[Background Information]
Book: Key to Traditional Chinese Medicine Chapter: Main Text (8/42) Category: Medical Discourse
1. Common Cold and Fever: It is not caused by the invasion of external bacteria or viruses into the body, nor is it due to the loss of control of the central nervous system regulating body temperature. It is merely that the body's epidermis and pores are blocked. It is divided into Wind-Cold (injury by wind), Cold Damage (injury by cold), Summerheat Injury (injury by summerheat), Dampness (injury by dampness), Heat (injury by heat), etc., which TCM calls Exterior Syndrome (syndromes where the pathogen is located in the exterior). Bacteria and viruses do not enter the body at all. The Classics state: "Cold evil binds from outside, Yang cannot escape, stagnates and becomes heat causing fever." See *Shanghan Lun* (Treatise on Cold Damage) and *Wenbing Tiaobian* (Systematic Differentiation of Warm Diseases).
2. Food Stagnation:
Improper diet, combined with Spleen-Stomach deficiency, leads to food accumulation in the intestines and stomach. Inability to digest causes food to pile up and ferment, generating heat energy and causing fever, most commonly seen in infants and young children. This type of fever presents with hot palms and soles, unlike the cold limbs seen in common colds. Fever subsides and recovery occurs once digestion is resolved.
3. Deficiency Labor (Chronic Deficiency):
Due to bodily weakness, Water and Fire (Kidney and Heart) fail to interact (Heart-Kidney non-interaction), Liver Wind shakes, causing alternating aversion to cold and high fever. This is body temperature escaping outward, unable to generate physical energy for the body; it is not inflammation. It is common in women during menopause and those with declining health. Therefore, one must differentiate between Heart-Deficiency Fever, Spleen-Deficiency Fever, and Liver-Kidney Deficiency Fever for treatment. One should not suppress fever; this is called Deficiency Fever.
4. Blood Stasis:
During injury, Qi and Blood circulation are obstructed by blood stasis. Accumulated blood stasis raises body temperature, causing fever. Resolving blood stasis cures the fever. Many patients who become vegetative after injury suffer from blood stasis obstruction. Some have their blood stasis frozen by ice packs, preventing dispersion; others are treated with strong anti-inflammatory drugs. Anti-inflammatories damage the Heart and Kidneys. Since Heart and Kidney belong to the Shaoyin Meridian, and Kidney governs bones, bones govern marrow, and the brain is the Sea of Marrow, the cause of a vegetative state after injury is the decline of Heart and Kidney function. Furthermore, anti-inflammatories weaken Heart circulation, making it unable to expel blood stasis. It is not a problem of brain injury. Resolving blood stasis stops fever; tonifying Heart and Kidneys allows blood stasis to be expelled, and vegetative patients can recover.
Many girls aged thirteen or fourteen receive German Measles vaccines during their first menstruation, or take anti-inflammatories for colds during menstruation, damaging Heart and Kidneys. This leads to prolonged menstruation or retrograde menstruation bleeding from nose and teeth. Hemostatics cause internal blood stasis. Subsequent colds treated with anti-inflammatories again damage Heart and Kidneys. This is the main cause of leukemia in twelve-to-thirteen-year-old schoolchildren today—fever unrelenting due to blood stasis. See sections on Uterine Cancer, Systemic Lupus Erythematosus, and Leukemia.
5. Eruption of Pox/Rash:
Fever during pox/rash eruption promotes the production of immune functions by the virus, similar to the temperature generated when grains or seeds sprout. Pox/rash must involve fever to drive the virus out completely, producing immune function, adjusting the Five Zang organs and Five Qi without obstruction. Thus, fever during rash eruption does not require suppression. Attention must be paid to diet during eruption to avoid complications. Once the rash fully erupts, fever subsides, recovery occurs, immune function is established, and illness is less likely to recur. See the section "Vaccines Destroying National Seedlings."
