Qian Zhongyang stated: In the case of convulsive epilepsy (Jing Xian), when a boy looks to the left without sound and to the right with sound; or a girl looks to the right without sound and to the left with sound, this indicates mutual overcoming (Xiang Sheng). This is because the left corresponds to the Liver region, and the right corresponds to the Lung region; it is due to the mutual overcoming between Metal and Wood. If during Yin, Mao, Chen hours (3 AM–9 AM) there is body heat, eyes rolling upward, movement of hands and feet, drooling from the mouth, and stiffness/tension of the neck, this indicates Liver Qi exuberance. If during Si, Wu, Wei hours (9 AM–3 PM) there is body heat and convulsions, mental fright and palpitations, eyes rolling upward, tight teeth with drooling, and twitching of hands and feet, this indicates Heart Qi exuberance. If during Shen, You, Xu hours (3 PM–9 PM) there is body heat with slight convulsions and panting, slightly deviated eyes, sleeping with eyelids open revealing the eyeballs, cold hands and feet, and pale yellow watery stool, this indicates Lung Qi exuberance. If during Hai, Zi, Chou hours (9 PM–3 AM) there are slight convulsions, restlessness while lying down, slight body heat, tightly deviated eyes, phlegm sound in the throat, white-colored stool, drowsiness with drooling, this indicates Kidney deficiency. If the fist is clenched with the thumb inside for girls, or outside for boys, this is considered favorable (Shun). Favorable cases are easy to treat; unfavorable cases are difficult to cure. If saliva enters the Heart and Liver, speech becomes impossible, requiring cooling of the Heart and calming of fright. Convulsions caused by downward rebellion of phlegm are incurable; symptoms arising after vomiting and diarrhea are also incurable. If there are cold sweat on hands and feet, frowning, rubbing the belly, and continuous day and night convulsions, it is named True Convulsion (Zhen Chu). One should use Renshen Tang (Ginseng Decoction), Chuanwu (Aconitum carmichaelii radix preparata), Quanxie (Scorpio), etc., to harmonize Stomach Qi. For convulsions caused by Wind-Cold injury, where breath is hot, yawning occurs, and hands/feet move, it is named False Convulsion (Jia Chu); use Daqing Gao (Major Green Paste) to disperse Wind pathogen. For convulsions after food stagnation, with body heat, drowsiness, vomiting, and refusal of milk, one must first stop the convulsions, then use Bai Wanzi (White Pills) to purge accumulation. For infants within 100 days, true convulsions stem from internal wind generation and are fatal after two or three episodes; false convulsions stem from external wind generation and are not fatal despite frequent occurrences. For external Wind injury, apply Daqing Gao to the fontanelle and use bathing methods.
During Yin, Mao, Chen hours, if convulsions occur with fever, thirst, preference for cold drinks, and constipation, it belongs to deficiency-heat of the Liver and Gallbladder meridians; use Chaishao Shenling San (Bupleurum and Paeonia Bupleurum Ginseng and Atractylodes Powder). If thirst with drinking, spontaneous sweating, and night sweats occur, it belongs to blood deficiency of the Liver and Gallbladder meridians; use Dihuang Wan (Rehmannia Pill). If drooling from the corners of the mouth occurs, it belongs to Liver Wood overacting on Spleen Earth; use Liujunzi Tang (Six Gentlemen Decoction).
During Si, Wu, Wei hours, if convulsions occur accompanied by thirst and drinking, it belongs to struggle between Wind and Fire; use Dihuang Wan to tonify the Liver, Daochi San (Guide Out Redness Powder) and Liangjing Wan (Cool Fright Pill) to treat the Heart. If thirst with drinking warm liquids, fatigue, and refusal of milk occur, it belongs to Earth deficiency causing Wood exuberance; use Dihuang Wan to tonify the Kidney and Liujunzi Tang to tonify the Spleen.
During Shen, You, Xu hours, if slight convulsions and panting occur, eyes slightly deviated, body appearing like deep sleep with exposed eyes, and pale yellow stool, it belongs to deficiency-heat of the Spleen and Lungs; use Yigong San (Extraordinary Assistance Powder). If hands and feet are cold, or panting/diarrhea/refusal of food occur, it belongs to deficiency-cold of the Spleen and Lungs; use Liujunzi Tang with Paojiang (Prepared Ginger) and Muxiang (Aucklandia Root). For chronic illness with Yuanqi (Primal Qi) deficiency, use both Liujunzi Tang and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill).
