Thirst caused by sores and ulcers should be treated by distinguishing the affected meridians and channels, as well as the deficiency or excess of Qi and Blood. If there is pain and fever but bowel movements and urination are harmonious, the pathogen is in the Fu organs; use Qingre Xiaodu San (Clear Heat and Detoxify Powder). If there is swelling, pain, fever, and constipation, the pathogen is in the Zang organs; first use Xiehuang San (Drain the Spleen Powder), and if ineffective, use Liangge San (Cool the Diaphragm Powder). If local redness, swelling, heat, and pain are intense, the pathogen is in the meridians and channels; use Xianfang Huoming Yin (Immortal Life Decoction from the Immortal House). If the pulse at the right Guan position is surging, rapid, and forceful, it indicates Stomach Fire consuming body fluids; use Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction). If the pulse at the right Guan position is rapid but weak, it indicates Stomach deficiency with shortness of body fluids; use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If improper diet damages Stomach Qi and causes loss of body fluids, use Qian Shi Baizhu San (Master Qian’s Atractylodes Macrocephala Powder). If excessive discharge of pus and blood leads to Qi and Blood deficiency, use Bazhen Tang (Eight Treasures Decoction) with added Wuweizi (Schisandra Berry). If congenital Kidney insufficiency leads to shortness of body fluids, use Jiajian Bawei Wan (Modified Eight-Flavor Pill). Other cases should be managed flexibly according to clinical presentation.
A child suffered from a sore on the right cheek, with thirst for cold drinks, yellow eyes, and cracked lips. This was identified as Excess Heat in the Spleen and Stomach, cured using Xiehuang San (Drain the Spleen Powder). Later, due to food stagnation causing thirst and generalized jaundice, a small amount of Xiehuang San was administered. After the jaundice subsided, the thirst worsened, indicating that the heat had receded but True Qi was deficient. The condition was cured using Baizhu San (Atractylodes Macrocephala Powder).
A woman suffered from an arm carbuncle, with dry mouth desiring hot drinks and frequent urination. This was identified as Qi deficiency of the Spleen and Lung. She was cured using Sijunzi Tang (Four Gentlemen Decoction), Huangqi (Astragalus Root), Ganjiang (Dried Ginger), and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
A child suffered from an abdominal abscess that ulcerated, followed by thirst for hot drinks. This was identified as Qi deficiency of the Spleen and Stomach. Thirst ceased after administration of Liujunzi Tang (Six Gentlemen Decoction), Huangqi (Astragalus Root), and Danggui (Angelica Sinensis). The wound healed after using Yigong San (Extraordinary Power Powder).
A woman suffered from facial sores, with thirst for hot drinks and a red face. This was identified as Spleen Qi deficiency with empty heat. She was first treated with Yigong San (Extraordinary Power Powder) and then cured using Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
A child who frequently consumed rich and greasy foods developed sores in the mouth and tongue, thirst for cold drinks, and constant heat in the hands and feet. This was identified as accumulated heat in the Stomach meridian. Two doses of Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) were given first, followed by two doses of Zhuye Huangqi Tang (Bamboo Leaf and Astragalus Decoction) for gradual improvement. Finally, Baizhu San (Atractylodes Macrocephala Powder) without Muxiang (Vetiver Root) was used for complete recovery. Another child with the same condition was cured using Qingfei San (Clear the Lungs Powder) followed by Liangge San (Cool the Diaphragm Powder).
A child suffered from scrofula (Luo Li, 瘰疬), with a red face and thirst. I diagnosed this as Empty Heat in the Liver and Kidney. Using Jiajian Bawei Wan (Modified Eight-Flavor Pill), Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill), all symptoms resolved within a month. Full recovery was achieved with adjunctive treatment using Jiwei Luhui Wan (Nine-Ingredient Aloe Pill).
A child presented with dry mouth, thirst, aversion to cold, and diarrhea. Various medicines failed, and it was believed the case was incurable. The pulse at the right Guan position was wiry and rapid, deep and hidden upon heavy pressure. Palpation revealed a hidden mass in the abdomen, size of a chicken egg. This was diagnosed as Liver-Spleen Gan accumulation (Gan Ji, 疳积). Chan Chu Wan (Toad Pill) was administered, resolving the mass over three months. Concurrently, Dihuang Wan (Rehmannia Pill) was taken, leading to gradual resolution of symptoms over another three months. Subsequently, Baizhu San (Atractylodes Macrocephala Powder) was used as the principal formula, assisted by Siwei Fei'er Wan (Four-Ingredient Child-Nourishing Pill), resulting in recovery.
A child’s buttock carbuncle was nearly healed when thirst appeared, accompanied by frequent urination, a persistently red face, and a surging, rapid pulse that was weak upon pressure, especially at the Chi position. I diagnosed this as congenital Kidney deficiency inherited from the father. The family disagreed and instead used methods to clear fire and generate fluids. The child subsequently developed vomiting and abdominal pain and died.
A fifteen-year-old boy suffered from facial sores, with heat in both feet and thirst. I diagnosed this as Empty Heat in the Kidney meridian. Due to indiscriminate use of various medicines, he suffered from urinary symptoms resembling strangury and died.
