For severe heat with panting, cough, and abdominal fullness, use Huanglian Jiedu Tang (Coptis Decoction to Relieve Toxicity) [Formula 21]; or if the heat is extreme causing cough, chest oppression, and mental disturbance, use Sanyi Chengqi Tang (Three-One Purging Decoction) [Formula 13]. For Shaoyang (Lesser Yang) cough, use Xiaochaihu Tang (Minor Bupleurum Decoction) [Formula 9], removing Renshen (Ginseng), Dazao (Jujube), and Shengjiang (Fresh Ginger), and adding Wuweizi (Schisandra Berry) and Ganjiang (Dried Ginger). For panting, cough accompanied by headache, use Fuling Banxia Tang (Poria and Pinellia Decoction) [Formula 38]. For cough, use Baihu Tang (White Tiger Decoction) [Formula 22] added with Banxia (Pinellia Tuber) and Jiegeng (Platycodon Root). For persistent cough, use Renshen Shigao Tang (Ginseng and Gypsum Decoction) [Formula 35], or Cui Xuanwu Renshen Shigao Tang [Formula 36]. For cough caused by phlegm rebellion, use Banxia Chupi Tang (Pinellia and Tangerine Peel Decoction) [Formula 37]. For qi obstruction and cough after sweating, use Wuweizi Tang (Schisandra Berry Decoction) [Formula 44].
For those with dry mouth, mental restlessness, and thirst; for those with thirst after sweating in Shanghan (Cold Damage); and for "Water Rebellion" (Shui Ni) where drinking water leads to vomiting, all are suitable for Wuling San (Five-Ingredient Powder with Poria) [Formula 24]. In Shaoyin (Lesser Yin) disease on the second or third day, with dry mouth and throat, use Sanyi Chengqi Tang (Three-One Purging Decoction) [Formula 13]. Or if internal heat and dryness are severe, with Shanghan pathogenic factors stagnant and retained fluids not dispersing, leading to unceasing mental restlessness and thirst, followed by a protruding abdomen with unbearable pain, only vomiting cold saliva, extreme thirst unable to be quenched even by drinking, panting, chest oppression, mental disturbance, and a sensation of impending death, use Sanyi Chengqi Tang (Three-One Purging Decoction). Upon swallowing, the thirst stops immediately, the patient appears as if healthy, and within a short time, profuse sweating occurs leading to recovery. At this stage, there is often diarrhea accompanied by sweating; alternatively, the medicinal power may only disperse through sweating, resulting in recovery without diarrhea. Or after sweating, emesis, or purgation, if there is mental restlessness, thirst, dry mouth, and a floating and large pulse, use Baihu Tang (White Tiger Decoction) [Formula 22] added with Renshen (Ginseng); for mental restlessness and excessive thirst, use Liangge San (Cooling the Diaphragm Powder) [Formula 23] added with Huashi (Talcum).
For Shanghan (Cold Damage) with mental restlessness and excessive thirst, use Liangge San (Cooling the Diaphragm Powder) [Formula 23] added with Huashi (Talcum). For heatstroke or Shanghan (Cold Damage) with mental restlessness after sweating, use Wuling San (Five-Ingredient Powder with Poria) [Formula 24]. For all cases of fire-heat causing mania, restlessness, panting, and fullness, use Huanglian Jiedu Tang (Coptis Decoction to Relieve Toxicity) [Formula 21]. For mental restlessness with thirst, use Baizhu San (Atractylodes Macrocephala Powder) [Formula 42], or add Huashi (Talcum); for severe cases, add Jiangzhi (Ginger Juice).
For inner annoyance (Ao Nao) and mental restlessness, tossing and turning unable to sleep, this is due to heat-fire stagnating internally preventing the free flow of qi. If accompanied by chest fullness and binding pain, or mental restlessness, slight sweating, or deficiency-related restlessness, Zhizi Tang (Gardenia Decoction) [Formula 28] is the primary treatment; if qi is deficient, add Gancao (Licorice Root) one qian; if vomiting occurs, or if initially mistakenly treated with pill-based purgatives, add Shengjiang (Fresh Ginger) half liang. For all cases of inner annoyance and deficiency-related restlessness, Liangge San (Cooling the Diaphragm Powder) [Formula 23] is very effective, and it is also appropriate to wash and soak the hands and feet in decoction to disperse the binding heat in the heart and chest. For mental restlessness with abdominal fullness, inability to sit or lie comfortably, Zhizi Houpo Tang (Gardenia and Magnolia Bark Decoction) [Formula 30] is the primary treatment. Or after purgation in Yangming (Greater Yang) disease, with irritability, heat, and inner annoyance, use Sanyi Chengqi Tang (Three-One Purging Decoction) [Formula 13]. For mental restlessness and insomnia after sweating, use Wuling San (Five-Ingredient Powder with Poria) [Formula 24] or Liangge San (Cooling the Diaphragm Powder) [Formula 23]; for dry mouth, groaning, delirium, and insomnia, use Wuling San (Five-Ingredient Powder with Poria) and Huanglian Jiedu Tang (Coptis Decoction to Relieve Toxicity) [Formula 21]; for mental restlessness, use Liangge San (Cooling the Diaphragm Powder). For Shaoyang (Lesser Yang) syndrome, with vexation in the chest but no vomiting, use Xiaochaihu Tang (Minor Bupleurum Decoction) [Formula 9], removing Banxia (Pinellia Tuber) and Renshen (Ginseng), and adding Gualou Shi (Trichosanthes Fruit) as the primary treatment.