During Hai, Zi, Chou hours, if slight convulsions and body heat occur, eyes tightly deviated, vomiting/diarrhea/refusal of milk, cold extremities, and excessive sleep, it belongs to Cold Water insulting Earth; use Yihuang San (Yellow Protecting Powder). If ineffective, use Liujunzi Tang with Jiang (Ginger) and Guirou (Cinnamon Bark). For Wind-induced convulsions with non-hot breath and limb fatigue, use Yigong San to tonify Spleen Earth; Gouteng Yin (Uncaria Decoction) to clear Liver Wood. If various symptoms arise due to internal depression of Wind pathogen causing fever, regulate Lung Metal and clear Wind pathogen. If external pathogens are resolved but internal symptoms remain, regulate Lung and tonify Spleen. If Spleen meridian depletion causes fright and convulsions, tonify Spleen and Lung to calm Liver and Heart, then fright and convulsions will cease naturally. For food stagnation causing convulsions and vomiting of milk, use Xiaoshi Wan (Digestive Pills). If food is digested but previous symptoms persist or change, it is Spleen Earth injury allowing Liver Wood to invade; use Liujunzi Tang plus Goutenggou (Uncaria Rhynchophylla Hook) to strengthen Spleen and calm Liver. For convulsions within 100 days, caused by congenital constitution or maternal emotional excess/rich diet, treat the mother concurrently, prioritizing stomach protection; do not directly treat the infant.
Clinical Cases:
An infant had fever, phlegm, and convulsions during Yin/Mao hours, cured by Baolong Wan (Embrace Dragon Pill). Upon recurrence, self-medication with previous pills worsened cough, asthma, and intermittent convulsions, with red/blue-yellow complexion, edema, or drooling. I reasoned: Cough/asthma indicates Spleen/Lung Qi deficiency; intermittent convulsions indicate Wood invading Earth; blue/yellow/red face indicates Liver assisting Heart/Spleen; edema/drooling indicates Spleen Qi deficiency. Used Yizhi Wan (Wisdom Pill) to tonify Heart Spirit; Buzhong Yiqi Tang (Center-Supplementing Qi-Boosting Decoction) to tonify Spleen/Lung, resulting in immediate recovery.
The grandson of Vice Minister Wang Yanghu suffered a femur fracture, reduced by orthopedics. Observing blue-yellow complexion and slight mouth corner twitching, I diagnosed Liver Wood insulting Spleen. Since Qi, Blood, tendons, and bones rely on Spleen Earth for nourishment, strengthening Spleen Qi would heal the injury. Prescribed Liujunzi Tang, Gouteng, Danggui (Angelica Sinensis) for over 30 doses, leading to full recovery.
An infant had blinking eyes and convulsions; Tianma Fangfeng Wan (Gastrodia and Saposhnikovia Pills) exacerbated symptoms during hunger, improving slightly after eating. This indicated Liver Wood controlling Spleen Earth. Prescribed Liujunzi Tang, Shengma (Cimicifuga), Chaihu (Bupleurum), Goutenggou for 2 doses, curing the disease; followed by Buzhong Yiqi Tang for complete effect.
An infant had convulsions, fever, and palpitations during Si/Wu hours, with fatigue, yellow complexion, anorexia, drooling, and desire for warm drinks during attacks. This indicated Heart Fire deficiency failing to generate Spleen Earth. The parent refused treatment, using cooling Heart medicines instead, worsening vomiting/diarrhea and exposing eyes during sleep, nearly developing into Slow Spleen Wind. Treated with Liujunzi Tang, Jiang, Gui (Cinnamon), assisted by Dihuang Wan, resulting in recovery.
A seven-year-old child had persistent fright convulsions and fever, worsening during Si/Wu/Wei hours, with extreme fatigue and desire for warm drinks after fever subsided. This indicated Heart/Spleen Qi deficiency. Morning: Buzhong Yiqi Tang plus Yizhiren (Alpinia Oxyphylla Fruit); Evening: Liujunzi Tang, Danggui, Goutenggou. Recovery ensued. Later, overeating caused vomiting/diarrhea. Treatment with Baohe Wan (Harmonize Stomach Pills) worsened chills/fever and convulsions, indicating re-injury of Spleen Earth and invasion by Liver Wood. Treated with Liujunzi Tang, Chaihu; chills/fever stopped, appetite returned. However, diarrhea persisted during Wu/Wei hours. Treated with Buzhong Yiqi Tang plus Fuling (Poria), Banxia (Pinellia), Goutenggou, resulting in recovery.