6. Drug Poisoning:
Drug poisoning caused by the Triple Vaccine results in headache, vomiting, and fever. Western medicine calls this aseptic meningitis or leukemia if white blood cells rise to 30,000–50,000 and fever persists. Detoxification resolves fever, and white blood cell counts drop, leading to recovery.
7. Teething Fever in Children:
Teeth are the surplus of bone Qi, and Kidney governs bones. When infants teethe, Spleen Earth deficiency fails to control Kidney Water. Earth failing to control Water is due to Spleen deficiency, not infection or inflammation. Regulating Spleen and Stomach cures fever. Anti-inflammatories and antipyretics should not be used.
8. Fright:
Unsmooth Seven Emotions, such as abdominal distension and fever caused by fright in children, resolve with calming procedures. Emotional stagnation, depression, and worry damage Spleen, causing fever. Reducing gas relieves fever.
9. Postpartum Cold in Women:
Postpartum colds with fever are called "Wind within the Month" and are treated differently from common colds. At this time, Qi and Blood are severely deficient. Treatment must regulate Qi and Blood while dispelling Wind. Fever subsides only then. It should not be treated as an infectious inflammation.
10. Fever of Unknown Origin:
Fever that physicians do not understand, misdiagnose, mistreat, or cannot treat is today called fever of unknown origin. Everything has a cause; why is there fever of unknown origin?
(II) Differentiation of Fever – Can Fever Burn the Brain? How Do Encephalitis and Meningitis Occur?
Today’s young parents panic at the slightest runny nose or fever, fearing the brain will burn out. Doctors tell them fever is caused by invading bacteria or viruses controlling the thermoregulatory center, claiming no germ invasion means no fever, and that fever complicates into encephalitis/meningitis, causing deafness, blindness, etc. Parents live in terror. What is the true cause of "brain burning"?
During a cold, skin pores are blocked by Wind, Summerheat, Cold, Dampness, Dryness, and Fire. Fever is merely the blockage of the epidermis and pores, preventing sweat discharge and heat dissipation. TCM calls this Exterior Syndrome; bacteria/viruses have not entered the body. If they enter the interior, it is Interior Syndrome, and fever typically does not occur.
For general cold fever, inducing sweat allows pores to dissipate heat, resolving fever and curing the disease. Diagnosis focuses on whether Wind, Summerheat, Cold, Dampness, Dryness, or Fire injured the body, and which meridians were affected, to appropriately dispel Wind, scatter Cold, clear Summerheat, remove Dampness, descend Fire, and moisten Dryness.
*Shanghan Lun* elaborates extensively on meridian lesions from Cold Damage, Wind Injury, and their mistreatment. *Wenbing Tiaobian* thoroughly explains Summerheat, Dampness, and Dryness injuries, supplementing *Shanghan Lun*'s explanation of the Five Qi.
How to differentiate Wind, Cold, Summerheat, Dampness, and Dryness? Briefly: The Classics state: "When humans contract Cold, they develop febrile disease. Cold initially attacks the Exterior, specifically the Foot-Taiyang Bladder Cold-Water Meridian. Taiyang is the master Qi of all Yang meridians, the framework of the body. Its pulse starts at the inner canthus, goes up to the brain, down the neck, along the shoulders, flanking the back, reaching the waist, running behind the body. Thus, pain occurs everywhere it passes."
Aversion to Cold (E Han) exists even without wind; Aversion to Wind (E Feng) exists only when facing wind. Thus, Aversion to Cold always accompanies Aversion to Wind/Cold.
Wind and Cold have Yin/Yang distinctions. Aversion to Wind belongs only to Yang meridians; thus, Three Yin meridians have no Aversion to Wind syndrome. Wind is Yang Evil, Cold is Yin Evil.
*Yuan Bing Shi* states: "Body heat with aversion to cold indicates Heat in the Exterior. Heat in the Exterior is shallow; evil fears righteous Qi, so one feels hot yet aversed to cold. Saying 'Cold in Exterior, Heat in Skin, Cold in Marrow' is wrong."
