurum Four Substances Decoction) is indicated, or Lu Hui Fei Er Wan (Aloe Fattening Infant Pill) added with Dang Gui and Lian Qiao. Delayed treatment may lead to symptoms such as eyes half-open during sleep, cold breath from mouth and nose, and cold extremities, eventually forming Man Pi Feng (Slow Spleen Wind/Chronic Pediatric Convulsion), a convulsive condition that is incurable.
If heat remains after the rash and sudden convulsions occur, this should not be treated as Ji Jing Feng (Acute Convulsive Wind). Instead, use Dao Chi San (Guide Out Red Powder) added with Ren Shen and Mai Men Dong to deliver Qi Wei An Shen Wan (Seven-Ingredient Calming Spirit Pill). Clear urine indicates treatability; scanty urine indicates difficulty. If excessive phlegm is present, Bao Long Wan (Embracing Dragon Pill) may be used, or Si Wu Tang (Four Substances Decoction) added with Mai Men Dong, Suan Zao Ren, Dan Zhu Ye, Gan Cao, Long Dan Cao, Huang Lian, Fu Ling, Chen Sha, and Shi Chang Pu. These can also be powdered and formed into pills with steamed dough and pig heart blood for administration.
After the rash fades, coughing is common. Persistent mild coughing indicates residual toxins; clear Lung Drink (Qing Fei Yin) added with Raw Gan Cao and Niu Bang Zi is primary. Severe coughing with rebellious Qi and continuous onset indicates latent fire in the Lung and deficiency of Metal causing leaf-like scorching; Qing Fei Yin or Qing Fei Tang combined with Ren Shen Bai Hu Tang and Liu Yi San is appropriate. Sudden coughing with body heat, even causing food and drink to be regurgitated, or hemoptysis, results from heat-toxin attacking the Lung; Men Dong Qing Fei Tang (Ophiopogon Clearing Lung Decoction) with added Lian Qiao, or Qing Jin Jiang Huo Tang (Clear Metal Descending Fire Decoction) should be used extensively. Severe coughing with facial edema, swollen eyes, distended chest, rapid breathing, hematochezia, epistaxis, cyan-red complexion, coma, irritability, and head shaking constitutes a death sign. Additionally, if inherent Lung Qi deficiency is forced by toxins to cause persistent wheezing, without hemoptysis or choking on food, Qing Fei Yin with doubled Ren Shen is suitable. One must not rigidly adhere to the theory of Lung Heat and solely use clearing-Lung and detoxifying herbs.
If residual heat remains after the rash, or severe heat causes bleeding, Si Wu Tang added with Yin Chen and Mu Tong promotes urination; as heat qi descends, recovery occurs. For bleeding located superiorly, remove Chuan Xiong.
If residual toxins enter the Stomach and fail to dissipate over time, causing black rotting gums,腐 flesh, bleeding, and foul odor, it is named Zou Ma Gan. Ma Ming San (Horse Cry Powder) is primary. In severe cases, urgently use Ren Zhong Bai, Lu Hui, Shi Jun Zi, Long Dan Cao, Huang Lian, and Wu Ling Zhi, soaked and formed into pills with steamed dough, taken with boiling water to clear Stomach Fire. Cheek swelling, cyan-black color around the mouth, tooth loss, lip collapse, and nasal destruction constitute death signs. After the rash recedes, if diet and movement are normal, but sudden cardiac and abdominal colic occur with profuse sweating like rain, this is due to deficient Yuan Qi and lack of care. Although externally asymptomatic, internally there is substantial depletion, caused accidentally by malignant Qi invasion, termed Zhong E (Sudden Evil Invasion), a condition where morning onset leads to evening death.
