The methods for setting bones have existed for a long time and vary; this song is composed to comprehensively explain their efficacy. If one can thoroughly understand the underlying principles, exquisite techniques will all be within one's grasp.
Fractures mostly occur in the limbs, manifesting as shortening, lengthening, or dislocation of the bones, or presenting with concavity, convexity, or lateral deviation. One must carefully palpate and massage the affected limb. For elongated bones, reduction by traction is required; for shortened bones, upward traction is needed; for protruding, depressed, or deviated fractures, careful measurement and correction are essential.
If a bone in the body is fractured and separated, it must be properly massaged using three fingers. If the fracture site feels like a pulsating vessel (indicating vascular or nerve stimulation), or if the bone is comminuted, there is little else that can be done (implying poor prognosis or difficulty in management). Before manual reduction, administer Baoming Dan (Preserve Life Pill) orally with wine; once the tendons and bones become relaxed, proceed with manipulation.
Limb fractures must be pulled and rotated to achieve proper alignment. Lower limb fractures with lengthening increase the labor of treatment. The sole of the foot must be firmly supported and padded; the area beneath the fractured bone should be slightly elevated. The heel should be elevated even higher; this ensures no complications after recovery, which serves as empirical verification. If not properly padded while the bone remains long, the deformity will worsen upon healing. This describes the manual technique for shortening bones. Hand fractures with elongation are difficult to treat and may result in disability. Deviations should be lifted from below the affected bone, pulled straight to eliminate angulation, and restored to normal alignment. After reduction, there should be no protrusion, and depressions should be smoothed out without abnormality. Testing requires simultaneous use of both hands; testing the foot requires placing both feet together.
Fujiu Gao (Restore Joint Paste) is urgently needed; spread it on smooth, fine cloth to fit the skin. The dimensions should match the injury site. The paste should be applied thickly and generously dusted with powder. Wrap tightly and secure the dressing. Splints made of Phyllostachys edulis (Bamboo) are preferred due to insufficient strength of other materials. Soak the bamboo until soft, gradually peel off the rough outer layer, and ensure the splint length suits the injury site. Arrange the splints tightly and securely; wrap with cloth without loosening. The wrapping cloth should be about two inches wide, sealed tightly from top to bottom. Tie three bands over the cloth: protect vital areas above and below the center. Bind the center first, then the ends; adjust tightness appropriately for stability. For oblique fractures of the limbs, tighten the middle and leave the ends wider. For transverse fractures resembling cut bamboo, keep the middle loose to allow Qi accumulation and the ends tight. Blood and Qi gather at the fracture site; use bands and hands to restore the palm meridians. Use rice bags to support both sides of the foot; immobilization here is superior to miraculous medicine. Administer symptomatic decoctions and pills daily. Wash with medicated water and change the paste every three days. After twenty-one days, the bone union becomes firm. Sexual activity and consumption of greasy or fishy foods are prohibited during this period. Zijin Dan (Purple Gold Pill) is used as a standard finishing regimen to ensure solid bone healing without adverse sequelae. I have compiled this manual technique song to be passed down to future generations, who must study it carefully.