All injuries by Wind, Summerheat, Dampness produce sweat; only Cold Damage produces no sweat. Cold congeals Blood and represents Exterior Excess. Qi rebels upward causing panting; pathogen in Exterior causes floating pulse. Tight pulse in Cold Damage vs. slow pulse in Wind Injury: Cold is forceful/tight, Wind is loose/dispersed.
Yu Jiayan said: Winter Cold Damage, Spring Warmth Injury, Summer/Autumn Summerheat/Dampness/Heat are the correct diseases of the four seasons. However, Summer/Autumn can have Cold Damage, Winter/Spring can have Summerheat/Dampness/Heat, which are guest diseases. Winter/Spring correct diseases: Sweating = Wind Injury; No sweating = Cold Damage. Summer/Autumn correct diseases: Sweating = Summerheat/Dampness/Heat; No sweating = Still Cold Damage.
*Neijing, Yin-Yang Yingxiang Dalun* says: "Wind excess causes movement; Heat excess causes swelling/inflammation; Dryness excess causes dryness/thirst; Cold excess causes edema/dizziness; Dampness excess causes loose stools." Thus: "Cold evil binds from outside, Yang cannot escape, stagnates and becomes heat causing fever."
From above, cold fever belongs to Wind, Cold, Summerheat, Dampness, Heat, Dryness/Fire. Disease is only in skin/epidermis, not entering Zang-Fu or Brain. Yet, anti-inflammatories damage Heart/Kidney, and IV fluids/ice pillows introduce Exterior pathogens into the interior. See "Are IV Fluids Nutritional or Deadly?" Heart and Kidney belong to Shaoyin. Anti-inflammatories harm them. Kidney governs bone/marrow/brain. External Wind/Summerheat/Dampness/Dryness/Cold introduced into Heart/Kidney causes encephalitis/meningitis. This results from mistaking Summer heat/cold for Winter Cold Damage, using sweating/anti-inflammatories incorrectly.
Encephalitis/meningitis are caused by mistaking Wind/Summerheat-Dampness fever for Cold Damage (incorrect sweating or purging/anti-inflammatories). TCM calls these Gang-Jing (Rigid Spasm), Rou-Jing (Flaccid Spasm), or Acute/Chronic Convulsions, not "burnt brain."
Spasm: Taiyang Wind Injury compounded by Wind-Dampness (like modern IVs/anti-inflammatories drawing Wind inward). Wind-Dryness moves, Cold pulls tight, Dampness sticks/constricts. Hence head shaking, jaw clenching, stiff neck, opisthotonos. Wind-Cold congeals blood/no sweat = Gang-Jing. Wind-Dampness releases fluid/sweat = Rou-Jing. Tendon tension/shrinking = Zhong. Tendon relaxation/laxity = Chi. Continuous contraction/relaxation = Chi-Zhong. Pathogen moving to Heart causes coma/unconsciousness.
(III) Should One Drink More Boiled Water During Fever?
1. How to diagnose fever? How to handle it?
Fever severity (temperature height) does not determine harm to the body. Wrong medical concepts, diagnoses, and medications are the real fatal blows.
Do not panic over 39°C, 40°C, 41°C, 42°C. Thermometers mislead people. Oral, axillary, rectal temperatures vary by time of day and age. Thermometers cannot distinguish time, location, meridian, adult/child, or cause. Treating merely to lower temperature is a human error, often more harmful than the fever itself (like hypertension meds causing complications). See Hypertension section. Best not to keep thermometers at home. Without one, how to diagnose?
Understand TCM Meridian theory. Different times, different meridian functions. Some fevers occur only at specific morning/afternoon times due to meridian activity/disease.
Compare same location, meridian, and time to measure correct temperature differences.
Between individuals, with same meridian/time: Use your palm against child's palm, back of hand against back of hand (not cross-wise, as meridians/Qi differ). Touch your Yintang point (forehead) to child's forehead. Normally, adult/child temp differs by 0.5-1 degree but feels normal. Difference implies fever.
Handling fever: First understand cause. Most are colds; sweating cures them.
