The parturient had a constitutionally weak physique and was exhausted during labor, resulting in insufficiency of Middle Qi (vital energy of the spleen and stomach) and stagnation in the chest and diaphragm. Although the Stomach accepts food, digestion and transportation are difficult. Physicians mistakenly diagnosed it as Food Retention (accumulation of undigested food) and arbitrarily used digestive-promoting agents; or due to Qi Depression (stagnation of qi flow), they exclusively used dispersing and regulating methods; or because of constipation, they rashly discussed purgative treatments. These errors are the origin of Postpartum Distension (abdominal bloating). The treatment method should primarily focus on greatly tonifying Qi and Blood, known as "treating obstruction with tonification" (using astringent or tonifying methods to treat blockage syndromes), which yields very rapid effects. First, administer Single Ginseng Decoction (Dushen Tang) mixed with scorched rice crust powder to unblock Stomach Qi. Subsequently, administer Yangsheng Huazhi Tang, so that Spleen transportation is restored and distension dissipates.
Chuanxiong Rhizome (2 qian), Danggui Radix (3 qian), Renshen Radix et Rhizoma (1 qian), Atractylodis Macrocephalae Rhizoma (raw, 2 qian), Citri Reticulatae Pericarpium (8 fen), Cyperi Rhizoma (processed, 5 fen), Poria (2 qian), Glycyrrhizae Radix et Rhizoma (honey-fried, 3 fen), Arecae Pericarpium (4 qian), Persicae Semen (skin and tip removed, ground, 11 grains).
Take the above herbs, add one and a half cups of water and one small cup of yellow wine, decoct until seven-tenths remains, and take warm.
If constipation exists, add Cistanche Stem 2 qian. If Rhubarb was mistakenly taken, add Astragali Radix 4 qian and double the dosage of Renshen. If distension is severe, Renshen can be increased to 4-5 qian. Mr. Danxi treated postpartum distension where patients consumed over half a jin of Renshen before bowel movements became smooth and swelling subsided.
Postpartum diarrhea is entirely attributed to Spleen Deficiency, though there are distinctions between those caused by Cold or Diet. Hot diarrhea is rare. The treatment differs from various diarrheas in miscellaneous diseases. Indeed, since Lochia (postpartum residual blood and fluid) has not yet cleared, sudden tonification is difficult; since Yuan Qi (original vital energy) has suddenly depleted, easy elimination/digestion is inappropriate. Excessive warmth may cause new blood to hemorrhage; sudden cold may lead to blood coagulation and complications. Generally, for this condition postpartum, one should first take Shenghua Decoction with added Poria 1.5 qian. After two or three doses, once the mass-pain (postpartum abdominal pain) is eliminated, one can proceed to tonify the Spleen, digest food, and warm the middle, treating according to symptoms. Diarrhea occurring immediately after delivery: take Heqi Shenghua Tang. Chronic diarrhea before pregnancy continuing after delivery: take Shenling Shenghua Tang, a temporary measure to rescue critical conditions. Once mass-pain is eliminated: take Jiawei Shenghua Tang. For cases due to Cold or Diet, treat separately. Only if confirmed as excess-heat diarrhea, remove Black Ginger (carbonized Zingiberis Rhizoma) and Myristica Seed, and absolutely do not add cooling/cold-natured drugs to avoid causing Blood-Cold, leaving hidden troubles. One must be extremely cautious.
