Toucheing clothes and bed (involuntary grasping of clothes and bed) has two etiologies: one is heat damaging Yin due to fire therapy in Taiyang disease; the other is extreme heat deepening in Yangming disease. If urination is free, prognosis is good; if difficult, prognosis is poor. These result from improper use of sweating, vomiting, and purging methods leading to deteriorated conditions (Huai Zheng). If the pulse is excess, purging may be used; if weak, purging is contraindicated. When deficiency/excess and Yin/Yang are hard to distinguish, Dushen Tang or Liuwei Dihuang Wan can restore health.
[Note] Toucheing clothes and bed is a critical and malignant syndrome. One cause is using fire therapy to induce sweating in Taiyang disease, causing excessive Yang to damage Yin. If Yin fluid is not exhausted, urination is free, and survival is likely; if Yin fluid is exhausted, urination is difficult, and death is likely. Another cause is extreme heat in Yangming disease, resulting from improper application of sweating, vomiting, and purging methods, leading to Huai Zheng. The deeper the heat, the more severe the condition. If the pulse is excess, purging is feasible and treatable; if weak, purging is intolerable and difficult to treat. This has become a critical Huai Zheng, where doctors often cannot distinguish between Yin/Yang and deficiency/excess, leaving no clear treatment path. Large doses of Dushen Tang, Liuwei Dihuang Tang, or Gan Sheng Di Huang Tang should be administered frequently, often resulting in recovery.
Taiyang receiving Yang pathogenic factors with retained water manifests as Xiao Ke (excessive thirst), fever, sweating, irritability, dysuria, and regurgitation upon drinking, with a floating and rapid pulse. Wuling San is indicated.
[Note] Taiyang receiving Yang pathogenic factors involves Shui Ni (water regurgitation) and Xiao Ke. In Taiyang Wind Strike, if there is excessive thirst for water but vomiting immediately after drinking, it is called Shui Ni; if water intake is high but urine output is low, it is called Xiao Ke. Fever and sweating indicate the release of Wind pathogen. Irrability indicates Heat. Dysuria and vomiting upon drinking indicate retained water. Floating and rapid pulses indicate Wind-Heat. Wuling San is suitable for all cases. Drinking warm water in abundance to induce slight sweating resolves the Taiyang exterior and promotes diuresis of retained water, leading to cure. If the patient cannot tolerate warm water and desires cold water, it indicates intense Heat; add Han Shui Shi, Shi Gao, and Hua Shi to Wuling San. See Taiyang Upper Volume.
Taiyang receiving Yin pathogenic factors with Water Qi manifests as Shang Han (cold damage) with no sweating and feeling hot, primarily presenting with dry retching, cough, and mild wheezing. Xiao Qing Long Tang is indicated to disperse exterior Cold and resolve interior Cold-Fluid. If accompanied by dysuria and hypogastric fullness, or diarrhea, remove Mahuang and add Fu Ling. For dysphagia, replace Mahuang with Fu Zi; for worsening wheezing, add Xing Ren; for thirst, add Hua Fen and reduce Banxia.
[Note] Taiyang receiving Yin pathogenic factors with Water Qi refers to unresolved Taiyang Shang Han exterior syndrome with fever and anhidrosis, accompanied by dry retching, cough, and mild wheezing, which are main symptoms of Fluid retention diseases. Xiao Qing Long Tang is appropriate to release exterior Cold and scatter interior Cold-Fluid, leading to cure. Alternatively, if accompanied by dysuria and hypogastric fullness, or diarrhea, or dysphagia, or wheezing, or thirst, these are possible accompanying symptoms of Fluid diseases, also treated primarily with Xiao Qing Long Tang. Dysuria and hypogastric fullness indicate Water stagnating in the Lower Jiao; diarrhea indicates Water moving into the Large Intestine. In these cases, remove Mahuang and add Fu Ling specifically for promoting diuresis. Dysphagia indicates severe interior Cold; replace Mahuang with Fu Zi to dissipate interior Cold. Wheezing with rebellious Qi requires adding Xing Ren to descend rebellious Qi. Thirst requires adding Hua Fen and reducing Banxia to avoid drying properties and generate fluids. See Taiyang Lower Volume.
