The *gu* (bone) refers to the leg bones below the knee and above the ankle, commonly known as the tibia. Its shape consists of two bones; the one in front is called *chenggu* (Cheng Bone), which is thicker; the one at the back is called *fugu* (Auxiliary Bone), which is thinner, commonly known as *laotanggu*. Extending downwards to the high bones protruding on both sides below the malleolus and above the dorsum pedis are the malleoli. The one on the medial side is named *neihuai* (Inner Malleolus), commonly known as *hegu*; the one on the lateral side is named *waihuai* (Outer Malleolus), commonly known as *hegu*. In cases of joint dislocation (*jia chu*), one hand supports the ankle bone while the other hand pulls the heel straight, employing tendon-pulling and bone-setting techniques to restore alignment. Upon hearing a click indicating reduction and restoration of mobility, bind with cloth straps and fix with wooden splints, then administer Shujin Huoxue Tang (Relax Tendons and Invigorate Blood Decoction). After one or two days, untie and inspect. If any misalignment remains, apply manual therapy to massage areas of tendon nodules until straightened, then administer Jianbu Huqian Wan (Strengthen Steps and Tiger-Hidden Pill). Once slightly improved, if one rushes to walk or labor, the ends of the bone may dislocate again. If displaced inward, the inner malleolus protrudes and swells; if displaced outward, the outer malleolus protrudes and swells. Blood stasis accumulates and coagulates, causing weakness in walking, making treatment difficult. One must wait until Qi and blood flow smoothly before ambulation is possible. For toe joint dislocations, treat them using the same method as for fingers. The *fu* (dorsum) refers to the top of the foot, also called *zudie*, commonly known as the foot surface. The bones here are those of the proximal phalanges (basic joints of the toes). Injuries here vary: mild cases involve only muscle and tendon injury and are easy to treat; severe cases involve misaligned bone gaps and are difficult to treat. Gently rub with hands initially to align bones and relax tendons, wash with Baxian Xiaoyao Tang (Eight Immortals Easeful Decoction), apply Wanling Gao (Myriad Spirits Plaster), and internally administer Jianbu Huqian Wan (Strengthen Steps and Tiger-Hidden Pill) and Bujin Wan (Reinforce Tendons Pill).
Generally, falls, blows, or fights cause internal damage to blood. If followed by jumping into water due to despair, external cold penetrates. When blood encounters cold, it coagulates; when cold enters deep into the blood, survival is unlikely. The treatment principle is first to dispel cold, then to drive out blood stasis. To dispel cold, use Magui Wenjing Tang (Cinnamon Twig and Ephedra Warming Meridians Decoction); to drive out stasis, use Zijin Dan (Purple Gold Elixir). If delayed for a long time, Qi stagnation and blood coagulation will lead to contracted tendons and meridians, resulting in curled hands and feet, inevitably becoming incurable. Urgently administer Dashenxiao Huoluo Dan (Great Divine Effect Unblocking Collaterals Elixir) to hope for gradual unblocking, possibly saving the patient. If a pregnant woman suffers an injury, do not recklessly administer trauma medicines, lest they harm the fetal origin, which would be detrimental. Instead, administer Antai Heqi Yin (Fetus-Securing and Qi-Harmonizing Drink), adding slightly agents that remove stasis and generate new tissue, so that Qi and blood harmonize and pain ceases naturally.