For feverish patients without thirst: Drink hot rice gruel with ginger/sugar or hot milk. Rub pure camphor oil or Baoxin An Oil on Lung and Bladder meridians. Cover with thick quilt to induce sweating. When sweat reaches forehead, wipe with warm wet towel. Fever resolves. No hospital needed. Everyone can solve cold fever, avoiding Hepatitis B, Leukemia, Uremia caused by antibiotics. See relevant sections.
2. Should One Drink More Boiled Water During Fever?
In winter, we see breath vapor; we absorb moisture from air. If body moisture is sufficient, no thirst. Colds or normal non-thirsty states don't need extra water.
In AC rooms, machines absorb moisture, air dries, thirst arises. Drink more water.
After exercise, excessive moisture loss causes thirst. Natural reaction.
During fever: Thirsty? Drink. Not thirsty? Body doesn't need it. Don't force water. Better to drink hot rice gruel with ginger/rock sugar or hot milk to boost strength and induce sweating. Far better than IVs or forced water drinking.
If patient wants cold water: Internal Heat/Fire/Inflammation. Drink water to cool. Do NOT cover with quilt; worsens condition.
If patient wants hot soup: Internal Cold/Deficiency. Do NOT use anti-inflammatories/fire-clearing. Use warming/dispersing method. Treat like non-thirsty case.
Understand cause. Treatment divides into Cold, Heat, Deficiency, Excess. Treating Heat-type fever (Wind, Summerheat, Damp-Heat) as Cold Damage fever causes convulsions/spasms/"brain burning."
Understanding TCM herbs: Herbal antipyretics are not slower than Western medicine and never "burn the brain." Why is TCM theory invisible in media/schools?
(IV) Correction of Fever Treatment Concepts
Treating fever is easy: Find cause. For cold fever, sweating resolves it.
Public thinks Western medicine is faster. Actually, Western medicine doesn't truly treat fever; it refers cases it can't handle.
Infant fevers are mostly colds; sweating cures them. Infant fever drops after injection because crying/sweating from needle pain reduces fever, not drug effect. People suffer antibiotic harm unknowingly. See "Western Meds Harm Stomach/Organs." Test this: Sterilize a needle, prick arm/buttock of feverish child until crying/sweating. Wipe sweat with warm towel. Fever resolves. No drugs needed. People mistake this for Western speed. Referrals happen when sweating fails.
Drug harm or incorrect antipyresis causes complications, mistaken for "brain burning."
Polio was common before; now rare, but knee/foot pain is common. Early antibiotics caused encephalitis/meningitis/polio. Improved antibiotics prevent these but cause Hepatitis B/Leukemia/Uremia. Unexplained joint pain tests positive but lacks cause. Is this medical progress or increased harm? Reports ignore medical harm progress. See Hepatitis/Leukemia sections.
TCM uses Shaoshang or Shixuan bloodletting for fever. Lung governs skin/hair. Crying and bloodletting stimulate Heart/Lung, strengthen circulation, open pores, expel heat, resolve fever.
During fever, heart beats fast, BP rises. This strengthens circulation to induce sweating. Sweat resolves fever; heart rate normalizes.
Anti-inflammatories reduce Fire. Inappropriate use harms Heart. Heart damage allows Exterior Wind-Cold to invade Heart/Kidney. Body cannot expel Exterior pathogen. Ice pillows close sweat pores; IVs draw Wind-Cold inward. This causes encephalitis/meningitis from mistreating Cold/Wind (incorrect purging/sweating).
(V) Are IV Fluids Nutritional or Deadly?
Ten bottles of IV fluid are less nutritious than a bowl of hot rice gruel with ginger/rock sugar.
Many view large IV bags as nutritional, believing they relieve fatigue/replenish strength. They look impressive and are seen as panaceas for fever.
But IVs are the culprits for future diseases, Leukemia, Uremia in cold/fever patients. Why?
Consider grain piles covered with straw. Grain heats up underneath. Do you remove straw to dissipate heat, or pour water to cool it? IVs act like pouring water. Temperature drops, but what are the consequences?
