Dysentery accompanied by an inability to eat (Jinkou Li, dysentery with loss of appetite), although attributed to Spleen deficiency, is actually caused by heat qi blocking the chest and heart. The common use of Muxiang (Aucklandiae Radix) errs in being too warming, while the use of Shanyao (Dioscoreae Rhizoma) errs in being too astringent and obstructing. Only authentic Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) added with powder of Shichangpu (Acori Tatarinowii Rhizoma), mixed with hot water made from high-quality Jingmi (Oryza Sativa Germen), or decocted with Renshen (Ginseng Radix et Rhizoma), Fuling (Poria), and Shilianzi Rou (Nelumbinis Semen Endospermium) is appropriate.
Changshan (Dichroae Radix) treats malaria, and Yingkesu Ke (Papaveris Pericarpium) treats dysentery; people generally despise these herbs, which is indeed understandable. However, if dysentery has persisted for a long time without abdominal pain, intestinal astringency is required; how can one refrain from using astringents? Malaria arises from phlegm and fluid retention, so the treatment method should involve inducing vomiting to expel phlegm and draining fluids; how can one refrain from such treatments? At such times, without formulas containing Yingkesu Ke and Changshan, what could be used for specific treatment? It is merely that other herbs are needed as auxiliaries within the formula.
All cases of malaria are due to the accumulation of yellowish fluid in the abdomen. Because this fluid does not circulate, chills and fever alternate incessantly; this is the root cause of malaria. Treatment methods: For pure-heat Summerheat Malaria, Xiangru San (Elsholtzia Decoction) is combined with Qingpi (Citri Reticulatae Pericarpium Viride), Beida Huang (Rhei Radix et Rhizoma), and two Wumei (Mume Fructus), decocted together and taken warm early in the morning. For Cold Malaria characterized by excess cold, Erchen Tang (Two-Cured Decoction) is combined with Qingpi and Liangjiang (Alpiniae Officinarum Rhizoma), with increased amounts of Shengjiang (Zingiberis Rhizoma Recens) for decoction, and five Shenbao Wan (Divine Preservation Pills) are swallowed early in the morning. The aim is to drain out toxic fluids to remove the disease root, after which chills and fever will naturally resolve.
A middle-aged person, whose internal organs had been deficient for a long time and who suffered from frequent loose stools, suddenly contracted malaria with severe vomiting. Only the exclusive use of Renshen could stop the vomiting; other antimalarial formulas were unsuitable. Therefore, Fuling Erchen Tang was modified with Renshen, Suosha Ren (Amomi Fructus Rotundus), and a doubled dose of Baidoukou (Cardamomi Fructus). After one or two doses, the patient felt significant relief in Qi and pulse, and the chills and fever ceased. This is because Baidoukou can digest food and promote circulation through the Triple Burner; once the Ying and Wei Qi circulate properly, chills and fever naturally subside. In future cases of malaria accompanied by vomiting, or in individuals prone to vomiting who develop malaria, Changshan must strictly be avoided. Instead, natural juice extracted from fresh Luobo (Raphani Fructus) and Shengjiang, half a cup each, should be decocted with three or four spoons of honey and two Wumei. Three or four Ju Fang Xionghuang Wan (Bureau Formula Realgar Pills) are swallowed. Once foul blood and phlegm-fluids are discharged, the condition should be regulated with Renshen, Chuanxiong (Ligustici Chuanxiong Rhizoma), Fuling, Banxia (Pinelliae Rhizoma), Suosha Ren, and Gancao (Glycyrrhizae Radix et Rhizoma). If vomiting persists and fever remains unrelieved, Zhenliao Xiao Chai Hu Tang (Authentic Minor Bupleurum Decoction) with increased Shengjiang should be used. If vomiting is severe accompanied by fever, first administer Zhi Mie Sheng Shu Yin (Antimalarial Raw and Cooked Drink); once vomiting subsides, follow up with Xiao Chai Hu Tang.
