Remove Cinnamon Twig (Guizhi) from the Guizhi Tang (Cinnamon Twig Decoction) formula. Add Poria (Fuling) and Atractylodes Macrocephala Rhizome (Baizhu) (each three liang). Prepare and administer the remaining ingredients according to the previous method. When urination becomes smooth, the patient will recover.
Hu notes: The text following the above decoction states, "When urination becomes smooth, the patient will recover." From this, it is known that this is undoubtedly a syndrome of retained fluid retention (Ting Yin).
In Shaoyin Cold Damage (Shanghan), the pulse is floating, there is spontaneous sweating, frequent urination, vexation in the heart, slight aversion to cold, and cramping/tightness in the legs. Instead, Guizhi Tang (Cinnamon Twig Decoction) was administered to attack the exterior; this is an error. This leads to cold extremities (Jue), dryness in the throat, restlessness, and vomiting/reversal of qi. At this point, prepare Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction) to administer to the patient to restore their Yang Qi. If the cold extremities resolve and the feet become warm, then prepare Shaoyao Gancao Tang (Peony and Licorice Decoction) to administer to the patient, and the legs will extend. If there is disharmony of Stomach Qi leading to delirious speech (Chanyu), administer a small amount of Tiaowei Chengqi Tang (Harmonizing the Stomach and Directing Downward Decoction). If excessive sweating is induced again and moxibustion with heated needles (Shaozhen) is added, Sini Tang (Four Reversal Decoction) should be the primary treatment. (This clause pertains to a True Cold syndrome. Misusing Guizhi Tang (Cinnamon Twig Decoction) to attack the exterior damages Yang Qi, resulting in deficiency of both Yin and Yang, as well as Qi and Blood. Therefore, Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction) is prepared to restore Yang Qi, and subsequently, Shaoyao Gancao Tang (Peony and Licorice Decoction) is prepared to nourish Yin and Blood. Administering a small amount of Tiaowei Chengqi Tang (Harmonizing the Stomach and Directing Downward Decoction) constitutes the contrary treatment method. Using Sini Tang (Four Reversal Decoction) as the primary treatment constitutes the direct treatment method. The main text and annotations are found in *Zhonghan Lun*.)

Question: The symptoms resemble Yangdan Syndrome. After treating according to standard methods, the condition worsened, presenting with cold extremities, dryness in the throat, cramping/tightness in both calves, and delirious speech. The Master said, "By midnight, the hands and feet should become warm, and the feet should extend." Later, this indeed happened as the Master predicted. How did you know this?
Answer: The Cun Kou (inch position) pulse is floating and large. Floating indicates Wind, and large indicates Deficiency. Wind generates slight heat, and Deficiency causes cramping/tightness in both calves. The clinical presentation resembles Guizhi Tang (Cinnamon Twig Decoction) syndrome, so Fuzi (Aconite Lateral Root) was added to the formula, and the dosage of Guizhi (Cinnamon Twig) was increased to induce sweating. Fuzi (Aconite Lateral Root) warms the meridians because of Yang Collapse (Wang Yang). The presentation of cold extremities, dryness in the throat, restlessness, internal binding in the Yangming channel, delirious speech, and confusion requires administering Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction). By midnight, Yang Qi returns, the two feet should become warm, and although the calves may still be slightly cramped, administering Shaoyao Gancao Tang (Peony and Licorice Decoction) again allows the calves to extend. Using Chengqi Tang (Directing Downward Decoction) to induce mild loose stools stops the delirious speech. Thus, it is known that the disease can be cured. (This clause also pertains to a True Cold syndrome. Because the symptoms resemble Yangdan Syndrome, Guizhi Tang (Cinnamon Twig Decoction) was mistakenly administered. Yangdan is another name for Guizhi Tang (Cinnamon Twig Decoction). Dryness in the throat, restlessness, delirious speech, and confusion are signs of Extreme Yin resembling Yang. Therefore, formulas containing Fuzi (Aconite Lateral Root) and Ganjiang (Dried Ginger) were used for treatment. The main text and annotations are all found in *Zhonghan Lun*.)