An infant within 100 days had convulsions with spontaneous phlegm/saliva. Jingfeng medicines worsened the condition. Yellow complexion with hidden white indicated Spleen deficiency failing to control saliva. Treated with Liujunzi Tang and Buzhong Yiqi Tang, resulting in recovery. Recurrence with asthma led to parent's attempt at dispersing wind/phlegm. I reasoned: Lung deficiency failing to grasp Qi. Treated with Wuwei Yigong San (Five-Ingredient Extraordinary Assistance Powder) plus Goutenggou, Chaihu, regulating Spleen/Lung and clearing Liver fire, leading to peace.
An infant had blue-black or sallow complexion. Inquiry revealed maternal chronic depression/anger. Treated mother with Jiawei Xiaoyao San (Modified Free Wanderer Powder) and Jiawei Guipi Tang (Modified Restore the Spleen Decoction); the child recovered as well.
An infant within one month had convulsions and nasal congestion due to Wind pathogen. Treated with Liujunzi Tang plus Jiegeng (Platycodon), Xixin (Asarum), taken by both mother and child. Additionally, crushed green onion heads and ginger applied warm to the fontanelle via paper, quickly relieving congestion and stopping convulsions.
An infant under one month had convulsions, vomiting milk, abdominal distension, and diarrhea due to milk injuring Spleen/Stomach. Mother took Wuwei Yigong San plus Loulu (Euphorbia lathyris root); infant took small spoonfuls, resulting in recovery.
An infant had fright, palpitations, abundant phlegm, and undigested milk in stool, indicating Wind pathogen combined with fright, Liver insulting Spleen, and Qi deficiency. First treated with Tianma Fangfeng Wan to dispel wind and calm fright; then Wuwei Yigong San to strengthen Spleen and stop convulsions, resulting in recovery.
An infant had crying convulsions, cold fingers/toes, and blue-black left cheek, indicating Spleen Earth weakness allowing Kidney Water to insult. Treated with Liujunzi Tang, Jiang, Gui for one dose, bringing immediate relief; followed by Sijunzi Tang (Four Gentlemen Decoction), Xiong (Ligusticum), Guibai (Cortex Eucommiae? Note: likely Danggui/Bai Shao contextually, but text says Guibai/Cxion), and Bu Gan San (Liver-Nourishing Powder), resulting in recovery.
An infant had fever, stiffness, limb spasms, and red left cheek, indicating Wind-Heat in Heart and Liver meridians. First treated with Chaihu Qinggan San (Bupleurum Clearing Liver Powder); then Liuwei Dihuang Wan, resulting in recovery.
An infant had fever and thirst. Using Xiehuang San (Draining Yellow Powder), heavy stool sensation, drooling, and continued desire to drain fire. I noted: Blue-white nose tip with little yellow indicates Spleen/Stomach deficiency-cold, Liver Wood insulting. Drooling indicates Spleen Qi deficiency failing to control; heavy stool indicates Spleen Qi sinking failing to ascend. Parent refused, using cooling Fright-draining agents. Result: Eyebrow/lip twitching, limb tremors. I stated: This is extreme deficiency transforming into Slow Wind. Started with Liujunzi Tang, Danggui, Muxiang, Paojiang, Goutenggou for 2 doses, no effect. Parent wanted to change medicine. I stated: Medicine potency has not yet reached. Added one slice of Fuzi (Prepared Aconite Root). Immediate relief. Removed Fuzi, continued for 2 doses, resulting in recovery.
An infant had blue inner eye color, convulsions, upward staring eyes, incessant crying. Use of Niuhuang Qingxin Wan (Calvescent Bezoar Clearing Heart Pills) worsened grinding teeth, oppression, and urinary incontinence. I reasoned: Severe Liver/Spleen deficiency. Treated with Buzhong Yiqi Tang and Liuwei Dihuang Wan, resulting in recovery.
An infant had convulsions and blinking eyes, belonging to Wind-Heat in Liver/Gallbladder meridians. First treated with Chaihu Qinggan San to clear Liver; then Liuwei Dihuang Wan to tonify Kidney, resulting in recovery.
Liangjing Wan (Cool Fright Pill): Treats fright malnutrition with heat convulsions, mental fright/palpitations, red whites of eyes, tight jaw, tidal fever with drooling, hand/foot twitching.

My Commentary: The above formula treats Wind-Heat in Heart and Liver meridians. If Heart/Liver deficiency presents with fright convulsions and tidal fever, use Mizhi Anshen Wan (Secret Instruction Calming Spirit Pill). If Liver Wood invades Spleen, use Yigong San plus Chaihu, Goutenggou. If Heart/Spleen deficiency presents with tidal fever and drooling, use Yigong San; add Muxiang for deficiency-cold. If ineffective, add Paojiang. (See details under Liuyi).
