Urinary retention may be caused by Qi deficiency, Blood deficiency, Phlegm obstruction, Wind closure, or Excess Heat. If due to Qi deficiency, use Ren Shen (Ginseng Radix), [character missing in original text, likely Huang Qi or Bai Zhu], Sheng Ma (Cimicifugae Rhizoma), etc., administer first and then induce vomiting, or induce vomiting by probing with Ren Shen and [same as above] decoctions; if due to Blood deficiency, use Si Wu Tang (Four Substances Decoction), administer first and then induce vomiting, or inducing vomiting within Xiong Gui Tang (Ligusticum and Angelica Decoction) is also acceptable; if there is much Phlegm, use Er Chen Tang (Two Aged Decoction), administer first and then induce vomiting. All the above cases employ the method of induced vomiting. If it is blockage due to Phlegm and Qi stagnation, use Er Chen Tang added with Mu Tong (Akebiae Caulis, one version says Mu Xiang Aucklandiae Radix) and Xiang Fu (Cyperi Rhizoma) to induce vomiting, thereby lifting the Qi. When Qi ascends, water naturally descends, because Qi carries water. For those with Excess Heat, purgation/drainage is indicated; use Sha Tang (Sugar Water) to mix with Qian Niu Zi (Pharbitidis Semen) powder, two or three fen, or Zhi Zi (Gardeniae Fructus) and similar herbs. For Heat, Dampness, and Qi stagnation in the Lower Jiao, clearing heat, drying dampness, and ascending are appropriate. Pregnant women often suffer from urinary retention because the uterus is pressed downward by the fetus. In "Zhuan Bao Lun" (Treatise on Displaced Bladder), Si Wu Tang is used with added Ren Shen, Bai Zhu, Ban Xia, Chen Pi, Gan Cao, decocted with Sheng Jiang (Zingiberis Rhizoma Recens) and Da Zao (Jujubae Fructus), taken on an empty stomach. One woman, after suffering from Spleen pain, developed urinary and fecal retention; this was due to Phlegm obstructing the Middle Jiao and Qi stagnating in the Lower Jiao. She was treated with Er Chen Tang added with Mu Tong. After the initial dose, [text seems incomplete here, implying subsequent treatment or reaction].
[Appendix] The Kidney governs water metabolism, and the Bladder is its fu organ. Water accumulates in the Bladder and is excreted via the Small Intestine; they are essentially connected. However, why does the Small Intestine correspond uniquely to the Heart? It is because Yin cannot exist without Yang, and water cannot exist without Fire. When Water and Fire interact harmoniously, Upper and Lower Jiao communicate, which is why Ying and Wei Qi circulate freely, and the opening and closing of the "Water Dou" (water passages) remain regulated. Only when Heart and Kidney fail to interact, and Yin and Yang are imbalanced, do internal and external obstructions occur, leading to sluggish water passages. When transmission is excessive, water passages become slippery; Heat causes obstruction, while Cold causes incontinence. In cases of severe Heat, urine is completely blocked; in mild Heat, urination is difficult and scanty. When both Kidney and Bladder are deficient, pathogenic Heat invades, so they cannot control water. Water carrying Heat flows sluggishly, resulting in frequent urination with dribbling; when both Kidney and Bladder are cold, internal Qi is insufficient, causing the bladder to slip spontaneously, resulting in copious, white urine, which increases further at night when Yin is dominant. For sluggish or slippery urination, one must regulate the Qi.
