Mr. Chen Wensu said: On the twelfth to thirteenth day of smallpox, the scabs gradually fall off, but the scars remain dark, with some areas depressed and others raised. The muscle tissue is still tender; bathing and washing should be avoided, as well as the consumption of five pungent spices (Wuxin) and fried foods. This is to prevent heat toxins from ascending to熏 (fuming/heating) the liver and diaphragm, which could cause opacities (zhangyi) to form in the eyes. If such a condition occurs, it is treated with Gujingcao San (Valerian Flower Powder). I believe that regarding the aforementioned symptoms: the eyes are the orifices of the Liver. Conditions may arise due to Wind-Heat in the Liver channel, Blood deficiency in the Liver channel, the interplay of Wind-Heat in the Liver channel, internal Heat from Liver Gan (malnutrition syndrome), Heat in the mother's Liver channel (if breastfeeding), or the consumption of rich and greasy foods. Therefore, the eyes are frequently affected. If early treatment is neglected, disability often results.
A child with smallpox had eyes that would not open. My father attributed this to Heat in the Liver channel. We used Xiaodu Huatang (Detoxification Decoction), taken by both mother and child, resulting in recovery.
Another child with eyes that would not open was diagnosed by my father with Heat-Toxin in the Liver channel. Chaihu Maidong San (Bupleurum and Ophiopogon Powder) was used first, followed by Siwu Tang (Four Substances Decoction) added with Zhizi (Gardenia Fruit), resulting in recovery.
A child with smallpox presented with red, swollen, and painful eyes, attributed to Fire in the Liver. Chaihu Maidong San and Gujing San were used, resulting in recovery.
A child whose smallpox was about to crust over had eyes that would not open. The pulse was floating and weak; pressing the Right Guan position revealed a slow and weak quality, indicating Spleen Qi deficiency. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Manjingzi (Vittex Seed) was administered for two doses. Manjingzi was then removed, and several more doses were given, resulting in recovery. Later, whenever he exerted himself physically, his eyes felt distended and could not open or close. He took Buzhong Yiqi Tang in the morning and Wuwei Yigong San (Five-Ingredient Extraordinary Efficacy Powder) in the evening, resulting in recovery.
A child with eye pain and inability to open his eyes was diagnosed with Wind-Heat in the Liver channel. Chaihu Maidong San and Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) added with Chaihu (Bupleurum Root) were each administered for one dose. Upon opening, red opacities were seen spreading across the eye. The previous prescription was continued with the addition of Gujingcao (Valerian Flower), resulting in recovery.
A child with smallpox kept his eyes closed for over twenty days. Medicines clearing Liver Heat and resolving toxicity were used. Although the eyes opened, the eyeballs were damaged, a result of failing to treat early enough.
A child with smallpox in the eyes experienced pain. The Liver pulse was wiry, surging, and forceful. Xiao Chaihu Tang (Minor Bupleurum Decoction) added with Longdancao (Gentian Root) and Sheng Dihuang (Raw Rehmannia Root) was administered for one dose, slightly stopping the pain. Then, Siwu Tang, Chaihu, and Zhizi were administered for one dose, completely stopping the pain. Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Chanju San (Cicada Slough Powder) were then used, resulting in recovery.
A child with smallpox in the eyes presented with a wiry and rapid Liver pulse, indicating an interplay of Wood and Fire. Siwu Tang, Zhizi, Mudanpi (Moutan Cortex), and Chaihu were administered for two doses. Jiawei Xiaoyao San was then administered for two doses, bringing the Liver pulse to a peaceful state. Siwu Tang and Mudanpi were then used until crusting occurred. However, green opacities remained in the eyes. Shed Snake Powder (Shetui San) and Sanwei Gujingcao San (Three-Ingredient Valerian Flower Powder) were used, resulting in recovery.
A child who habitually consumed rich and greasy foods developed smallpox in the eyes with pain, thirst, and firm stools. The Left and Right Guan pulses were surging, rapid, forceful, wiry, and long. This indicated that both the physical signs and the pulse were excess patterns. Chaihu Zhizi San (Bupleurum and Gardenia Powder) and Xiehuang San (Drain the Yellow Powder) were each administered once. Then, Chaihu Zhizi San and Chaihu Maidong San were used, resulting in recovery.
An infant with smallpox had eyes that would not open. The Liver pulse was rapid and forceful upon pressure. Chaihu Zhizi San was administered to both mother and child, and the eyes gradually opened. Later, due to the mother's labor and anger, the eyes closed again. Jiawei Xiaoyao San was used, resulting in recovery. When they closed again, Chaihu Maidong San was used, resulting in recovery.
A child with opacities in the eyes was diagnosed with Gan syndrome involving the Liver and Kidneys. Jiwei Lushui Wan (Nine-Ingredient Aloe Pill), Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill), and Fendan San were used. The opacities gradually receded, and Chaihu Maidong San was then used, resulting in recovery.
