Baizhu (Atractylodis Macrocephalae Rhizoma), Qingmuxiang (Linderae Radix), Wuxijiao (Rhinocerotis Cornu, now prohibited/replaced by Tapir or Buffalo horn in practice, here translated as Rhinoceros Horn for historical accuracy), Xiangfu (Cyperi Rhizoma), Dingxiang (Caryophylli Flos), Zhusha (Cinnabaris), Helile (Terminaliae Chebulae Fructus, roasted), Baitanxiang (Santalum Album Lignum), Anshixiang (Styrax Resina, also known as Jinyinxiang; separately ground into powder and boiled into a paste with non-fermented wine), Shexiang (Moschus, ground), Bibo (Piperis Longi Fructus), Longnao (Borneolum Syntheticum, i.e., Ice Piece/Borneol), Suhe Xiangyou (Styrax Oil), Chenxiang (Aquilariae Lignum Resinatum) (each 2 liang), Xunluxiang (Olibanum, i.e., Ruxiang, 1 liang).
Grind the above ingredients into fine powder. Form pills using refined honey, sized like Euryale seed (Qianshi) or longan seed. Wrap externally with wax. Open temporarily when needed for use. Administer with ginger decoction, warm water, or wine, at one's discretion.
Wang Hu notes: The above prescription is an agent of acrid-warm nature that moves and penetrates. Most of its herbs enter the Yangming Qi level. Among the ingredients, only Xijiao (Rhinoceros Horn) is cool in nature and additionally enters the Yangming Blood level. I believe that for patients suffering from sudden collapse due to Cold invasion who are unable to speak, it is appropriate to use this formula temporarily to open the stagnation of Qi in the chest and diaphragm. Once the patient regains consciousness, other medications should be employed.
Wu Renzhai stated: "Zhonghan" (Cold Invasion) refers to Cold pathogens directly invading the Three Yin meridians. Zhonghan is more severe than Shanghan (Cold Damage). If not treated urgently, death may occur within a day or night. If Cold invades the Taiyin meridian, pain occurs in the Zhongwan region; Lizhong Tang (Regulate the Middle Decoction) or Huoxiang Zhengqi San (Agastache Powder to Rectify the Qi) combined with Lizhong Tang is indicated. If the Cold is severe, with a deep and fine pulse and cold feet, Fuzi (Aconiti Lateralis Radix Praeparata) must be added. If Cold invades the Shaoyin meridian, pain occurs around the umbilicus and abdomen; Wuji San (Five Accumulations Powder) combined with Wuzhuyu (Evodiae Fructus) is indicated. If the Cold is severe, with a deep pulse and cold feet, Sini Tang (Reverse the Collapse Decoction) combined with Wuzhuyu is indicated. If Cold invades the Jueyin meridian, pain extends from the lower abdomen to the genitalia; Danggui Sini Tang (Angelica Decoction to Nourish Blood and Unfreeze the Collaterals) combined with Wuzhuyu is indicated. In severe cases, Fuzi must be doubled. In cases of extreme cold, cyanotic lips, limb reversal (JueNi), absent pulse, and retracted testicles, the Congyi method (scallion moxibustion/ironing) or frying Wuzhuyu for ironing should still be used, along with moxibustion on Shenque (CV-8, Umbilicus), Qihai (CV-6), and Guanyuan (CV-4), 20-30 moxa cones, which is optimal. Recovery is possible when the pulse gradually returns and the limbs become warm. If no medicine is available at the moment, urgently soak the four wrists of the hands and feet in cool water. Observe the cyanotic or purple veins and puncture them with a three-edged needle to let blood out; this can also cure the condition. Alternatively, pricking the tips of the ten fingers to bleed is also good. Or, administering a decoction of single-ingredient Wuzhuyu can also serve as emergency rescue.
Wang Hu notes: Regarding the above discussion stating that for Cold invading Taiyin, Huoxiang Zhengqi San combined with Lizhong Tang is indicated: Since Cold invading Taiyin involves the Spleen and Stomach as interior-exterior pairs, if there is Cold food stagnation in the Stomach, Huoxiang Zhengqi San is suitable for combination. It also states that for Cold invading Shaoyin, Wuji San combined with Wuzhuyu is indicated. The medication in this formula is miscellaneous and irrelevant to the Shaoyin meridian, lacking a primary targeted ingredient. As for using cool water to soak the four wrists and puncturing with a three-edged needle to bleed, this is only suitable for Yang syndrome Heat JueNi with hidden pulse and retracted testicles. Applying this method to Yin syndrome JueNi with absent pulse and retracted testicles depletes the deficiency further. How extremely absurd and erroneous!
