A member of the Li family, a prominent clan in Donghao, Jurong County, contracted Shanghan (Cold Damage) for several days. As there were no physicians in the village, he had been relying on shamans and wizards for five or six days. Various treatments were applied indiscriminately, including sweating and purging methods, resulting in a complicated disease state (Huai Bing). At that time, I was fleeing bandits from north of the Yangtze River and taking refuge in the Jiangzuo region. I stayed at his house. In the middle of the night, I heard groaning. Upon inquiry, I learned that he had suffered from Shanghan for over ten days. I examined him and found that both his upper and lower lips were eroded and ulcerated; his voice was hoarse, and his throat was dry. His tongue had a white coating, and his teeth appeared colorless and dull. I said, "This disease is named Huhu (Fox-Infestation), which can be rapidly fatal." I then prepared Xionghuang Wan (Realgar Pill) and Xiexin Tang (Drain the Heart Decoction) by candlelight and administered them. After a few days, he recovered.
In May, while seeking refuge in Lisha, east of Weiyang (modern Yangzhou), a young man from a prominent family had suffered from Shanghan for eight or nine days. His body exhibited a deep yellow discoloration; his nose and eyes were painful; his shoulders, neck, head, and waist were stiff and rigid; his stool was dry and difficult to pass, and his urine was as yellow as gold. I diagnosed: "The pulse is tight and rapid. The spleen has first been affected by dampness, and summerheat and heat have accumulated in the Foot-Taiyin Spleen Meridian. Stagnant food residues contend with this pathogenic factor, causing stagnation and steaming without discharge. This results in sweat on the head and face but none below the neck. If the breath from the nose feels cold, and the pulse at Cun Kou (Radial Artery) near the palm is imperceptible, death will ensue. Now, the pulse corresponds with the symptoms. I will administer Yinchen Tang (Artemisiae Scopariae Decoction) combined with Wuling San (Five-Ingredient Powder with Poria)." After a few days, he recovered.
A patient presented with body pain, yellowish complexion, wheezing, chest fullness, and headache, yet could eat normally, and bowel and bladder functions were regular. Some practitioners often prescribed Yinchen Wuling San (Artemisiae Scopariae and Five-Ingredient Powder with Poria Combination). I examined his pulse and found it large and deficient; he also had nasal congestion and irritability. Given these symptoms, this was not due to damp-heat contending with stagnant food, but rather cold-dampness affecting the head. Zhang Zhongjing stated: "Body pain, fever, yellowish complexion with wheezing, headache, nasal congestion, and irritability, with a large pulse, normal appetite, and a harmonious abdomen free of disease, indicates cold-dampness in the head, hence nasal congestion. Administering medication into the nose will lead to recovery." However, Zhang Zhongjing did not provide a specific formula. The formula is recorded in *Waitai Jianyuan* (Prescriptions from the Outer Cabinet) under the section on eliminating redundancy for infectious cases: It treats epidemic heat-toxin penetrating the zang-fu organs and sinking into the bone marrow, potentially causing jaundice. Use Goudi San (Melon Pedicle Powder): 14 Goudi (Thunberg Fruit), 14 Chi Xiaodou (Adzuki Bean), and 14 Shumi (Millet), ground into powder. Take an amount the size of a soybean, insufflate into the nose. Constrict the nose so that yellow fluid drains out. Do not blow the powder deeply into the nasal cavity.
